Questions to ask during subcision consultations

I have consultation appointments with 4 dermatologists, and need to choose one for subcision.

 

What questions should I ask?

I want to ask how much experience do you have? I asked the nurses on the phone when making the appointments, however they generally say he/she has been doing it for many years. (But this does not actually answer the question, because I wanted to know how many were done). I also like to know if there are any complaints, etc, but I dont think anyone will tell me that. I searched acne.org and realself.com for reviews of the specific doctors, and only one has any reviews there.

 

Also, I am confused by fillers (if needed). Should this be at the same time, and done by the same dermatologist?

If your scars are atrophic, I treat them at the same time of subcsion, usually with HAfillers.

@SkinTreatmentYou do not need to link my posts, that is not necessary. Please ask your question and I will respond in 24-hrs. I have the worst acne scar cases to help daily - over 100 of them by PM.

This is supper simple, your flying international, ... any Dr will inject your right away after subcision, you have no time for ongoing treatment, you take on this risk. Secondly Dr Lim is a top 1% Acne Scar Dr he takes on the liberality if something goes wrong, there is a risk. What sould you do to aboid risk, wait 3 months between Dr's treatments, Microneedle at home once a month(Derminator Google it) and alternate with Glycolic peels. Get filler once your swelling goes down, this is different for everyone, one week or a few, you want to be able to see where to fill properly.

BTW I am not actually going to Dr Lim. I just started learning about acne scars by watching his videos (he also mentioned acne.org and moderator BA in one of his videos)

Please give me your opinion on this treatment plan:

(two dermatologists have suggested this to me)

 

SUBCISION - break tethers

INFINI or INTRACEL MICORNEEDLING - same session to start collagen growth

After a few sessions..

FILLER - depending on the results from above (not used as spacer to stop reattachment of tethers)

 

I have read many of your posts, I think this is different from your suggestion:

SUBCISION break tethers

FILLER after swelling, approx 1 week used as spacer to stop reattachment

INFINI MICRONEEDLING increase collagen growth - after at least 3 months to allow healing/collagen growth from subcision

 

I am going back to ask why not filler after subcision, and microneedling a few months after. When making the appointment I asked the nurse, she said the heat from rf microneedling would destroy the filler.

 

Also, I will ask does he believe only subcision (plus HA filler as a spacer) will have good effects (ie. produce collagen growth). I am not sure about his opinion, maybe he feels that does not produce significant collagen so he suggests INFINI/INTRACEL immediately after?

Checkout the main scar treatments sub, find the FAQ pinned on top there, goto the bottom, there are Dr's or call around and ask who does cannula subcision or nokor subcision (needle type) to dermatologists and or plastic surgeons, consult a few before you decide who you like, even if you have to pay as treatment is expensive, better like them. Ask how many subcisions they do a month and if they treat your skin type. If they just do laser run they will just blast you and not solve your issue.

Also just to clarify, my doctor suggested rf needling in the same sessionbut he DID NOT say an explanation. I am going back to discuss with him, in the post above I wrote maybehe feels...

@SkinTreatmentI would not do RF needling in the same session, you will stunt the effects of the manual methods. Many Dr's do this as they get more $$$$ are generalists not acne scar specialists. They promise better results when we all know that healing takes time and this is a long marathon thing not a race to treat. The longer they keep you swelling the more they can say see look it is better. When you wait 3 months between Manual methods you actually observe what worked and did not. RF needling you can alternate between subcisions or tca cross sessions if done every 3 months. Also they are counter indicated, ... why would you cause coagulation when you want subcision to break through the scar tethers and the body to make new collagen. We treat the deepest issue first. Manual methods are always superior. It's all about money really he get's double or triple if you do rf needling as well. This is my experience helping thousands, ... but if you can't wait or only have a day to see someone internationally maybe it makes since to you. The only one I would let do multi procedures on a patient is Dr Lim and he even warns them that is not a optimal way to treat.

When you say alternate, do you mean to wait one month between derminator and glycolic peels? I just had my first subcision done by Emil (what a nice doctor by the way!) and I'm trying to come up with aschedule for best results.

June - Subcision #1

July - Derminator

August - Glycolic peel

September - Subcision #2

June - Subcision #1

July - Derminator

August - Glycolic peel

September - Subcision #2

Post Pics, you may need TCA/Phenol cross. Depending on your severity, at home treatments provides little effect.

@A_ZWe are sidetracking someone else's post, ... you can make your own 😉 One month between, ... you wait a month after your sub to do either a glycolic peel or dermastamping. You alternate between, then 3 or more months latter(whenever you want, you treat with Emil).

Regarding what the other poster said, ... there is some confusion here. We peel at home for texture, ... this is not a deep peel but cell turnover between treatments. TCA Cross is for small tiny icepicks and that is best done by Emil during your next subcision if you even have this issue. You can do both of course at home and Emil.

Use a retinoid nighly for cell turnover.

But sounds good.
