First time on roaccutane, slightly scared, have a few questions.

So I recently had a huge flare up of acne which I hadn™t seen since I was 16 (I™m 24 now). I recently moved to another country which maybe had something to do with it. So I went to a dermatologist and was on doxycycline for 2 months. It definitely improved my skin and it doesn™t look as inflamed anymore and there has definitely been a reduction in the number of pimples. 

 

So I went back to see the dermatologist and he said my skin is improving on the doxy but he wants me to switch to roaccutane for a month and see how that goes, as the antibiotics eventually lose their potency. I™ve obviously seen the good that roaccutane does but like a lot of people reading about the side effects does scare me a little. I™m definitely going to give it a try and see how it goes but I just had a few questions which I was hoping someone would be kind enough to help me out with.

 

  1. First off, do you think my acne warrants roaccutane? I™ll attach some pictures. I™m very concerned about going too strong with the medication and my skin freaking out. 
  2. Does everyone experience a purging phase when on roaccutane? I experienced what seemed like my skin purging when I started the doxy 2 months ago so I really don™t want to have to go through that again. My skin is currently at a manageable level and the idea of it getting worse than it™s ever been as some people seem to say, is unthinkable for me right now. If the purging is unavoidable, how bad is it usually and how long does it usually last?
  3. Are there any instances of roaccutane actually making your skin worse? I know usually you hear about people with severe acne using it but I don™t really consider mine to be that severe (maybe moderate, I™ll attach pictures.) so is there any chance my skin will break out even worse than before due to using such a strong medication when maybe my acne doesn™t warrant it?
  4. Are the side effects very common? I™d say my skin is definitely on the more oily side at the moment. I™m not too concerned with the dry lips and skin. It™s more the effects on your mood and other things which worry me. I™d say I™m someone of quite a positive disposition so hearing about depression as a side effect is definitely a concern.
  5. I currently use a 5% benzoyl peroxide gel on my face at night which has given me good results in the past. Should I continue with this when on roaccutane if I do get some more breakouts.
  6. My dermatologist was super casual about the whole thing and I really wasn™t expecting to be put on roaccutane. I™d always thought of it as something for the serious cases, not something you switch to after 2 months of doxy when your skin is improving. The only side effect he talked about was dry lips. Is it possible I™m being put on a less intense version of roaccutane which might have less of the negative side effects?

 

I work as an English teacher (in a foreign country) so I do get self conscious about my acne and worry that the roaccutane is going to cause a huge flare up which will in turn give me stress and anxiety about standing in front of a class every day, which I™m worried could lead to the depression and other things.

It is possible that accutane can cause or exacerbate outbreaks of fungal acne, because accutane can alter gut bacteria and therefore compromise the immune system. The evidence is not strong, but hardly any research has been done in this area.

Have you considered that you might now have fungal acne (caused by taking the doxycycline) not ordinary acne ? Fungal acne (malasezzia and/or candida) is CAUSED by taking antibiotics.

I dont think so, it was a lot more red and inflamed before I started the doxy. In terms of the number of pimples and the overall redness and inflammation the doxy has definitely helped.I also have 0 acne anywhere else apart from across my cheeks and jawline (back, chest, forehead completely clear).
