What is up with my chin? (pic)

Hey guys! I went to see my derm again and tried asking my derm what these things on my chin were. I stretched out the skin on my chin and jaw so my derm could see the bumps easier. She took a look at it and told me they were closed comedones. I've got Retin-A Gel this time so that should work better instead of breaking me out like Retin-A Cream. I'll update if this works for that!

I'm just happy to know they're closed comedones, which are just stubborn things, instead of scars that are permanent. Phew!

I was on doxy for about 1 and a half years on and off. I would say these popped up about 6 months into taking it. I refuse to ever take antibiotics long term again. I think that because antibiotics lower your immunity, these bumps are allowed to exist. Its either bacterial, fungal, or viral in nature. If I could just figure out which it was for sure, I know I would be able to treat these.

Does this pic look like scarring to anyone? It doesn't to me.

holy shit, 6 months into taking doxy you got them? Im on month 5 right now and i developed like two bumps (showed derm, he thinks they are fibrous papules) on nose.

Should i just immediately stop taking the antibiotics, or slowly ween off of them?

The weird thing is, i have two of these on my nose, but i had one before taking doxy, and another developed whilst on doxy (5 months in).

Should i just stop taking doxy instantly, or do i have to slowly ween myself off of the doxy?

Next derm visit, do me a favor & ask her how LONG it typically takes for such bumps to clear. I used a gel retin-a for some 2 years (as mentioned), worked up to .1% after the first year, & then used tazorac for 1.5 years (& its stronger than retin-a), and the bumps did not budge. I mean, does it really take YEARS to "unroot" these things? I don't mean to discourage, but my derm pretty much told me the same. Maybe it's no lie, but it does take a long time...???

FYI, the gel is much better than the cream, IMO. I love gel retin-a (and tazorac!!!); works like a charm for like 99% of my acne. Best of luck to you with that ;) .

I went at these damned things with a bobby pin today, to see if i could extract anything. No luck. What the hell else can I do at this point? I've tried everything.

Siouxsie,

Hate to break it to you, but it doesnt look like the retin-a is going to anything, especially if youve seen no change in two years. I saw no change and a few months with it, and realized it was a lost cause.

There is apparently no solution to these as of yet. We've got to keep pluggin away. These things suck ass.

I had the same thing a while ago (at least i think), though much less severe, and I just squeezed the pus out of the bumps. I applied Bp after, and they haven't really come back since.

My bumps are not so close to the surface of the skin. Squeezing them does nothing. What you seemed to have was clearly white heads, but unfortunately, it's not so clear that's what it is for some of us.

There are also some areas where these bumps are where "stretching" the skin makes them flatten out, so that I wonder if they are not indeed some kind of scarring (which is affecting the elasticity or something).

I wonder if this is associated with hair growth. I know that at least for the facial hair under my lip, my follicles are raised and bumpy. But as you have found, they aren't painful and there is nothing to be squeezed out from them.

Have you tried any physical exfoliants for this?

Yes, I've tried exfoliants. Being a woman, I have non-visible peach fuzz at most, so facial hair is not an issue....I can't even see hair follicles on my chin ;) .

i have a persistant problem like this too.

i started taz for whiteheads. they were treated w/ exfoliation 1-2x a week. Then this type of acne was left over, the problem we all have here, and i began exfoliating Daily which lead me to breakout with a huge amount of whiteheads prone to inflammation. stopped exfoliation, increased taz % and my nose pores went crazy filling up with more solid stuff than ever before. but now i'm too scared to go back to ex foliating and everyone tells me not to exfoliate because tazorac does this already. yet this problem still persists and my nose problem is not getting better w/ strong taz. i already know taz will not do anything for these things. so i'm looking into BHA/AHa to see if i can battle this problem.

it could be just hormornal. remember, these things come and go, come and go. i wouldnt be surprised if my hormones were the direct cause of these things. so if you control that, maybe it will stop forming?

i realize that forcefully getting some out helps a lot if they are close to the upper layers of the skin and the substance in it is covered with a thin layer of skin (thus easy to remove. but this takes a while to happen. you have to WAIT until the shit comes up.. if it ever decides to do so. <_<). but there is a huge chance of scarring if you are dealing with a comedone that doesnt have a hard substance in it and will not budge, it's probably covered with a thicker layer of skin + deeper in the skin but enough for you to see it, which will simply scar your skin if you apply any pressure. i hate those. i get them on my chin and cheeks.

sometimes they turn inflamed =(

i guess i'll have to continue to maintaining my nose with an extracting tool for now. but this will stop as soon as i find a good bha and aha to add w/ tazorac and maybe THIS problem will go away too.

good luck to you all -__- ...

Siouxsie, sorry to hear that the Retin-A or Taz didn't help with those chin bumps. :( I didn't get to ask the derm how long it takes to remove them, but I'll try to ask next time I see her. That is depressing though ‚œ two years and nothing! Perhaps they are not closed comedones after all.

I've been using the Retin-A gel, buffered with Cerave PM, and alternating the Retin-A every other day with AHA Souffle 12%. So far no reduction in chin bumps. They still show up in pics (especially in bad lighting) and look AWFULLY dimpled, making it look like I have a whole mess of comedones on my chin. Ugh.

Any females here have hirsutism? PCOS maybe or any possible symptoms of it? Just trying to find any other links...

Wow i think I have the answer to what i have..im 43 and in the last two years these small white bumps in my chin area have gotten worse...Heres my story: June of 2009 had a bout of perioral dermatits...derm put me on doxycycline and that helped clear initially..The bumps started within the year and I continued to back and forth for 18months on doxy...The bumps always were there. sometimes I could expess a logdged comedo other times I had blackheads. I have tremedous orange peel looking skin and my pores are tremedous. I went on spironolactone in march of 2010 and it hasnt helped much with this at all. If I stretch the skin out its always red, irritated looking, scared, lumpy and there are tons of small pin sized white dots...The white dots look like they have pus in them and you could take your thumbnail and force it up...Sometimes a little plug will release and other times it just breaks the skin and will fill up for a day until it finally dries up...they always re-suface..I often thought my skin doesnt shed properly and builds up a thick layer of dead skin which in turn clogs the pores. Lately Ive noticed I have tons of peach fuzz facial hair and its like the dead skin is getting caught on the hair follicle and will then cause an actual pimple. I just read about Dystrophic Calcification and feel as though this might be something I have as I just had blood work and my calcium and phospate levels were raised...I am also anemic and vitamin D deficient...Its all well and good but I need to know how to treat and stop them from coming back...I use differin and ziana in small doses because they rip and dry the area so bad that all the white clogged pores and bumps look bad..Ive encluded a picture of my chin so I would love to hear back from anyone who suffers like me.

I'm just checking in as my problem has not gone away. I exfoliate every now and them and use aha. I've found that it helps so little, that it isnt worth it, just makes the area red. These bumps won't budge and its a huge bitch to deal with.

Hi all, I'm new here and I'm glad I found this message board. I, too, have these annoying bumps on my chin area for a few months now. Interestingly enough, I was on an antibiotic regimen when these things started appearing. My doc said they're probably caused by oil buildup but I have my doubts. He prescribed a different antiobiotic for me, something other than the doxycycline I was on for a year, and I'm really hesitant on going on this new one.

These things are so bothersome... I'm afraid they're permanent. I wonder if laser treatments would work..

Unfortunately, creams and antibiotics will not help in treating your raised hypertrophic scar formations on the chin area. The only treatment that will significantly reduce the size of your scar cluster is the 585 nm pulsed dye laser which penetrates deep into the underlying skin tissue. Repeated treatment coupled with steroid injections and silicon gel sheets will give you great results. Good day.

Thanks for this info!

Do you know this from personal experience? Can you tell that these are indeed hypertrophic scars just based on photos?

Yeah, I hope there's a solution out there :(

I really hate these things... it almost makes me appreciate my acne, since they at least go away. These ugly bumps seem to be permanent.

Well, they don't feel like raised scars either! My chin has that cottage cheese look, but it feels rather smooth. The bumps do appear to be under the skin..... I suppose it is possible for scaring or calcification to form under the top layer(s) though.....?

The hypertrophic scarring is beneath the skin so your chin will naturally have the "cottage cheese" look while being smooth to the touch. I've seen many cases of this type of scarring. Scarring of this nature will NOT go away on its own. There is no "plug" somewhere in the underlying skin tissue causing the backing up of sebum. The 585 pulsed dye laser is unique because it breaks up the scar tissue while stimulating collagen so it'll end up evening your skin. Other lasers typically used for acne treatment remove the top layers of the skin but cannot penetrate deep enough to minimize the scar. If you are below the age of 21, you should focus on minimizing the chances of new scars forming and then seek 1 or more treatments depending on the severity. Don't waste your time on antibiotics and creams unless its an ACTIVE acne site. For an active acne site, I've issued benzaclin coupled with a neem soap face wash 3 times a day. This regimen coupled with massaging the scarring daily seems to make the scarring less noticeable over time while eliminating the chances of new scarring. When you reach the age in which acne is no longer an issue I would highly recommend the 5-10 minute pulsed dye treatment which may be coupled with silicon gel sheets and steroid injections depending on what your doctor recommends. Since the size of the individual scars is small, you are more than likely to see significant improvement. Good luck!

Are you a dermatologist? I really appreciate the insight, but can you be sure this problem is scarring? I can see a plug when I stretch the skin. I've looked closely at my skin, and it really does look kind of like a callus build up of some sort. Each spot has a hair follicle with white hair growing out. Also, they appear to be less or more severe at different times. This wouldn't be the case with scarring. Are you positive??

Thanks a lot!

i agree with you strangeskinbumps...I dont think mine are scarring because i can express and plug sometimes and see it attached to the hair follicle. Mine are also worse when my skin is dry
