Hello every one i really need your help

age= 23 years

skin= oily

scar is now almost 10 months old it all started with a acne, which was a bit big and then it gets like inside the skin from between and outside from sides, and then a scar which slowly turns black and red.

i have tried home remedies on it i applied lemon juice for 3 weeks at night.

didnt get any results , i consult with doctor, he gave me betnovate N cream, which i 1st used on this scar, about 2 weeks or less, then i read dissadvantages and i quit.

2nd doctor was a skin specialist and she gave me , trinition 5% in night, trinition + hydrequinin on day, and a oil control face wash after 2 months nothing really happened, she then changed the medications and gave me isotrex gel to apply on 2 times day and on night apply face whitening cream which was also very expensive, i gave her 4 visits and nothing really happened, i changed the doctor again, he gave me capsules to eat and iso acne gel to apply on it, in one month my whole face got covered up with acne :@ , till then i didnt visit any doctor, im applying Trinition cream on night now from 10 days, nothing is happening its getting darker and its growing because i try to remove the skin around it, when i wash face skin layer over it like plastic gets removed :( what can i do? please any one help me, i dont want chemical peeling or expensive treatments, i have spended so much on this treatment and it is now 2 times larger.

i got some itchy feeling and its not confirmed that it is a wound or scar :(

It doesn't looks like an everyday acne scar to me.It looks deep and solemn.If i were you i'd ring my dermatologist and fix an appointment!

i can talk about ur acne 1st.

u may be purging from the med u r using.if it doesnt stop within about a mnth,then may be u r intolerant to one thing or other in it.

what cleanser do u use?

why do u need a whitener cream all over ur face?

in case u use anything that has to do with acne/scars etc ,always use sunscreen with high spf and both physical/chemical blocks in it religiously.

meds make u sunlight sensitive.

let ur skin heal a lil' on its own.u seem to hurry with too many things.

let it be.

how is ur diet,lifetstyle and activity like?

as for i scar,check the scar forum.see if u find similar.

i anyway know less about scars.

i dont use cleanser, i use medicated face wash for acne control which acts as a cleanser.

my diet is good, and i dont use any sunblock.

and yes i only use whitener to reduce the spots, not all over the face.

does ur cleanser have SA or any bha/aha?

diet is very subjective.

'healthy and 'good diet' isnt a general term.

u may have issues with say dairy or gluten or any grain or sugar or some veggies or some fruits..

its all that suits YOU.

not what is healthy!

Id keep making dermatology appointments until someone can help you with that scar. It looks pretty deep. Is that from a pimple? Or was it caused from something else? Are you picking at it? I can't tell if that is dried up blood why the indent is so dark.

it was a acne , yes i picked at it but not that much, it turned deeper in middle and sides became up like a mountain which has a flat top and lava in it, sorry for bad english

Hydroquinone or a retinoids are pretty good with hyperpigmentation and scarring, but you have to be reallyy patient with them. You're scarring is a bit different though. Maybe try getting a laser treatment for scar removal? I'd also suggest wearing sunscreen so that the scar won't get worse.

Your mark looks like it is not healed one bit, the centre looks inflamed/infected. Do you still get a discharge/pus from the mark? If so then I would not apply anything on it that would aggravate it. You need to heal it. The redness around the edges indicates that your skin is inflamed so putting harsh chemicals on it will make things worse. Your skin needs time to heal this.

1. Stop touching it.

2. Antioxidants and vitamin C are your friend! Boost your intake of vitamin C (tablets are cheap). Boost your intake of zinc too!

3. Use a gentle cleanser morning and night. Don't rub your skin hard when washing. Don't try to take the top off your wound!

3. Find a product that is not going to irritate your wound, but will help it heal. Something similar to DermaQuest's DermaHeal would be good for you ( http://dermaquest.com.au/products/all-skin-type-care/dermaheal ). Use this at night.

5. Sunscreen! Everyday! Sun exposure will make it worse, trust me on that. Anyone who is serious about looking after their skin must wear sunscreen every single day.

The other thing to remember is that when you start a new skin care routine 10 days is not enough time to see if it is working. You need to use the same products for at least a month before you can judge its effectiveness. Sounds like you have swapped doctors too many times in the last 10 months. There is no instant fix for this. You need patience.

The blackness of the scar worries me a bit. You might need to have a doctor remove some of the infected tissue or excise the scar to let it fully heal (I'm not an expert here though, so not sure). I'd try to get an appointment with a better dermatologist asap.

From the size or the dark spot it must of been a very inflamed and infected pore.

 

The dark purple/black is from blood pooling inside the ruptured pore. It probably happened from picking and trying to extract it.

 

STOP touching it and pulling layers of skin off of it.Leave it alone and be gentle with it. Eventually it will come close enough to the surface where it can be extracted.

 

I have gotten these before but never that big.

I was worried that the black could be eschar, in which case it may have to be surgically removed. But I hope it's just blood pooling. I can't really say which is more likely, but I suppose his derm should've recognized if it was eschar.

In any case, a visit to a doctor and at least some antibiotics are in order, I would think.

It does look like that. I suppose it could be from a fungal infection. The OP did say that a thin layer of skin grows over it and he pulls it off. If you look at pictures of eschar you'll see that some of them have that very thin dried sheet of dead skin over the lesion.

Thank you every one for your kind replies, im searching for a good dermatologist this time, in upcoming week i will schedule an appointment , im using nothing on it, now im not touching it and im not removing any skin, im very gentle with it from a year but now its irritating me a lot, i was very patient with doctors, i have spended a lot of money and time on it guys, im not sure what to do, but yea no pus come out and nothing i feel when i touch it, its like a dead piece of skin , and yea it itches on corner only, i will post a pic just before this happen.

It sounds like a bad infection - or just how you described it, a piece of dead tissue from the infection. I think it's a good idea to see a derm as soon as possible just to rule out a malignous growth, since it is a bit odd looking (to my untrained eye anyway) and it's always better to be on the safe side. A good derm should know how to heal it well, in any case, and it's a very small lesion so I'm sure you'll be fine. Good luck. :)
