i have the very same issues. Pimples that do not really pop scar over but comes back. Ive had acne and this in no way behaves like acne. Dermatologists finding with skin scrapings was drumroll....im apicker. It has been a long 3 years that I have had to deal with this. Ive been through bouts of depression and i have treated my skin as a fungal infection , acne, bacterial and now as parasitic. I am actually getting good results from taking moxidectin. It is available at feed stores. Moxidectin is like ivermectin but less toxic and stays in your system longer. The day after I took it my lesions are flatter and healing. My face is not so chicken skin . Few years back my cousin had a beautiful pitbull that unfortunately contracted a mite infection. Poor baby was not being treated aggresively and i took him home to try to eradicate the parasites. It was intense, lots of bathing, rubbing lotions to kill and repel. I never thought of it till recently that I may have been infected. Im hoping this is what it is. An answer has been desperately needed.

I know you have mentioned things but most require a prescription and my doctor is, well, less than helpful. (My last dermatologist went as followed...Sit it waiting room for 1/2 hour past appointment time...sit in exam room for 20 mins...been seen for five minutes and asked how my face is doing since its required you wear a mask and cannot take it off due to covid...great see you in a year...and this is how every appointment goes).

I have all the same issues as others have mentioned here;
Rupture of bump (at the start it presents itself on my face as deep cystic acne).
Layer of skin comes off.
And behold...the alien "white plugs". |::strongsad:
I get these mostly on my chin. A few times on my nose. And the most embarrassing and largest/deepest plug once on my private area. 

The only thing I will add is during my scabbing/healing process it will sometimes leak a clear/yellowish liquid and the sore will get a crusty honeycomb look to it. 

I waited anxiously and excitedly for my first dermatologist appointment for 7 months. I am not one to go to the doctor for every ache and pain, so this was a huge step for me. However, my excitement was quickly met with defeat. My doctor was ADAMENT I am a picker. I was not (I am now to remove the plugs, but at that time I really wasn't). She asked in quite a demeaning and condescending manor, "Why do you think its anything other than acne?" (Hello...never had these foreign plugs in my acne before) 

I have not taken antibiotics in years, I know it was mentioned that may be a partial cause or irritate this more? I have even changed my diet. 

Here are the medications I take, as well as my routinely used face products:
Spironolactone 150mg
Tretinoin (small dot 3x a week)
Clindamycin (small dot every morning) 
Ketoconazole Shampoo (used as face wash a few times a week)
And now Clotrimazole (foot antifungal, as needed during flairs....this does give me an itchy redness little rash if used too much, I has pretty sensitive skin)

I also use Cerave Hydrating Face Wash, and La Rouche Posay Mositurizer. I try not to use anything near the affected area when I have an open flare up of the plugs. 

I'm curious if we can all find a common factor?
Why do we all have this issue?
When did it start for you?
Do you find stress affects it for better or worst?
I think figuring out any common factors could help. 
Has anyone had a cyst removed or broken bone?

I've reached my breaking point again today when a coworker asked if I made out with a toilet, and lied and said it was a burn. I cannot deal with this anymore. It's been maybe 3 years dealing with this. I'm female, 29. 

Photos of my dilemma..the full face is my plug issue and rash from overusing things I believe.

Im a bit gutted as I have had quite good success using a sulphur cream, terbinafine and salicylic acid. I now dont have any lesions just a tiny dry scarred patch where the last one was BUT Im going to have to take antibiotics!!! I have a tooth abscess that is agony and Ive been prescribed antibiotics. Im so worried this is going to set me right back.

For anyone struggling though I recommend a probiotic supplement, sulfur cream (just a little as its so drying) and terbinafine (athletes foot cream) and salicylic acid.

Ive been following this thread for years and have tried every mentioned treatment but still get the plugs in one placemy nose. Its horrifying and embarrassing. My confidence and social life have plummeted. Sometimes I break down and cry. I know it sounds pathetic, but theres no way to describe how tiresome this condition is,especially when its on your face. Sometimes you just gotta have a good cry.

 

I use an AHA face wash and salicylic acid, whichseemto keep the rest of my face reasonable. But thelesionsstill are popping upon the tip of mynose.

So in addition to the AHA/BHA combo, Ijust started using chlorhexidine/ketoconazole wipes (they make these wipes for pets but are safe for human use). Chlorhexidine seems promising as an antifungal and antibacterial with minimal resistant microbes.

Im wondering if this is maybe a fungal *and* bacterial condition? Maybe with fungusas a root cause and bacteria as a secondary/opportunistic invader? Ive been on tons of antibiotics and even a round of an antifungal (Itraconazole). With the antibiotics there was no clearance (actually got worse in some ways), and w/the antifungal there was some improvement, butwhen the spots burst open and ooze yellow + form yellow crusts,it makes me think theres a bacterial component. Especially because there wasnt complete clearance with the oral antifungal.

So Im curious about what will happen now that Ive incorporated a potent antifungal/antibacterial into my routine. Ithink this starts deep down in the follicle. A gentle AHA/BHA or some other type of keratolytic agent is essential to exfoliate the pores and prevent clogs/oils from creating the perfect habitat for a moisture-loving fungus already naturally present on the skin. But you cant overdo the exfoliation because the fungus seems towreakhavoc when the moisture barrier isdry/irritated. Benzoyl peroxide is way too harsh IMO. Sulfur might be an option.

Next is curtailing the population of fungus/bacteria that are growing out of control and causing further inflammation and dilation of the pores/hyperkeratinization/plugs. Really hoping the chlorhex/ket combo can get the SOBs in line.Im gonna give it 3-4 weeks and report back.

If anyone has any info on how to get rid of existing plugs, that would be much appreciated.Ive found that once a plug is a certain size/hardness, no topical will get rid of it. I tried for *months* (almost 1 year) on certain spots, and in the end I picked at it to remove the plug,which really upsets me as I hate the idea that Im damaging my skin...but just want relief. Theres like an instant decrease in pain/redness when the white plug comes out, but obviously in its place is a lovely excoriation/wound (that sometimes starts oozing yellow/orangebacteria/impetigo?).

We need answers. I see people describing this condition all over the internet. Shoobermy wounds look exactly like yours.

Also, go to an infectious disease doc!Do not go to a derm. You will get labeled a picker and sent home with a tube of mupirocin that will make it worse.

Shoober mine is identical to yours, only thing that kinda works is very strong steroid cream, I also have reynaulds phenomenon, another vascular/ discoid lupus type of symptom.

I was prescribed about a year of antifungals, strong oral ones as well prescription antifungal topicals. My skin was the worst then, its not fungal.My biopsy was inconclusive but consistent with lichen planus/ which is very similar to discoid lupus. Clinical features hard to differentiate.

unless its mycobacterium or acintomyces or even Lyme disease related.

my lesions heal with a dark border and light center and slightly pitted. Exactly how discoid lupus does. But iv never seen a more identicalphoto to my lesions as yours shoober. 4 out five derns said connective tissue skin disorder. Oh and discoid lupus lesions have plugs like that. I know its baffling and sad Docs cant even figure out with biopsy.

let me know any update on anything you figure out shoober, thanks for sharing your photos!two things also help, are-aloe from aloe plant- has to be real Aloe and I wash with 3% salicylic. I think its immune response to any trauma on skin.

I havestruggled with this skin problem for over 10 years now. I have been following this thread and tried everything. A few times I have thought I had it figured out but it comes back. I too have considered discoid lupus or some kind of viral wart condition or lymes or leishmaniasis.

I have only brought this up with one new doc who asked me if I use meth. I don't and never have. Thank you to all who are sharing their doctors visits. I feel too emotionally devastated by this to bring it up again. The countless testimonials of misdiagnoses and victimization give me little hope.

I am 58, have diabetes which is autoimmune. I drink raw goats milk kefir and kombucha daily which give me lots of health benefits but no help with the lesions and white plugs, so I'm not sure probiotics are the answer. I continue to search through diseased skin images trying to figure this out. Off to get an aloe plant.

Plugpurgatory, I think that my wounds have had bouts of fungal and bacterial infections at times, but as secondary issues and clearing those infections has not stopped the plugs. "Healed" areas have skin that sloughs off and reopens. Then the oozing, the pain and itching.

The more I'm thinking about it and the more reading the more sense Discoid Lupus is making for myself. Between symptomsand photos it's checking off a lot of boxes. I also have an older brother with several health issues. He doesn't get thesesores and plugs (as far as I know) but as he's aged he's had many undiagnosed issues. Started having seizures, odd skin issues, sores in mouth...and that's just the start of what I'm aware of.I'm trying to find a doctor that will listen to me, run a damn test, and take me seriously. I will keep updating with any news or progress .

I recently had the mouth sores and switched tooth paste and mouth wash, situation improved.How ironic shoober,my brother went to derm yesterday and doctor suggestedhumira.He Also has no precise diagnoses. We both (my brother and I)have sensitivity to gluten. We carry the celiac gene HLA-DQ. Both tested negative for celiac though, through blood work. I have high inflammation/ ANA. Also diabetes mellitus and Celiac are the same genetic antigen group I believe.I notice if I havegluten doesnt help my skin situation.

Elbee, Were you ever tested for Lyme? The aloe makes the lesions heal a bitfaster. Also dries them out.

Doctors should never be denying tests,they dont know whats wrong until they test, Like a lot of insurances wont cover Lyme test or others,they only cover the basics.

I submitted my case to CrowdMed,Crowd-Sourced Medical Diagnoses. I'm not sure if cases are actually resolved through this site, but its doing me no harm to submit my issue. I think the more people that submit a case, the better recognition and chance it will be solved. Takes about 30 mins give or take.

I apologize for the aggressive posting. I need to figure this out. I need a remedy to this issue. I have spent 8 hours researching today. Not even exaggerating, 8 freaking hours here. I have a virtual appoint Monday (major eyeroll) but I'm demanding a punch biopsy. I have an ongoing list of "maybe" diagnosis...

Please someone look up Basal Cell Carcinoma and tell me if you can relate to any of the symptoms.
Or Squamous Cell Carcinoma/ Actinic Keratosis....

EDITED TO ADD: PAY ATTENTION TO NUMBER THREE & FOUR!!
(3) a nodule (the plugs?!)

(4) crusted indentation in the center that may develop tiny surface blood vessels over time (When the plugs are pulled they bleed profusely but breifly!!)

  1. An open sore that does not heal, and may bleed, ooze or crust. The sore might persist for weeks, or appear to heal and then come back.
  2. A reddish patch or irritated area, on the face, chest, shoulder, arm or leg that may crust, itch, hurt or cause no discomfort.
  3. A shiny bump or nodulethat is pearly or clear, pink, red or white. The bump can also be tan, black or brown, especially in dark-skinned people, and can be mistaken for a normal mole.
  4. A small pink growthwith a slightly raised, rolled edge and a crusted indentation in the center that may develop tiny surface blood vessels over time.
  5. A scar-like areathat is flat white, yellow or waxy in color. The skin appears shiny and taut, often with poorly defined borders. This warning sign may indicate an invasive BCC.

Shoober, when I initially went to derm, he said he has seen Bcc and SCC that looked like what I had on my skin. He biopsiedand it came negative for cancer and negative for fungus. Its almost does look viral but I think some kind of immune response to any trauma done to skin (Khoebner phenomenon). They will start kinda like a giant underground zit, that never pops then when finally doesnever heals, then turns keratosis/ plug and plaque like. Lucky me haha. This started several years ago, before that I had close to perfect skin. They always prescribe stronger and stronger steroid ointment. Minus the one doc who had me on a year of diflucan and sporonax pulse pack that did little to none, I had a TOE test which was Mildly positive, he said its psoriasis from a yeast allergy. I went to so many derns with different test and theories and never a cure. Its very frustrating.

there was a post early in this thread of a person who used cancer type of cream then the plugs developed. Those photos also kinda look similar to what weird plug things come from my skin. Please continue to update, I appreciate you sharing your experience, feel like potentially we can learn andhopefully improve.

Jacklyn86 & Shoober,

I too obsess with looking for a diagnosis. I have so much emotional devastation over this condition that I choke up when trying to explain it to someone. Your posts are helping me see how hard it is to tease out a clear problem statement and how helpful getting a clear concise description of the sequence of how the wounds happen will be.

I feel guilt because I do pick at the plugs. I try so hard not to, but the longer they stay there, the more pain & itching they create. When there are lots of them, the wound throbs and is distracting. Once I pluck out the white plugs, things calm down and fairly quickly my skintries to heal.

Q1 - anyone else having that sequence or is the inflammation stage my own secondary issue?

Q2 - if any previous people who previously have posted that they were diagnosed and received treatment are still reading this thread, would you please say if your issues have remained resolved or not?

I keep circling the basal cell carcinoma images. Also a previous poster gave caution to us all that he had had bcc and our images looked close. Seems like that would be something all these docs mentioned in above posts would have recognized.

Q3 - So what are the white plugs? Keratin?

Apple cider vinegar that's ll u need and there gone have had this problem for the past year and felt so self conscious now I feel better hen ever before I no longer have this problem hope this helps and it's cheap

This is interesting.. could we all have a severe vitamin deficiency? I know mine started after a strict calorie diet back in 2015.Ive been following the regiment that is listed in the article after researching phrynoderma or toad skin.I also use Lotmotrin Ultra on my spots. This is specifically Butenafine Hydrochloride and seems to work better than the other topical antifungals. Ive seen improvement and Ive been taking the vitamins for about a week. Not cured but there is definitely a change in whatever is going on with my skin.

Interesting Read Dandyda, I read about essential fatty acid deficiency, started taking fish oil, evening primrose, I do think that helps my skin and hair. Was making me kinda tired though. So I stopped. What supplements did you start taking? -Thanks for sharing.

I also did purchased a beta carotene and vitamin E, beta carotene is like accutane in large dose, good for skin but will shred your stomach even in a small doses , I couldnt take. Vita E, havent taken but It on the dresser.

I actually followed what they recommended in the article with varying dosages because I couldnt find exactly what they recommend. I should point out that they took nicotinamide (Linked below) While researching, I found a Joe Rogan episode that discusses how nicotinamide (NAD) is essential to slow down aging. You should listen! Its episode #1234 with David Sinclair.

Overall it was an expensive trip to Vitamin Shoppe! Approximately $120 but I was convinced I had nothing to lose as Im labeled a picker with every doctor visit. I noticeably see these things are drying up rather than just crusting over. Best of luck! I can attach images of the bottles if anyone was interested.

Im taking:

A&D combined (50% beta carotene and 50% retinyl palmitate) x5 at 10000 iud

* Only for a month then the dose drops significantly. This can be dangerous because the body wont expel excess so consult a doctor!

B-12 (as Methylcobalamin) x1 at 2500 mcg

C, x1 at1000 mg

I took a listen to David Sinclair, interesting stuff, I intermittent fast actually and take Reverstrol. Any side effect from the A+D beta combo? Also what brand, maybe I will try a different brand. The one I got was Solgar,from vitamin shop Dry Beta 3,000 mcg. I take Zinc too but when I do I sleep like easy ten hours. Which is great because Im not a great sleeper.I do think a lot of whats happening with My skin is internal, I appreciate this approach and think very valid info, thanks!

oh the NAD this is a new one you brought to my attention, any noticeable side effects from that, Im gonna purchase that one too. I saw the amazon link, do they sell that brand at Vitaman shop or only online?

Ok Im back Im still using the products from the ordinary I took a break to try differen gel. It seems to be working the best so far. As most of you know Ive struggled with this for over 10 years. If you try differin gel make sure you moisturizealso I use aqua pore since its non comogenic. Try it guys its truly helping me. Im still scarred but once this problem is solved Ill work on that. Hope this helps one of you.

I forgot to add differin gel is over the counter now no rx required.I buy mine off of amazon now and it slowly removes the build up of dead skin and dries out the plugs.

I wanted to chime in over here after following along for years. I finally found a dermatologist that is determined to find an answer and a resolution. I had a punch biopsy done today, as BCC was a possible concern. She was able to rule out fungal/bacterial/worm/yeast etc. She left an open line for me to communicate back and forth with her while we wait for my results. Im relieved. Finally a derm who let me talk, tell my story, didnt interrupt, never mentioned picker, looked through all of my photos, even read along with some of your posts! She said she (obviously) cant diagnose any of you guys on the thread for legal reasons. But shes on board and in it for the long haul. She did a punch biopsy on a spot that was about two weeks old and one on a spot that was under 48hrs old. Shes going to have them tested for everything under the sun. She had also suggested the possibility of her teaming up withan Endocrinologist, Immunologist,and a Rheumatologist after we get the results from the biopsies. Something set off a lightbulb for her while she was looking through the pictures andhearing my story. Theres a possibility thatsomething internal hasgone haywire since I went fromzero acne to BAM....unknown,deep, long lasting, scarring, dont know what this actually is, 3 year battle. That made her raise a brow for sure and brought up the topic of internal issues. Shes researching on her end, Im researching on my end. Were staying in communication with each other. But shes not giving up on this and said that even if this leads to it being handled by a different doctor, shes standing by me the whole way.

Ill be sure to update after theappointment for the biopsy results (Sep 18th). I never thought about this being something other than skin related, but maybe it is a glimmer of hope for some of us that the problem may actually be coming from something off balance inside. Definitely makes me wonder. And after a quick google search (I always double check a doctors statements nowsince this skin issue started) she may be onto something. Im curious, have any of you seen animmunologist/rheumatologist/endocrinologist for this???

Iv seen, rheumatologist, allergist, gastro, general md. Id like to see endocrine Im sensitive to gluten.

May I ask how she ruled out fungus/ bacteria or yeast before having biopsy results? Im curious,Wood lamp?Please share results of your biopsy and any updates, Thank you

After trying chlorhexidine, I think the bacterial/fungal issue is probably secondary to what's going on as others have said. It helped the yellow scabbing but didn't get rid of the deep white plugs.I feel like the root cause is something to do with the sebaceous glands, like they've just become inflamed/clogged somehow. I also get a thick build up of dry dead skin where these spots are. I only get them on my nose, which is also the only spot I've gotten repeated sunburns over the years. I recently switched over to twice daily sea salt soaks, followed by salicylic acid and azelaic acid. I don't think this will 'cure' whatever it is but at least keep it less red/inflamed and prevent infection. I'm interested in Differin buthave a doctor's appointment for early October with a complex medical dermatologist, soI'm going to let the spots marinate until then and see about a biopsy.

 

I was wondering if any women in this thread haveexperience with spironolactone. It's a diuretic and androgen inhibitor that some people take for acne. The sebaceous glands can go crazy in the presence of certain androgens, causing inflamed/overgrown glands that appear as a pimple while attracting bacteria, fungus, oil, etc. So maybe it's a hormonal problem?

Ive been on ahigh dosespironolactone for a year. It has helped my skin in general, but not this issue whatsoever.

I had an appointment with a new dermatologist yesterday.The good...I received a diagnosis!I wasnt scolded and told its just acne. Leaving the appointment however I was equally as disappointed as I was relieved. Relieved because if it has a name, a diagnosis, it has a remedy to cure - or at the least ease this issue. Disappointed because Im not entirely sold on the diagnosis. Disappointed because why didnt my first derm (or any other doctor Ive seen for this matter) recognize and diagnosis this sooner? For that matter, why have so many of us not had an answer sooner if the diagnosis was correct? These questions are part of the reason that makes me question the diagnosis. But I am not writing it off. This was on my list (but not high) of suggestions of what I thought it could be that I discussed with primary care physician. She was proactive about prescribing something as there was no harm if it were wrong. I am on day 5of a 7 day high dosage antiviral, and will update at the end of this prescription. Subsequently due to the diagnosis after I finish this high dosage script I will continue taking this antiviral at a smaller MG for all of my eternity.Im defeated, burnt out but Im keeping the faith. I hope theyre right, but it feels wrong.

Hello I am new here :). and Thank you for all of your posts on this topic.

I have the same issue with original poster.  Something in the base of hair follicle gets inflamed. It does dry but does not heal for a very long time unless I open it and pick the white plug. Then it forms again with many smaller plugs sometimes ... Sometimes it gets healed right away. Tissue feels so hard when healing ...

What worked so far : 
If I eat limited amount of olive oil and restrain from animal fats everything goes well. I managed it for 10 years or so with 1 issue in 3 month or so but by picking them I have 20 or more holes in my face. Small holes nevertheless but it certainly does not get inflamed. 

What worsens it: 

Whenever I eat animal fat, especially mozeralla or cheddar etc on pizza or fatty red meat or fried things (including with olive oil) BAM they come back. 

So, I have only guesses :

- It must be something keratin based , it is a plug or spike in shape for sure, it may be stem cells because they are repopulating fast after unskilled ejection of sample (pick) . If there is a hole in there they are filling it. 

- I have hyperkeratosis on my legs, and I have my entire DNA tested. I am almost always vitamin A deficient and my skin is too dry.  In my DNA test it shows I can not convert vitamin A from carrots and other carotene containing vegetables. (My grandfather is from Greece KAVALA - they drunk lots of alcohol with animal liver at seaside unfortunately and I dont like meat especially internal organs such as liver) 

- When I was 14, my mother and I visited a cancer patient who was obese their home had no sunlight at all. The patient (65 years old lady) and her doughter (40 years old another lady) both died from cancer in 5 years. I believe I first got it from that home. I was very tired and lying down on their couch and then got sweaty and scratched my groin when I was in wc. Right after that I had lots of bumps on my groin with round versions of these plugs ... I ve picked one put it informaldehyde sent it toa  lab. (mom is histology professor I ve got it from her and send it to her friends pathology lab) The result was residue of normal epidermal cells... After examining bumps with naked eye, people in that pathology lab who are also family friends * decided it is some form of syringoma. The things on my face first appeared 10 years after when I was consuming too much carbohydrates and animal fats when I was 25 years old.  Then I started to eat yogic diet with IF one meal a day and they disappeared for a very long time (12 years).  Last 2 years I needed fuel badly due to hard mental work... I also started to use stimulants - Black tea and smoking when I add animal fat and cheddar plugs - spikes come back. I am 39 years old. Still consuming one meal a day, no garlic no onion no spice no alcohol only smoking and black tea ... I need it badly, but cant add fatty red meat or fatty fish to my diet ... 

*father and mother have multiple degrees : dentistry pathology histology 

After reading this post carefully I decided to try two things : 

1) Experiment : Get a plug. Heat butter to 31 degree of Celsius (skin temperature), put the plug in... See if it grows ???
2) Mix olive oil and  tea tree oil 5 drops for 20 ml olive oil. Rub it on problematic areas... If the repair is done and spike is left without purpose, olive oil may help skin to eject it. If it is candida or malassezia tea tree oil may kill it. If it is keratonegic stem cells, oiling the skin after hydrating increases skin repair 2.5 folds according to Japanese scientists they recommend putting vaseline on skin. Tried that on some burn sores (half with vaseline half with antibiotic) the one which is cleaned with soap and treated with vaseline healed indeed 2.5 times faster ...

I will let you know after 2-3 weeks of rubbing olive oil + tea tree oil and eating animal fats and fries

PS: (Some additional info)
- 3 different sports professionally till 17 years old ...(butterfly swimming , kickbox , basketball) 
- I've got sick only 3 times in last 15 years no flues or influenzas (mers - nCov19 - zona )
- smoked between 22 and 32 years old 
- I ve got loss hair in irregular patches (26 years old) started to shave my head. I am very happy with it , because of instant drying and easy cleaning and no hair cut saloons :) 
- I have eczema but does not flare it is under control unless I eat lots of wheat or spicy foods or lots of lemonade. 
- Got some hpv warts because all family and a skin specialist forced me to use topical corticosteroids for excema once. They sit there for 3 years ... 
- When serious skin injury (pressure cookware blew up to me and got serious burns) , oddly my skin heals very fast and you cannot differentiate from new ... ! But not that spiky bumps ...
