Hi,
I'm a (almost) 22 year old male, I have taken accutane for 8 months (about 10000 mg in total i think), and i started experiencing excessive hairloss the last months of my treatment. I ended the course in september 2009 and hairloss started around june 2009. So, i'm shedding for almost 2 years now, but my hair still seems rather thick (it was never superthick), no receded hairline whatsoever.
What does this mean, will it eventually cause me to go bald or will my hair become very thin? When i first told my derm, she didn't believe accutane caused my hair to fall out and she said i had MPB, but why isn't my hair receding then (this was 2 years ago)? (i'm not taking any drugs like minox or fin to stop the hairloss). Will my hairloss stop one day? Sometimes it freaks me out a little and i'm surprised i still have this much hair.
family history of mpb: on my dad's side every man is bald, on my mom's side everyone still has a full head of hair.
I'm an 18 year old male and I have been on 20 mg a day of accutane for about 1 and a half months. I first noticed my hair shedding more than usual at the first month. No hair clumps in the shower or randomly falling out, but when I rub my hair to shampoo or rinse, about five hairs get caught in my finger. If I rub my hair over a piece of paper for 15 seconds, about 10-20 hairs will come out. Sometimes I just comb over my hair once with my hand and 4 hairs will fall out, and sometimes none will. I have noticed a lot more body hair and eyebrows/eyelashes in my bed sheets though, and my eye lashes are definitely falling out more than usual (not crazily, but they are)
I'm worried because everyone here seems to have taken 40-80 mg accutane and noticed hair shedding after their treatment. Should I stop now and hope the shedding stops (will it)? Or should I continue and hope the shedding stops and/or my hair grows back post-accutane? I mean, I feel like if I already started shedding, accutane already damaged my hair cycle or whatever, and so whether I'm on it for 2 months, 4 months, or 6 months, the hairs have already been damaged. Please, someone tell me this isn't true ? Doesn't help that I'm in my obsessively check hair/emotionally unstable phase...
My scalp and hair is EXTREMELY dry and I haven't done much to oil it up, and I've been avoiding showers for a couple days because I'm afraid of more hair coming out, so I haven't been putting conditioner. Could this be the cause, what should I start putting on my scalp?

Honestly, when you start to realize that your hair loss may become permanent, you finally understand acne can be solved in the long run, but hair loss cant. If i were you i would stop accutane right away, so even if your hair cycle is already damaged you at least have a higher chance of getting your hair back. Trust me.

Honestly, I can't for the life of me figure out if my shedding is more on the normal side and paranoia or actual shedding. It never comes out in clumps, and it only sheds 10-20 hairs if I aggressively rub my scalp for a good 20 seconds, if not longer. In the shower, I get about 2 tiny thin hairs and 1-2 longer black hairs everytime I rub my hands through for shampoo and the like. Once in awhile I'll get 5-6. I never notice clumped hair in my towel or the shower sink, and my pillow has about 4-5 spaced out hairs that I really have to get up close and look for.
Was your hair loss similar to this? Was this similar to the rest of the people in the thread noticing shedding? So far I have no visible thinning, but does the thinning only start to appear after months of shedding or was it overnight?
I think, I'm going to finish my 20 mg dose for this month (two week supply), and then for next month I'm going to drop down to 10 mg. If I notice my hair shedding any more than the current rate at 10 mg; I think I'll stop.
The thing that irks me is that I was never comparing my normal hair shedding before, I only started to check after being on accutane for awhile. I remember myself going through a whole hair loss panic phase 1 year and a half ago where I swore to myself my hairline was receding and my hair was coming out in my hands in the shower... but that turned out to be all in my head; I'm hoping this one is too....

Accutane can speed up the process of Mpb if you have a family hx. I am a girl so I am not sure about why there is no receding?
Have you thought of having any blood work done as sometimes hormonal or low levels of iron etc... can cause constant shedding, which Accutane has been known to disrupt.
Good Luck!

Just drop accutane now, period. I know i sound like some freaky anti accutane person right now, but around 3 months ago i was the same as you, holding so much hope for accutane and thinking on the "positive" side. However, reality strikes, and in the end its not the doctor thats responsible for your health, but yourself. You really got to weight your options and make a logical decision, instead of analyzing while telling yourself everything is going to be okay.
Since you're on such a low dose, you're really not going to see significant results until towards the end of the course. That is why going on another month of 10mg isnt worth the risk of losing your hair, possibly forever. In my opinion the only people worth risking their hair of never coming back are the ones on extremely high doses for their acne, which has a higher chance of eliminating their acne for the rest of their life.
From what i read, no, its most likely not in your head. And yes, thinning hair is a long term process, you're not going to notice it overnight. Just once again keep in mind that just because you stop taking your next pill, doesnt mean your hair will come back right away, this is why im telling you to stop taking your accutane right away.
Its all up to you in the end, but i hope you take my words seriously, and all this is from someone that only did barely 3 months of accutane, and still shedding about 2 weeks post accutane.
One last thing to notice about your hair is, is it flat, frail and weak? Does it look dry and lifeless, losing the ability to "stand" after a shower like it used to be?

I'm going to drop it; hopefully I can find other ways to minimize my acne, but I'll at least keep my hair. Hope stopping it this early will stop further hair loss, and that my entire hair cycle isn't damaged . Thanks for the advice. I've also noticed that if I don't cover up my temples and brush them all the way back, the area looks a lot thinner and patchier; I can't 100% say if that's new (I think it is), because I wasn't comparing before. But anyway, I'm not taking that chance.
As to the question, during the day my hair looks dry and lifeless; however, it still can stand if I brush it that way. After the shower, it actually looks kinda good cause it's a little moist, but after 15 minutes or so it dries out again and goes back like before.

No problem, just helping out someone having the same problem. I don't want anyone going through the same thing i do, hope your hair grows back soon.

Thx! I haven't done any bloodwork after my treatment. During treatment everything was fine but i'll look into it.
Also, i've been looking into the genetics of balding recently and i don't think i have mpb. According to wikipedia to develop mpb, it is necessary to have a bad variant of the androgen receptor gene which is located on the x-chromosome. You inherit the x-chromosome from your mother (if you are male). My grandfather who is 75 still has a full head of hair so i assume he has the good variant. He also is mildly colorblind just as me. Color blindness is also x-linked so i think there's a very high chance that i inherited his x-chromsome (since i have no other colorblind family members that i know off). So, if you add to this my crown or temples haven't changed after 2 years of hairloss i guess i am just experiencing chronic telogen effluvium caused by accutane.
Wow, I don't know if God is playing tricks on me or I'm hallucinating or something. After not taking accutane for 48 hours; when I rub my hair for a good 15-20 seconds in a pretty rough manner, I had to TRY to lose 3 hairs (one or two weeks ago I would lose over 10). One fell out during the first rub, none during the second, and finally two during the third rub.
Anyone know what's up???? I thought accutane stays in the blood for weeks. I guess I should order a full blood test by an endochronologist or something and see if some nutrient is screwing with me.
Edit: Part of me wants to go back on the accutane before my face blows up again... I feel like the hair thing might have just been Body dysmorphic disorder (I jump from hair, to acne, to face shape, to whatever...). What do you guys think? Waking up with 5 hairs on the pillow and losing 3-5 in hands after shampooing; is that accutane induced hair loss/shedding or just OCD? I wish my derm could tell me, but of course he just touches my hair and looks two seconds, says he sees nothing and says do whatever, stop or continue.

3-5 hairs in your hands after shampooing is nothing. I would loose 50+ hairs when just combing my hair in the morning, let alone shampooing. Even if your hair is perfectly healthy you loose about 50-100 hairs a day.

Did the hair loss start at a little bit then gradually increase over time or did it you just start losing 50+ overnight? Also, how exactly do you count your hair loss while brushing or in the shower? I mean, do you see 50+ in your comb or on the sink or actually falling out from your head? In the shower do they group up near the drain or do you count 10 or 20+ on your hands after each rub?
See, I can't say I have major hair loss; but I know two things for sure: I'm losing a little more hair than usual and my hair in general is more brittle. My right temple, while it hasn't completely receded, is a lot patchier than it used to be, but I'm hoping the patchier spot doesn't eventually become a recessed spot.
The major thing that worried me isn't showering or the pillow, but when rubbing my hair back and forth aggressively for 10 or 20 seconds, I'd lose 15 hairs. If I just comb my hair down into the sink over and over again for 20 seconds, I'll lose 20ish hairs. Whereas now, after stopping accutane for two days; after two rubbing sessions of about 10-20 seconds, I lost about three or four hairs. I also counted two hairs in my pillow after actively looking for them and getting up close. I did the rubbing again today for 10 seconds and about 4 hairs fell out each time.
Anyone know the cause of my sudden hair virility after 48 hours with no accutane? Is that a sign of a certain type of shedding or anything?

I'm not really sure tbh. My derm never warned me of hairloss as a side effect, so i didn't pay attention to it. But yeah, quite suddenly i noticed more hairs in my hands when showering and stuff.
I brushed my hair above the sink and then counted the hairs in the sink. I never counted them in the shower .
If i were you i would stop using the drug if you experience increased shedding. It's just not worth taking the risk in my opinion. If i knew what i know now, i would have never taken it and tried finding other solutions.

I stopped it a few days ago; not going back on. I'm sad though cause it definitely receded my right temple a bit (didn't touch my left--weird), and it seems like the last few days the shedding stopped, then all of a sudden today it jumps back up. Seems like some days there is very little to no shedding and others there is a moderate amount of shedding. Hoping I stopped in time so that it didn't completely damage my hair cycle.
So basically I have some hair loss and my acne is coming back . I've tried duac/retin-a and xenia and stuff for 5 years now with antibiotics. I got close to clear last summer, but it all came back this christmas.
Edit: So I saw my dermatologist today and he told me my hair is fine and to go back on accutane... just want this to be over xD.
Welp, my hairloss is definitely related to accutane. Two to three days after stopping accutane my hair shedding completely stopped, nada, gone. I shed maybe 2 hairs in the morning and maybe 3 in the shower.
My derm told me to test out if it was accutane or a normal shedding phase, and so I started accutane again. The morning after I restarted accutane; I tested out my hair shedding and bam, after rubbing it a few times here, and a few times there, 20 hairs fall out. Did it again later, and another 10 fall out.
What I don't get is everyone says accutane takes 2-3 weeks to get out of your system? How come my symptoms come and go within 1 or 2 days? Is my body just getting rid of it quickly or is this indicative of a different type of accutane hair loss?
Hey all,
I have been posting on this thread from the very beginning and I wanted to give an update. I have been experience accutane-related hair shedding for a little over 4 years now. The thing that is crazy is that I still have a very thick head of hair. I have taken propecia for about 3 years because I was afraid my shedding may be leading into MPB as that gene runs on my fathers side. Maybe the propecia is the reason my hair has kept its density, I don't know. When I comb my hair I notice a ton of short little thin hairs, which would make me think my hair is miniaturizing and I am going bald, but my hair has really not lost much thickness at all, so I am thinking these hairs are regrowing thicker, then possibly thinning again and falling out in a cycle that keeps repeating itself. I have no problems if my hair remains thick but I have a feeling it is going to catch up with me at some point in the future...

Your kidding right. Everyone loses hair everyday, and it's completely normal. It's no reason to stop accutane especially after 2 days and losing only 3 hairs, not to mention you were rubbing your head really hard. lol...

That's after I had stopped accutane for two days. Once I restarted it (just took one pill); immediately, the day after I would rub fairly hard for a good 10 seconds and 10-15 hairs would fall out. If I repeated more would fall out. In the shower, whenever I shampoo or condition, I'll notice a good 5-7 hairs in my hand. Usually of those 5-7, 3 or 4 are thin wiry hairs and 2-3 are normal black ones.
Been a couple months since I posted.
Well, my shedding/thinning has gotten worse. My crown thinning is getting positively grim, and is beginning to extend further forward over my crown to the top of my head, and I'm noticing temple thinning as well. The hair at the sides of my head is also rather thin.
Depressed, wanna die. Fuck Accutane.

I have the exact same thing. Baldness only runs through my fathers side of the family. I've been shedding for 2 years now but i don't really notice any difference in my hairline or crown. Thickness is also still good. I must say the majority of my hairs that fall out are quite normal regarding color and thickness and they all have a white bulb at the end.
My derm gave me prescription clobetasol which i apply to my scalp every night for about two weeks and it's supposed to get rid of sebhorric dermatitis. So far the itchiness in my scalp has subsided a lot.
I used clobetasol propinate for 6 months (.5%) until it thinned my scalp out and made it uber sensitive. Be mindful that topical steroids will thin your skin, thin skin affects the ability of a folicle to hold a hair.
I also tried Betamethason Valtorate .1% which is a milder steroid, it helped with the itchieness and made me feel like I was doing something to combat my hair loss, but both topicals proved ineffective.
I am at 2 and a half years of consistent shedding with about 30% hair density on the top of my head, still losing about 200-300 hairs per day and not getting better.
I have an appointment with a specialist in auto-immune related hair loss on Monday, hopefully he will have some answers. I will update if he can "shed" some light on the situation. Sorry that was a bad joke.
Good luck to all!
Cheers,
Matt

Thanks for the heads up. I'm just gonna stop using it.
