I am on month 3 of accutane, my acne has reduced but there is no changes to my closed comedones.

These closed comedones has been on my face for 1-2 years.

My dosage is at 30mg , do i need a higher dosage for these comedones?

As far as i know accutane does nothing for comedones. The best treatments are topical retinoids or b.p. or chemical peelings. After you are finished with accutane you could try them because now they could be very irritating for your skin.

This is exactly what I struggle with. I am just starting month 5 now and although my inflamed spots are subsiding the uninflamed stuff is still there.

The only way Accutane will get rid of comedones is if it irritates the area around it enough to inflame into a zit.

You will notice this with high strength retinoids as well. Retinoids never let a comedone exit gracefully. The irritant factor of a retinoid will just irritate the comedone until it turns into a full-on zit.

Is manual extraction by aesthetician the only way to remove?

Pretty much something to loosen the comedone (e.g. a high strength salicylic peel) plus manual extraction is the only thing that has worked for my stubborn closed comedones.
