I think of all the horror stories we read on this forum, subcision is by far one of the safer techniques. I've read the occasional complication, but have not seen outright disasters. In skilled hands, subcision is pretty safe in my opinion. Nothing in the study is magical or top secret. All of these techniques have been used before and by many doctors right here in the US. I think if you find a good caring doctor, I don't see why they could not perform these same techniques. They're all very basic techniques that have been used forever to treat acne scarring. The difference is in the sequence of how they do it. And yes, all of these procedures are technique sensitive and has more to do with the skill of the doctor.
Does subscision work on deep boxcar scars?
in general subcision is only effective on rolling scars because they are bound down by scar tissue. up until recently enerjet which is subcision + filler is the only subcision method that is effective on boxscars

Lighting and angels are also different. In India, cost and skin pigment are also concerns. They are using old techniques that have been used for decades in more advanced countries but most patients in India can't afford more advanced technology. Even needling has been around forever in Asia. I think the combination can produce nice results but question why the study claims any of the patients ended up without any scars. I don't see smooth, scar-free skin, in any of the photos. I do see good improvement.

It might be new in India but it is not elsewhere and I think it is misleading to claim any of the participants are "scar free". No_hope, I recall many threads where you were strongly opposed to needling, TCA or anything that would pierce your skin or possibly cause holes. I distinctly remember a thread where you were against subcision. Perhaps you are just more open-minded now, and as a result, giving these methods a chance and seeing the potential for improvement. your right. before i was very negeative becausen i didnt have experience in actual treatments that make a difference. now that i have had some treatments done it has changed my point of view. i beleive in needling now as before i didnt and thats thanks to actually trying good methods. this study is just mind blowing i wish every doc could adopt this study and start practising it. its a shame other parts of the world that are less advanced in technology are ahead in the acne scar world.
I have had 7 dermarolling sessions done by professional with around 2 months between each. I have not seen any sort of improvements at all. 2mm needles used.
My scars are mostly rolling. Have shallow and deep ones. I will grade myself grade 3 scars where once the skin is stretched the scars appear gone. However my temples are where the deeper ones are located. It's already been a year past my last dermarolling. Not even the most shallow scars have any effects from a 2mm dermarolling done aggressively with blood oozing out in every single session.
However I believe the report is kind of real assuming you believe there's no microswelling from the dermarolling OR retin a. The only thing that I didn't do is subcision. But the doctors here are all chickens they don't dare to do subcision on me because they said my scars arent tethered down. Grade 3 scars and aren't tethered down? Come on....
This study seems to be related to the skill of the doctor rather than technique. These guys work in a hospital and performed the procedures for research publication purposes. So you can assume that they're slightly more skilled than the average outside clinic doctor and more motivated to do a good job for their paper.
In the US I'd imagine doctors would avoid subcision because it's more delicate and risky. Why bother with surgery that you can screw up when you can just use a laser instead? In India, their faculties aren't as well-funded so they have to resort to simpler methods.
In principal I agree with their methods. Scar tissue are tissue that heal wrongly and need to be broken up and encouraged to heal normally. They're using multiple methods of attack to break up the scar tissue, while delivering retinoids to the scar tissue. Retinoids are the only proven drug that works. Keep in mind you can never go back to scar-less skin, so the improvements they've shown is very impressive.
The hardest part is probably to find a good doctor to perform subcision. So maybe you can fly to India and have it done by those guys. The other stuff, TCA and dermarolling can be done anywhere.
