Please help, is this normal post TCA Cross?

@icikiwir Sure. this is one . Not as big as you guys's but the hyperpigmentation issue is similar. What bothers me is most of the ones are not noticeable at all but these few are really stubborn and I do not know why probably the doctor apply more on these spot

@tcany one thing is i think hydroquinone may take 2- 3 months to start see result. I will wait at least May 1st. And do not forget sunscreen!

@icikiwir Honestly i do not think they get deeper or wider or shallower, I feel they stays the same, but the redness make them look wider because of the tissue damage. And also when i search Reddit, lots of people feel the scar got deeper and wider initially because of the hyperpigmentation, but after the hyperpigmentation fade they do not feel that way anymore

@icikiwir hydroquinone should be able to treat all hyperpigmentation, but I am not using it since my skin cannot tolerate it

@icikiwir Can i ask: do you think the redness spot's skin texture is little different than the other normal skin? Mine seems not the same texture with the other skin

It works for PIH, post inflammatory hyperpigmentation, which is increased melanin. It seems like he believes it's PIE which is broken capillaries that developed at the wound sites.

typically you use hydroquinone to treat hyperpigmentation. he's saying that hydroquinone isn't used for your situation. I would contact him again and ask him directly if he thinks it's PIE. Did he not recommend anything for it and to just wait it out?

I don't have redness. Keep following up with doctor and follow what he says. Or you may want to go to another derm and get a second opinion just in case.

@icikiwir i found this from the internet:

A quick test to know if you have PIE or PIH is by squeezing the spot where you have the mark.If the mark disappears or turns white when you apply pressure, it is PIE.If the color remains the same then it is PIH
The hyperpigmentation can also be red. I am pretty sure I got PIH instead of PIE and mine are red. probbaly because I have fair skin

@tcany got it. I feel not only the the skin has different texture, the skin is a little thinner. But i learned this is a normal phase when wound is still healing.

At this point seems we have not better choice, only wait for 2 more month to judge

@roserose23 when is your dr appointment? Maybe hell suggest something. I asked about VI peels, mine said its not good to do, but I saw other derma suggesting mild peels including VI peels early after TCA cross for PIH.

@tcany Mine is March 10th. my original appt was at Feb 28th and he had family emergency and rescheduled me to March 10th. i was so disappointed since I need to wait 10 more days to get all my answers

@icikiwir pretty much the same.

I visited my derm today he said showed me the before and after photo, seems some spot is better? but just so red hard to tell. The PIH is normal and will fade away with time but if I want he could do a vbeam laser on me. He also mentioned it is still in the healing stage I will wait until 4 month .

@icikiwir Ive been using creme with hydroquinone for 1 week, the dark brown pigment is a bit better, so I will continue with it. It is safe to use for 3 months max, do I wont do anything else for now. Then I have to do other treatments for the skin texture, but dont know which yet..

@roserose23 Ive visited a few derms and one of them told me laser could increase hyperpigmintation because its early after the peel, might irritate skin even more. Hows the texture of the skin?

@tcany Yes, I also want to wait another month before doing any laser treatment although my derm offered it for free in case it make the scar look worse. He did not mention texture, but I feel if the skin is still healing then it is expected the texture is different?

BTW what did your derm say about the hyperpigmentation is getting darker and darker? Mine said it is because of the sun exposure. But I swear I did not have direct sun exposure I always wear sunscreen and hat when I am outside. although I live in LA so it might be too much sunshine every day

@roserose23 I highly doubt its from the sun, if you wear spf. I just moved to Floroda from New York and my pigmintation got dark in New York and it wasnt sunny, I spent most of the day inside. My derm told me that hyperpigmination is due to melanin, wjy it happens is because the skin has been very irritated with a high concentration of TCA. It happens with some peoppe unfortunately.

Nice that your derm offered for free. I hope you wont need it and it will heal on its on 🙂

@tcany Thank you for your kind words! For mine it is just weird ,some spot got the PIH and not fading at all while other spot fade quickly, maybe because different area irritated differently when TCA applied to them. When I read Reddit, one of the user said she did not have any PIH issue the first time but the second time she was also all red for several months.

Hope we all can heal quickly. I will update here if I see any changes, please also update if you can. thanks
