3 months post dermabrasion. Ive been avoiding sunlight but haven't been wearing sunscreen like I'm suppose to. A little underwhelming

Sorry about that. Have you done consistent subcisions + good HA fillers for the cheeks or suction each 3 months for 1-2 years? These seem like it'd help you the best fill these craters...

If you recall earlier I said you have below the skin problem not much from top. You have to keep on filling from the bottom or your scars are still tethered .

Wasn't working for her, as far as I recall, she had little to no success with subcision/fillers. I agree that's the only thing I can think of to tackle underneath atrophy. Dermabrasion is always best for icepicks with healthy skin in between but also isn't perfect. It leaves tenuous scar tissue as with anything that's extremely sensitive to practically everything. The good result from Dermabrasion is mainly top layer tissue heals amazingly even if it's flawed (like hyperpigmented or tenuous) but atrophy deals solely with tissue and material missing in the middle.

What could dirigo do at this point to get legitimate progress on the gaps/atrophy? Is fat filler an option to generate volume or are shifting risks too risky here? Sculptra again? HA every 3 months for 1-2 years?

What does this mean? Get more filler?

I got a consult with dr. emil. He didn't seem confident in his taylor sub skills and leaned more doing multiple cannula, which i am not interestedin. In the name of being ethical, he disclosed that a recent patient had 3 hematoma after he did a taylor sub for him. And that some of his patients had prolonged swelling that made their face look rounder.

It's been 13 months since my surgical Dermabrasion and I can say it works. It looked better as it healed.

 

Since then I've also done 3 rf microneedling with the secret pro machine (don't recommend lutronic) . But the dermabrasion is what worked the most.

Id say the poster who said softening the scar edges is key was correct. Unlike harmless boy who said he saw results right away...I saw them gradually as the redness went away. The redness took the whole 12 months to go away.

I may get this done after my procedure with Rullan.

He speaks English perfectly. Hes in his 60s and his dad is in his 80s I think. The bandages naturally fell off after 9 days.

I would get a hotel in San Diego and drive to Tijuana or get an Uber then go back to San Diego. Tijuana can be sketchy.

3100 usd but try talking to him in Spanish maybe you'll get something lower. I opted for no sedation (only numbing shots) and it should've been cheaper to be honest but he wouldn't budge. If you get sedation someone needs to be with you.

I'm getting my 2nd in Thailand since I'll be vacationing there. Hopefully it turns out just as good.

Some updates....

In 2024..the doctor in thailand said something interesting...it pretty much echoed what harmless boy said. He said that a 2nd dermabrasion is not what he recommends for me because i had rolling scars and those only worked for boxcars. He recommended co2 laser for mine. I told him I wanted a 2nd dermabrasion anyway and he went way too superficial to me....i didn't even scab. I should've done a laser treatment with him instead.

It's very hard to say what helped me the most because but here is the summary of what i've done...

1) 5 subcisions- i recommend dr. rullan for this because the 2 i had done by other doctors were not effective. the technique matters and the one in san diego (it was a younger doc i forgot his name, go back for his name, made it worse)

2) 5 rf microneedling- the korean brand rf Lutronic is TRASH and makes your skin sag. Secret RF by Cutera is better. The science behind this is new and iffy though.

3) 3 sculptra- i don't know if these helped or not...and the science behind it is also iffy and new also because some people are saying it doesn't produce collagen..it just produces scar tissue underneath.

4) 4 lasers- 2 were light fractionated co2, and 2 were erbium. I like the co2 more than erbium. One physician assistant described erbium as softening the edges in a "top to bottom" method while the co2 being "bottom to top" which forces the skin underneath to push out new skin.

5) 2 dermabrasion- one of them was light and one of them was moderate..apparently these don't help my shallow rolling types but it didn't do any harm either.

 

Future treatments...i'm going to 1 strong co2 (ultrapulse) to see how it goes. i only have texture issues now... they look like big pores now.

Good to hear from you @dirigo39 I did ultrapulse co2 nearly 9 months ago with about 10% - 15% improvement. Would you still recommend dermabrasion amongst most other treatments for those dealing with moderate to severe boxcar scarring and icepicks? I am deciding between another CO2 resurfacing full ablative and going to TJ to get the same treatment as you.

Also do you have any recent pictures of your skin for comparison?
