Greetings, I am writing from Turkey, I will write using translation, I hope I can express myself. As you can see, I have been suffering from acne scars for as long as I can remember. I started my first treatment in January 2024. After 4 months of treatment failure and long research, I told my doctor (he is a dermatologist) that I wanted to have surgery. I had 4 sessions of subcision in total. In each session, he made an incision with a cannula and tied those ligaments. I could hear it breaking. In the 4th session, he used the so-called smart filling and crystal filling along with subcision. His name is CaHa. I don't see any improvement in the brand (Facetem S). I told this to my doctor and he said I should wait. For a week after the procedure, my face looks swollen and the scars seem to have disappeared. Then my face looks like a balloon and deflates. I don't know what I should do next.
These photos show what my skin looked like 5-6 years ago. As a result of the treatments I did at home (TCA peeling, TCA cross, microneedling), my skin became what it is today. I think the shape of my acne scars has changed. While doing the TCA crossover I accidentally widened the scar and turned it into a rolling scar. I think my current scars are rolling? or freight wagon. I don't know, I don't recognize my wounds. All I know is that my face is swollen, that is, edematous, and when I lift my face, my scars disappear. How will I explain this to my doctor?
Your scars look rolling to me more or less.
At this point, I'm betting just lots of sessions of ablative fractional laser with PRP or microneedling with PRP may still give you results, but your skin will never be perfect. That's just how scarring goes. So you can get improvement until you feel you are where you need to be. But you're right, it's expensive.
Also, make absolutely sure to prevent more acne as you do this. Take the quiz for more on that.
I'm not as much of an expert on fillers as I am with more traditional treatments. However, the one thing I do notice when I look into fillers is that you can use your own fat as filler and that seems like a good idea to me. I get nervous when man-made fillers get introduced, particularly long-term fillers which can migrate over time and cause issues.
I used bellafillers and it's a miracle. However, you need to be super cautious and NEVER OVERDO fillers! You can see my experience here. I think filler will help you as your scars are not that deep.
