Acne.org’s Real World Take
“Spiro” has been shown to work in a real and noticeable way most of the time. However, messing with hormones is messing with hormones and it’s best to proceed with caution.

Broke me out - Not just an "IB"

I went to my dermatologist for something non-acne related and she noticed I had 1-2 pimples. She asked me if this was typical for me. I told her that I usually only get 1-2 pimples around my cycle and she told me that spironolactone would be perfect for me - that it would clear the hormonal "acne" I was suffering from during my cycle. I don't usually jump on the pharmaceutical bandwagon, but I was getting annoyed at the monthly pimples so I figured I'd give it a shot. I had heard a lot of good things about spiro, and I even have a good friend who has benefited from taking it.

The first two weeks were great. After that, my entire face was breaking out in all kinds of ways. Deep, cystic pimples, whiteheads, blackheads, etc etc. I had acne in places I've never had acne before. It was awful and lasted weeks, if not months after I stopped using the spiro. I had persistent flesh-colored non-poppable bumps covering my entire forehead for a long time after. I also blame the spiro for a few other medical things that happened to me after that. Overall, really regretted trying this. Glad it works for other people, just didn't work for me.

Effected my vision

This product caused effected my sex drive increased tremendously which wasn't terrible lol but the blurred vision is why I had to discontinue using the product. I used it long enough to see minimal results but my eyes became very sensitive to light and my vision became blurry.

Help!
Hello. I am ending week 3 using this drug. 50 mg in morning, 50mg at night. It seems as if my acne is getting worse and worse. I was told that is supposed to happen but it is causing me pain and swelling of the acne and crying every day. I am also using retin a micro gel with it 3-4 times a week. Did anybody else use that while prescribed to Spiro? I have always had acne but it has never been this bad. To the point where my pimple is as big as a boil... So painful and ugly. I want to know that I'm not alone and if I should stick with this routine. Thank you.

Could anyone answer this? Will this help me?
I am a teen and I have had cystic breakouts on my forehead for about three years now, and just recently I experienced one cystic pimple every now and then on my cheeks. A couple years ago I was prescribed with tretinoin cream and that has helped with my marks and hyperpigmentation but not the actual cysts. I just went to my dermatologist today and she prescribed me with Spironolactone 50mg and I will soon check in with her in about 2 weeks so I can go up to 100mg. Before I start taking it, is there anything I should know about it? I am going into high school and play a lot of sports so I do not want to have to experience any joint or muscle pain. Can Spironolactone help with my cystic breakouts on the forehead?

Did Not Work
This product did not work for me, and I was on it for roughly 4+ months. I have moderate acne and oily skin.

It's for serious, cystic, hormonal acne not moderate so maybe that's why? If you have moderate do you try benzoil p at all?

Spironolactone - Heart palpitations, panic attacks, chest pain, anxiety
I would give this product 0 stars if I could. I have never written a product review in my entire life. I'm a 22 year old, perfectly healthy female. I exercise regularly, I eat a well balanced diet and have suffered from moderate acne for the majority of my teens/early 20s. I am completely mentally stable, I don't do drugs, have great friends / job / self esteem etc. After coming off the pill and roaccutane my acne started to re appear and so my dermatologist reccommended spiro. It worked alright for the first 3 months until I started getting horrible horrible heart palpitations which lead to 10 panic attacks in 2 weeks. I HAVE NEVER HAD ANXIETY OR A PANIC ATTACK IN MY WHOLE LIFE. The worst was when 4 in a row in 2 hours and had to be taken to the emergency room. Everyday for those 2 weeks I was constantly experiencing chest pain and heart palpitations, constantly fighting off another panic attack with deep breathing etc. I had a million theories why it might be happening, but the doctors assured me it wouldn't have been aldactone! Only after my GP and I did some research (I'm a psych student studying psychiobiology), and I went off spiro, did I return completely back to normal. PLEASE NOTE: this drugs HAS NOT been TESTED systematically for ACNE. It is and OFF LABEL DRUG. Be careful what you take, it may not have the same effects, but be goddam careful.

Depression new low/suicidal thoughts (TRIGGER WARNING)
I'm 19 years old and have been dealing with acne of all sorts since I was 12. Different things have worked for me at different times for the most part (epiduo at age 12-14, ortho tri cyclen at 15) but since graduating high school about 2 years ago, I've had this acne that I have not been able to get to go away no matter what I try. About a month ago, I went to the dermatologist and ended up being prescribed spironolactone and cefadroxil. The cefadroxil was supposed to be for the period of time that it takes for the spiro to "kick in" but I ended up discontinuing it due to side effects and seeing absolutely no difference in my acne. I was prescribed these because I expressed my concern with taking hormonal birth control considering I had extreme depression while on ortho tri cyclen and when I had an IUD. I also was not ready to go for Accutane (and unfortunately never will be). So, with no warnings from the dermatologist, I was feeling hopeful about the spironolactone. A few days ago, I had an intense dissociative/depressive episode where I had to be physically held back by my boyfriend for 1-2 hours because I was trying to leave his house so I could go to mine and take my life. I did not feel like myself, and certain details from the night seem to be very fuzzy in my memory. All I can seem to remember is seeing an image of a pill bottle (oxycontin) in my mind for at least 20 minutes straight and feeling as though it was calling my name, telling me to come take all of the pills. I also remember I was extremely angry at my boyfriend for trying to stop me, and was trying to push past him as he tried to talk me out of it. I should note that NOTHING like this has EVER happened to me before. I've definitely been in a funk the past month while on this medication, but I wrote it off because I suffer from mild depression anyways. My mom and I were surprised and angered to find that there are a plethora of hits when you search spiro and depression. TL;DR: Spironolactone increased my depression to an extremely dangerous point, and in the mean time didn't even work on my acne. NOT worth it at ALL.

Horrible Anxiety
I read several great reviews on here about Spiro and was convinced it was the answer to my hormonal acne problems. After a month I didn't see much of a difference but didn't feel any negative side effects either so I asked to double my dosage to 50 mg a day. Within 5 days I started having chest pains and terrible anxiety. I had a full blown anxiety attack and even after going back down to 25 mg a day the anxiety didn't go away and my skin continued to break out. I've been on it for two and a half months and have decided to quit due to emotional health. I even had to go on anxiety meds. I'd rather go back on birth control than continue with this. If you have emotional reactions to this, don't let your doctor try to convince you it's not from the Spiro, it very well might be and you should discontinue right away.

I'm about 30 years old and this drug makes me feel 70
The WORST medical decision I ever made. After the first two months of the 'initial break out' that only some doctors warn you about, I decided I couldn't take the other side effects. It was after getting off of the drug, however, that creates my biggest concern/complaint/warning for you. For the first time in my life, my hairline started to recede, and I developed more facial hair. I know it seems counterintuitive, but I asked my dermatologist about it, and he said it could be an effect of this drug. If you use this medicine, I strongly encourage you to read the potential side effects before you start the regimen. Especially considering that I was only on it for a couple of months and had such a negative response even after getting off of the drug, I feel it is necessary to warn other people.

Finding a Doctor

The right dermatologist can make a big difference to your patient experience and the success of your acne treatment plan. Here are the steps to find a dermatologist who is a good fit:

  1. Use the search feature on the American Academy of Dermatology website to look for board-certified dermatologists in your area, and filter the search results for doctors with a special interest in treating acne.
  2. Read online patient reviews of any dermatologists you are considering and ask people you know whether they have any experience with these dermatologists.
  3. Do your research and go to your first appointment with questions prepared.
  4. Listen to what your gut feeling tells you once you see a dermatologist in person. If you are not completely comfortable, try a different dermatologist.

Finding a Doctor

Only a select few plastic surgeons specialize in acne scar revision surgery. Be certain to find a provider who specializes in acne scar repair and who is passionate and experienced in this area.

Be sure to:

  • Look at before and after photos, the more the better, especially patients with similar scarring to your own.
  • Be realistic about results. Look for improvement, not a cure.

Questions to ask a potential scar revision specialist:

  • Are you board certified? Be certain that they are board certified.
  • How long have you been performing these procedures? Normally, the more experience the better, however, some younger surgeons may be more on top of the latest procedures.
  • Can I speak to some of your other patients? Ask for references for several patients who had similar scarring and speak to them about the process and their satisfaction with results.

Red flags:

  • Their story changes: As you discuss different treatment options, if they tend to change their mind easily, or agree with whatever you say, consider this a red flag. A confident, experienced surgeon will possess strong, unwavering opinions.
  • Your gut tells you "no": Trust your gut. If you just don't feel that the doctor is the right fit, trust that and move on. On the other hand, if you feel they are the perfect specialist for you, trust that feeling.
