Orange peel texture - chemical peel/Laser?

Hi, 

I had a ablative C02 laser for acne scars around my cheeks and temples which didn't do anything to improve the acne scars and I think made the overall texture of my skin worse around where the scars were and  the skin in-between. the overall skin where it was normal now has a very large pore/orange peel type look.

after waiting a while for my skin to heal I saw a different dermotoglist and have been having subcsion with scultra for the scars. This has improved a lot the area where I had pitted scars but i'm still left with this large pore, orange peel skin type look that actually looks worse than just having the scars did as i feel its over a larger area. 

My new derm has suggested after my 3rd subcsion to treat the texture to do a different laser - but if thats whats caused this texture problem in the first place I don't see how that will help? although Ive read lasers like clean and clear help with texture? 

what would someones advice be for smoothing all the surface of the skin over the large area, not actual large acne scars, would a chemical peel help to smooth this out? or do I do a different lighter type of laser? 

I also stopped using any Retin -A or acid toners - as they didn't help with any texture and people have said a broken skin barrier can make pores look worse.

any help please?

Glycolic acid is a mild chemical peel that can help with enlarged pores. Minimum of three applications, spaced out 6 weeks.

have triedGlycolic acid - and itdidn't do anything, I think as my skin changed from a laser the texture is something else than pores and more perminate - almost like damged skin -

have seen a few posts about this skin textureposted after laserbut no-one seems to follow up if they managed to fix it

When did you have the laser? If it was less than 6 months ago I'd stay away from other teatments and let the skin heal. Ablative CO2 is very very invasive. Your bad texture can come from irritation of the skin due to zoo many procedures too soon.

You can try to nourish the skin not just ablate or peel it. Often times that helps a lot. Mesotherapy, PRP, noutishing vitamin masks/ creams.I am very happy with a vitamin power recovery mask right now.

If you want peelings lactic peels are very safe and effective, so around 40-60%. Glycolic too but they are a bit more harsch. If you feel comfortable and do your research you can do them yourself. Otherwise dr's office.

And yes, unfortunately a lot of people have negative experience with lasers and regret having themincluding me.

Oh for pores good dermatologists recommend salicylic acid peels.I cannot give you a personal account I haven't tried itout yet, but you can research it

In that case, I agree with the above poster. You should give your skin a break and let it rebuild the acid mantle.

Hi Mark. The same thing is happeningto me after CO2 laser. It's getting worse every day. I'm researching my options, talking to many dermatologists, but no answers. Did you find out anything? Did it get any better? See my full story bellow.

  • I had a CO2 laser treatment on my upper face 7 weeks ago. I have a very oily skin with enlarged pores. Aim was to improve texture and tighten pores. It was a CO2RE deep mode fractional with a 60-62 jules, 5% setting.
  • It healed ok after a week.
  • 10 days post-op I started back BHA 2% in the evening that I used before.
  • 4 weeks post-op I used tretinoin 0.05%. I used it for 3 nights, then stopped because of shedding and irritation.
  • 5 weeks post-op I noticed deterioration and it has been getting worse since.
  • It's now 7 weeks post-op and my skin has an unnatural orange peel texture. It's shiny, waxy and sticky to the touch. The pores are much larger than before, more of the visible and forming lines. The skin feels tight on my face like I'm wearing a mask.
  • A lot of people online are reporting similar symptoms after laser, or with relation to retinoids, with no resolution (google "co2 laser" or "tretinoin" + orange peel").

Does that sound like similar to your situation? I would really appreciate if we could connect and research together. I'm reaching out to the best dermatologists around the world. Money is not an issue.

I've seen somewhere that a fractionated CO2 laser can smooth out the grid marks and / or texture issues caused by a CO2 laser. What you can do is have a specialist perform a test area... maybe 1" x 1" in diameter on the area of your forehead to see if it solves the issue. If it works, then do the entire area(s). You may need to do it 3 times for it to be completely gone. But try a test area first. It will probably only cost a few hundred bucks for the test area... but better to do the test area first instead of the entire face at first.

Every thing you have just listed points to a compromised moisture barrier (tight skin, texture, overuse of harsh topicals, laser). Your skin isn't deteriorating, it's compromised because of all the harsh treatments. As someone who has severely damaged their moisture barrier through harsh topicals you're best option is to discontinue the harsh treatments and give your skin a break. The biggest factor is time, your skin needs to time to turnover, and to regenerate the ceramides, cholesterol and overall structure of the skin barrier. Seriously think about giving your skin a break from all these treatments for at least 6 months. My moisture barrier has been healing for over 2 years and it's still not 100% but is getting better slowly.

I've been working on my damaged skin (tretinoin) for about a year now.La Roche-Posay Cicaplast Balm was the most useful addition I've added. It did more healing in a month than I had see the previous 7 months. Apply lightly though.

Anyone find anything to resolve this issues. I'm 2 years post laser and my skin has only declined instead of healing.
