I started isotretinoin at 40mg a day, 7 days ago. I am 65kg, 18yr male. First night I had a hard time sleeping, I figured it was because I watched too much tv. I normally sleep 6-8 hours and i usually fall asleep within an hour. No stress, no changes in diet, or exercise. I can't think of any possible anomalies that would cause this kind of insomnia. It takes me at least 2 hours to fall asleep and I can only sleep for about 3 hours a night. I often get muscle spasms in my chest while I am in bed which is something has not happened before treatment. Since day one I have had a hard time sleeping, and I have zero previous experience with insomnia. Melatonin does not help much. I am calling my dermatologist today to talk about it. From what I gather this is not a common side effect though some people report this. One website even listed it as a serious side effect and to call my doctor right away. At 7 days I don't have any of the common side effects, but I can't sleep at all. Has anyone else experienced this or have any wisdom to impart on me?

I also notice a slight problem falling asleep at night. My mind wanders more than usual but it's not a terrible problem for me... Maybe ask your derm if you can take some melatonin at night or something to help you fall asleep?
