Hi, welcome everyone. My name is max, Im 20 years old, I have severe acne scarring and Im on Isotretinoin 30mg everyday.
I want to document my journey to help other too.
This post will be about my acne scars. I want to know which treatments do I need. First I will tell you my story and then, upload images so you understand what Im talking about. English is not my first language so forgive me for any mistakes.
here it goes:
I always had smooth skin and my parents never had any acne scarring. My acne started appearing at the age of 13, I always had a bad diet, eating junk food and processed sugars. I grew up on a hot island, all year round summer plus calima sometimes (dust).
I would play sports in school and sweat a lot, I wasnt educated on the acne department in my life, so I use to always pick on my acne with dirty hands from playing with basketball or touching the floor, etc.
At 14-15 my face had a lot of acne, it was full, it started on the sides of the lips and eventually moved to the cheeks when I was 15-16. I started Isotretinoin treatment when I was 14 but didnt finish because of personal reasons. I took it for like 4-5 months only, had to take it for 1 year. Anyway, my acne was very bad at 15-16, so I decided to go to a dermatologist clinic that ny mother recommended, and what they use to do there was basically pick on my acne with medical tools and use fancy cremes, I believe acid was one of them.
I went there for like around 25 sessions until I was 18-19.
I started Isotretinoin 30 mg again 3 months ago (today is December 29, 2023), I have to take it for 1 year.
My acne is almost gone right now, but I have a lot of scars on my cheeks, more noticeable on my left cheek.
I want to point out, I dont know if it helps, I didnt have so much acne these past 2 years compared to when I was like 15. Im saying this because, this year I went from 73 kg to 88 in like 4-5 months and I think my acne scars are worse (I will upload the photos with dates wherever I can). From what I read, when you gain weight, the acne scars should be less noticeable. Anyway, I will loose the weight again in the next months back to 80ish.
I have done a little research about acne scarring and treatments but I dont find a lot of people with the same level of scarring I have. And on the streets, its the same, I dont find a lot of people that are like me, if not never.
Its making my self esteem go down these past years. I work as a waiter so I overthink it when people look at me.
I know there a lot of dermatologists that are scammers either with photoshop or with the lighting in post-photos.
Im living in New York right now, and probably for the next few years. I want to know, what doctor and treatments do I need?
I was considering Dr David Lim, but he is far, and that would mean I could not go very often, because of time, finances and inconvenience.

I have to say, I will quit again my Accutane treatment today. I knew about the side effects of dry lips, dry skin, dry eyes. But I started reading more yesterday about peoples experience, and other side effects that I read that Im starting to experience too. Hair loss, lower back stiffness. I dont want to risk it. Im stopping Accutane on December 30 2023.
@dan I talked to my dermatologist, he said its temporary. I decided to not stop taking it.
Cool. Yeah, make sure anything you do Accutane-related is in close consultation with your doc.
@dan Do you any idea what treatments would I benefit from the most and any doctors recommendations around New York?
I think your skin is probably one of those combo types where you'll most likely benefit from multiple things. If I'm just guessing I'd say maybe:
- Some TCA CROSS
- Some subcision (with suction)
- And then maybe a few sessions of either ablative fractional laser or microneedling + PRP
But it's important to find a good person and do your research on your own first too. I don't know anybody in NYC, or anywhere else for that matter. I just look at the science part.
Also, usually, they say wait 6 months after Accutane to do any scar repair, but then again, even that is controversial. Basically, take things slowly and make sure you're being careful and staying on top of everything. More than anything, always be your own advocate and stay as informed as possible.
I don't love the idea of filler personally because it can migrate and what not, so if it was me I'd do what I could without filler. But...I am honestly not an expert in real life on this stuff. I'm more of a "science" expert and have read everything about this stuff but haven't seen a ton of people do filler in real life like a lot of doctors who specialize in this have. So, I'd for sure consult with a few docs.
