That almost certainly seems to be PF. Look in my sig at the bottom for a link, I think I've found something that works well for folliculitis.

PG... your story is a near replica of mine...except I went into the hot tub about 4 years ago. About 10 minutes after getting out I started getting what i thought were hives all over my shoulders. Next day I went to the store and and picked up some benedryl cream to put on it because I thought it was a allergic reaction. A couple months later, my derm started treating it as acne which lasted for about 3 years. Over that time, it progressively got worse. Finally this year I got a new derm who diagnosed my case as PF, and I have since done both sporanox and lamisil with little luck... The little itchy bumps are pretty wide spread now covering most of my back, chest, shoulders, upper, arms, and abs... I'm pretty desperate at this point myself, so you let me know if you find something that works! Good luck with your search... Also if anyone understands how a simply entering a hot tub would alter the amount yeast my body would start producing from that point on, please let me know. I had no skin issues prior to entering that hot tub...

Does lotion worsens acne or Pityrosporum Folliculitis? My skin is mildly dry and I keep seeing these lotions that keep saying that they will renew skin and help even skin tone. I have scars from acne/PF and was hoping that these lotions will help fade these scars.
Any help is appreciated.

1. which OTC product should I try first? I'll go to Walgreen's tomorrow or Friday to look for something..
2. When I'm at the derm next week, should I ask if they can do a test for this?
3. I've been trying to treat acne for years without really responding to anything, so now I'm on an extensive treatment. how many folliculitis cases get mistaken as acne? do acne meds make folliculitis worse? I feel like I'd be slightly relieved if this is diagnosed for real I can treat it the right way
4. I'm worried about stopping the Doxycycline so soon after starting it, but if I use something to treat folliculitis in conjunction with antibiotics, does this ever prove positive?

Hmm, it's possible it is PF, is it ever itchy? And do they tend to spread?
You could try using ketoconazole shampoo or cream, sold under Nizoral. Head and shoulders or anything with zinc pyrithione works well also.
Acne treatments will work PF worse. PF is fungal, and therefore competes with bacteria for space on the skin. Kill off the bacteria, and the fungus is free to spread as much as it wants. You may have success treating it whilst on anti-biotics, but there's a good chance it will recur if you don't stop them. I'd ask them to test for it - insist, because they will probably just tell you that you don't know what you're talking about.
I have been fighting hot tub folliculitis since November of 07. I have tried antifungals and antibiotics orally to no avail. A week after I stop taking them the spots come back on my chest, shoulders and upper back. What I have found that seems to be working, for now anyway, is to shower as usual, I dry off, get a wash cloth wet with hot water and add a healthy squirt of Hibiclens. Don't be stingy here. I rub down with that all over my upper body and let that dry on my skin before putting clothes on. I do not wash that off until my next shower. You have enough on if your skin feels like you just applied lotion. Hibiclens does not spread well without the wet washcloth.
I am now down to just getting small bumps where elastic touches my skin all day but this is at least tollerable and they dry up and fade quickly.
I am also using Selsun Blue for my scalp. My scalp feels much less itchy and I dont feel the bumps after just 2 days of shampooing with it.
The above method has had a good effect on my skin in general.
Things that didn't work so well were 70% alcohol applied 2 times a day, oral anitbiotics/antifungals. I also tried, in the shower, lava soap as a exfolliant, hibiclens, and bodyclear in that order. I don't think applying the hibiclens in the shower then washing it off gave it enough time to work as an antimicrobial.

Just wanted to pass along any info. about the pityrosporum folliculitis topic. I myself have struggled with it for quite some time and it seems to come and go over a period of months. I used to work for a dermatology office, and I was originally diagnosed with a staph infection, and was treated with strong category antibiotics, and then I ended up pregnant with my son and had to come off of them. Amazingly enough, the papules and pustules seemed to go away for a time, then here over the past year they have come back more often. I have taken Ketoconazole (Nizoral) orally, and seems to help the symptoms for a time, seeing as how my new dermatologist has labeled it a basic yeast infection on my forehead.
I have been told though, that really the only way to rid a person's body of the pityrosporum yeast, is to do a total cleanse and to have a strict diet--virtually no sugars and no foods that convert to sugars---yeah, I know right!! This advice of course has not come from any doctor, it has come from some nutritionists and alternative medicine experts. I have taken the steps to do some yeast cleansing, but have been told that unless I couple this with the strict diet (candida cleanse diet), that I will not completely rid my body of it, and it will continue to come back. Makes some sense, as I've been told that over the years, my body has become probably overgrown with yeast levels, due to several factors.
Just thought I'd throw this out there, and see if anyone has anything to share on this or maybe takes anything away from it.
This got me really curious. Im pretty sure the yeast is in everyone, so you theoretically cant get rid of it, its when it overgrows thats the problem..right?
Well assuming im right, it leads me to my next point. If you were to do a "total cleanse" and have a strict diet for a month or two and allow your body re-balance the good/bad fungus/yeast/whatever...you [could] slowly start easing up on the strict diet and return to your normal BUT healthy diet.
I may try this, im pretty sure i have candida (although it was due to a very poor diet for a whole year and not antibiotics) so it'll be like knocking 2 birds with 1 stone. This could also "fix" the root of the problem and stop the reliance on all these topicals.
Anyways if anyone is interested, check my log as i update it daily. Ill be doing alot of research and testing these next few days regarding yeast/fungus related stuff.

Hi Everyone!
Several months ago, I developed Folliculitis on my left cheek. It cleared after about a month, but in the last few weeks, I have had a crop of really itchy bumps pop up on parts of my face and neck. I noticed that this started after I applied BP in large patches to my face.
The bumps almost feel like poison ivy, but look very much like non-inflamed comedones. Some of them come to a head, and all of them feel a bit weepy. Anyhow, after doing some research, I figured that I probably have "Facial Candida" or Folliculitis caused by a yeast that is naturally occurring on the skin but that can become pathogenic when allowed the right circumstances. In my case, I think that the BP disrupted the bacteria/yeast balance on my facial skin and allowed the Candida to run wild.
Anyhow, my point is this: after trying to treat these spots like they were acne (which only made it worse, of course), I have found something that actually works. I've been using *wild handpicked/organic* OIL of OREGANO to treat it topically and internally. I'm also drinking Pau d'Arco tea, which is a powerful anti-fungal. The brand of Oil of Oregano I use is called "North American Herb and Spice", but you can do some research online about other options. It's pretty expensive, and it is NOT the same oregano you can purchase at a grocery store. That stuff has no anti-fungal properties. Read carefully about this before trying it. You will want to get either pre-diluted medicinal oil or oregano (it usually comes in a carrier or jojoba or olive oil) or get the pure essential oil and then MIX IT WITH JOJOBA OIL before applying it topically or ingesting it. It is VERY powerful stuff. I can't stress this enough! Even in my olive oil carrier, it burned a bit when applying to my Folliculitis spots, and it can be very painful if it gets in your eyes or is applied inappropriately (for example, please don't try to cure vaginal yeast infections with this... )
Anyhow, after 2 days of applying this stuff and taking a few drops sublingually per day, my Folliculitis spots have completely dried up and are on their way out! They no longer itch, either. Apparently you can also use cold-pressed organic coconut oil for topical application for skin candida infections. I haven't tried that one yet because this is working so well, but I might have some on hand in case I have to continue treatment -- it is best to mix up anti-fungals every few weeks so that the candida can't adapt!
Just thought I'd share, for those of you out there with stubborn Folliculitis and/or an acne-like rash or set of bumps that doesn't seem to respond to treatment!

No it doesn't always itch. I got it on my forehead, chest, shoulders and back and mostly it's just the ones on my back that itch(Far from a problem though and only happens occassionally)
Good idea to try the Nizoral shampoo, but I have much better experience with the cream so if you can get a cream with 2% ketoconazole(Nizoral, Daktarin gold...) you should use that.

Okay, here's my story. I'm fairly sure no one here is a M.D. but we all seem to have this crap in common so that gives us a certain degree of experience in the field. That said:

A year ago in early July I take a plane back to the States from Japan where I work. After I arrive I go sleep in my old bed which hasn't been cared for in a year. Back at home my parents have a great number of pets: 3 dogs, 4 rabbits and a cat. Any of these so far sound like possible factors in transmission? Two days later I get the first of what would become many, the @#%@^ed red pimple of doom on my shoulder. After bursting it like a normal blemish I was surprised to see it return that night in the same spot accompanied by another towards my chest. 24 hours and 4 bumps later making a suspicious circle around my neck, I decided it wasn't normal.
Went to a family doctor. Was diagnosed as staph infection. Was given 5-day Doxycycline plan.
Bumps cleared up on Doxycycline. AFter 24 hours OFF the medication the f#%@ers started to come back. I was given another 5-day pack. Again, went away, then came back. By this time I was now back in Japan. I went to see a derm. who turned out to be a pretentious @$$hole who sneered at the thought of, "Americans prescribing Doxycycline." He gave me a topical wash that smelled like alcohol. He said I was to used it for a week. 5 days later, without oral antibiotics, the stuff was all over my neck. huge blemishes of Biblical proportions. Frustrated I went to another derm. He gave me an antifungal cream to mix with the wash then apply. This continued to do crap and it just kept going BUT about 4 days later the blemishes stopped spreading and just started being annoying in their own respective locations before clearing up.
A week later I could enjoy myself at the beach.
Flash-forward *LOST whoosh sound*
Almost to the DATE one year later, on July 8th I took a flight back home to the US and on July 13th I was back in the doctor's office for the SAME D@MN THING!!!!!!!!!!!
I first saw a family doctor, told him my story, he put me on Doxycycline 10 days. Blemishes decreased but were still present. On day 8, about to run out of meds, I went to a derm. She prescribed a month of Doxycycline with the addition of evoclin (clindamycin phosphate) foam rub. Slowly but surely the bumps began increasing in number even on the meds. I continued the doses but 24 hours after the plane ride back it started to spread like wildfire on my back and lower back.
Back to another Japanese derm.
She put me on: Roxithromycin with some Vitamin B2 and B6 pills and a clindamycin rub.
Shyt hit the fan.
After just 24 hours on it it spread even worse, blemishes were refilling and forming in a matter of hours not to mention increasing in number and size.
So here I sit at 5AM typing as my last threads of sanity fade away before my very eyes.
I worked my ass off for a year to have a nice vacation and I see it ripped to shreds by being on Doxycycline which probably means 'How to give yourself sunburn without really trying' and now I can't even fathom putting on a swimsuit because I'm covered in staph or fungal-filled red shyt FOR THE SECOND YEAR IN A ROW!!!!!!!! It probably wouldn't be nearly as irritating if it wasn't timed EXACTLY on my summer vacation!!!!
I took myself off the antibiotics in case this is fungal. I'm subject to yeast infections as a male when on antibiotics and read that PF can be caused by yeast going insane. My bumps do not generally itch though. Now if it were fungal, it wouldn't have reacted to the Doxy right? Well at some point was there a bait and switch? Did it go from being bacterial to fungal related? I mean, if the antibiotics were working, EITHER OF THEM, I don't see how I could suddenly get this outbreak of the blemishes post my plane ride.
So folks, what do you think? Do I stop my antibiotics or despite the outbreaks should I continue with them and the rubs? It's like a damned if you do, damned if you don't right?
Please help.
Hey. It certainly sounds like folliculitis, however, the antifungal cream should have helped significantly. I currently use ketoconazole cream daily, but this only does just so much. I recently found that if I spot treat areas with selsun blue shampoo (selenium shampoo) and leave it on overnight, the spots are gone!! USE CAUTION however, as too much of this left on overnight will burn your skin and cause peeling. SPOT TREAT only.
My recommendation to you is to make (yes I had to force them) your derm pop one open and have it cultured. That is the only way to know exactly what you have. It only takes a second to do and results are back in a couple of days.

If you are going to use a shampoo I would first try Nizoral. However, both Head & Shoulders and Selsun Blue are also good alternatives.
When using the shampoo you should rub it on the affected area and rinse it off after about 5-10 min. However, a leave on product is often more effective than a shampoo, you could try Propylene Glycol, Ketoconazole etc.
You should post a picture so we can see how it looks like.

Updating on my folliculitis regimen -
Things are much better now - I get far fewer flare ups (mostly around my period), and when I do flare, it is very mild and clears quickly. I am clear of this most of the time though.
Right now I am using a 2% salicylic wash 1-2 times a day, and a 15% mandelic serum once a day. I occasionally rotate in other topical treatments such as propylene glycol and tea tree oil. I think that helps reduce the chance of product resistence. I think using this rotation method has been a huge help in keeping this problem under control.
I have also made dietary changes, which I also believe is a big part of what cleared this up for me. I cut out sugary foods (I have had almost no sweets in the last few months), and reduced my consumption of white breads and alcohol. I take antifungal supplements, which I rotate every couple of weeks and also take a probiotic.
Anyway, to anybody who has been unsuccessful at clearing this through topicals or prescription meds, you might try the dietary and product rotation approach. It has really helped me a lot.
Alright, I'm finally clear, which is why I've not posted here in ages.
All I do is let the foam from the head and shoulders classic clean shampoo I use every night run down onto where the PF is, leave for maybe a minute, and rinse.

Not had any breakout in a long time, not had a single PF spot in a long time, but still some strange discolouration which I've heard is associated with PF. I'm not sure if it's totally gone, but as long as I keep with the H&S, no reason to risk it
At first though, to get it down to being hardly noticable I had to apply a thin lsyer of H&S and leave it over night on affected areas. This would burn greatly, but did it every other night, exfoliating to remove the big dry patches of dead skin from the burns, and when I got clear, just used the foam in the bath to keep it that way.
Been almost 2 months now with nothing but H&S
Oh btw, I'm on tane aswell, but this method cleard me up before the tane, and I still use it now.
I must have wasted close to £1000, but this £2 bottle that lasts a month really works

Now here's my thing
My face was CLEAR until May 08, I got wicked high stress and anxiety that I have had the entire summer. If this is the case, then this couldn't be folliculitis, no? So I'm hoping this is nothing more than just clogged pores. (Advice appreciated)
As for my back acne, I've ALWAYS had a problem with it, but I just don't know...most of it is just these rough bumps that usually end up being hard plugs. Are these comedones or is it possible that this is indeed folliculitis? None of these are itchy however.
OK, new to the forum & found you guys after a google search on folliculitis. WOW am I blown away that we're all experiencing the same problems with our skin AND our docs!! Like many others in this thread, I have suffered for YEARS with this stupid crap. Jaganess, your photos could be my face & back - I am going to find a way to post some pics tonight when I get home so that you can see.
I do have a bit of a different story, though. Here's my info:
1. 37 year old female
2. Have lived in the hot/humid climates of the southern U.S. my entire life, never had an issue with PF or acne before 3-4 years ago, and have struggled horribly with it since then
3. Diagnosed with two different blood disorders within the past 2 years, one of them is autoimmune the other genetic
4. Currently see a hematologist for the blood disorders and a rheumatologist for the early onset of mild symptoms associated with SLE lupus (an autoimmune disorder that seems to be common in people with my blood disorders)
5. Have been to more dermatologists & have taken more oral/topical meds for this stupid skin crap than I care to admit. I've wasted hundreds & hundreds of dollars, and nothing has come close to even REMOTELY helping.
6. I am a competitive athlete, currently involved in multiple leagues playing tennis, and I am also training for a half-marathon (so I sweat daily... a LOT)
7. I struggle with my back, chest & the edges of my face most of all - all of which looks moderately OK in the morning & gradually works up in to an extremely itchy, inflamed mess by evening. The rash definitely spreads, as it's gradually moved to cover more & more of my back & chest since it first appeared.
So, here's where I am currently... About 2 months ago I took a fabulous beach vacation where I spent an entire week swimming in the ocean & relaxing, but panicked because I had forgotten all of the meds that my latest dermatologist told me were "essential" to clearing my skin (minocyclene, which was making me sick, and retin-a micro gel, plus a clindamycn foam type stuff). Lo & behold, after a week swimming in the sea & no meds, my skin looks better than it has in years and the nausea I'd been fighting for a month was completely gone. I promptly came home, dumped all of my skin meds in the garbage and decide that I've been right all along - this isn't acne, and I'm sick of the medications that seem to be making things worse instead of better. I'm now doing the following:
1. Salt water baths 2-3 times per week
2. In the off days, or mornings when I run & have to shower instead of soak in the tub, I use salt water dipped cotton balls & allow the salt water to air dry on the skin on my face, neck, chest & back.
3. I've ordered the ZNP Bar to try - I currently use a combination of Nizoral shampoo on my hair/back/face/chest, and Dial soap on the rest of my body.
4. I spray athlete's foot antifungal spray on the areas on my back before exercise & before bed, this seems to help keep things somewhat calmer, but definitely isn't curing anything.
I do have a derm appointment next month with a new doc, and am going to demand that she test one of the areas on my back or face -- I'm convinced now that it's fungal. My PF is quite itchy and painful around the line where my sports bra rubs, and moderately itchy/red everywhere else. I refuse to give up my exercise routine as it helps keep my blood/autoimmune issues at bay, so I am doing the best I can to deal with the skin issues.
I feel so VALIDATED to know that I'm not the only one out there struggling for a correct diagnosis and treatment. Why does it have to be so difficult?!! ugh...
Pityrosporum folliculitis is a common inflammatory skin disorder that may mimic acne vulgaris. Some adolescents with recalcitrant follicular pustules or papules may have acne and Pityrosporum folliculitis simultaneously. Clinical response is dependent on treating both conditions.
These patients demonstrate that follicular papulopustular inflammation of the face, back, and chest may be due to a combination of acne vulgaris and Pityrosporum folliculitis, a common yet less frequently identified disorder. Symptoms often wax and wane depending on the patient's activities, time of the year, current treatment regimens, and other factors. Pityrosporum folliculitis will often worsen with traditional acne therapy and dramatically respond to antifungal therapy.
