The nuclear option. It works reliably and provides long-term remission of acne, and even severe acne, in about ⅔ of people who properly take an adequate dosage. However, it permanently and irreversibly changes the skin and the body forever and users may experience long-term side effects for the rest of their lives. It’ll nuke it, but there is no going back.

awful
do NOT take this medication. It has ridiculous side effects and makes women infertile in most cases. Please save yourself the troubles.

What side effects are you referring to besides what you mentioned about it making women infertile?

I personally know a lot of women with kids that was on Accutane in the past. Only issue is not getting pregnant while on it.

Didn't cure my acne, but did make it more manageable.
I was on Accutane for about 8 months, from 2013-2014. Like many others, I had a terrible initial breakout that pushed a bunch of gunk to the surface of my face over the span of 2-3 weeks. I would wake up in the morning, and my breakouts would look completely different from the night before, because of the rapid rate at which Accutane was working. After the first three weeks, the breakouts slowed, and I started dealing with bigger, deeper cysts that would take about from a couple weeks to a month to go away. At some point, my derm put me on bactrim to help with the cysts that were still lingering. Once my course was over, my skin was pretty close to perfect. I had no more oil, no more breakouts - good times! My skin stayed clear for about three months, and after that it gradually began reverting to its previous state. At the time of this review, it's been about two years since my accutane course... my skin is super-oily and I have about 5-10 cystic types and 10-20 comedonal pimples on my face at any given time. Accutane did have some positives for me. It managed to clear my back for the most part, and I'm still at the point where I don't really need to worry about bacne. My facial breakouts are also not as bad as they were pre-accutane. Unfortunately... given how much the course cost me, how much I had to put up with over 8 months (eczema, constant dryness and itchiness, headaches, flushing, etc), and how quickly my skin reverted back to being oily and breakout-prone, I'd say my first round of accutane was a bust. I'm still open to going on a second course, but not anytime soon.

Often 2nd courses of accutane yield the worst side effects. Please research thoroughly if you decide to try again.

The best there is
I went on Accutane about a year ago and I had moderate to severe acne before starting my treatment. I also had moderate acne on my back chest and shoulders. I took a generic isotretinoin capsule once a day of I think 400 mg (maybe I can't really remember) for 6 months. It was quite a pain to start, I'm female and my doctor required I take birth control during the entire course of treatment even though I was abstinent the entire time. I had to get a hormone implant called nexplanon a month before I could start the treatment and since I might have to have my wisdom teeth out while I was taking it I had to do that too before I could start. But after all that it did work really well, my acne was completely gone by the time I finished the 6 months. My skin was very dry and I did have a few side effects, I had dry skin, nose and eyes for almost the entire 6 months plus a few months after I stopped taking it. I did not have any severe side effects and had no complications. Now almost a year after taking the drug, I still get pimples occationally but it's very mild, I still have some minimal scarring from before I took the drug but to my knowledge it was not supposed to fix that at all just prevent any further scarring. The acne on my shoulders, chest and back is completely gone and has not come back at all. I would recommend this drug, even though it was a bit of a hassle to start and had some scary possible side effects, my acne is almost completely gone and I'm very happy with my skin. I wish you luck if you have acne!

Did not work and have lifetime of irreversible side effects
I took this stuff about 18 years ago for 3 or 4 months and totally regret it. Lost my previously thick hair (started thinning, curling and falling out) within a year and never grew back (person who recommended it to me lost his hair too). Suffer from IBD so now on a lifetime regimen of Imodium to barely get by (totally affecting career prospects). Suffer from dry skin (which leads to premature wrinkles and aging people!). Suffer from some joint pain and probably other effects unknown or to come after all these years passed. I still get acne too. Be careful in taking premature reviews seriously! Now every once in a while I search on the side effects of this, just wish I had access to the internet info back when I took it.

Accutane Day 70
Hello Folks! So here I am on my accutane day 70. I started accutane on 10 Jan this Year. And today, I am feeling severely depressed because I have got a major zit on the right side of my cheek and I do not know if it is due to me being on my menses. What I know is that one more zit=more dark marks on my face that I will have to deal with for months. My dark marks are my biggest insecurity. Everyday I have to cover them with makeup and have everyone stare at me like why I am wearing tons of makeup in Summer when you are bound to sweat like a horse during the whole day. So, for last two weeks I did not have any zit. I stopped breaking out since week 8 but the zits I had beforehand are taking ages to go down. They are lingering on my face and turning real dark at the same time. At times, I feel that my face will always remain like that with zits popping out and leaving behind their dark shadows. And you see around people with clear skin, free, living their lives at 100% while I am just avoiding any social gathering. Even at work, I cannot talk to people by looking in their eyes. I try to isolate myself as much as I can so much my face looks hideous with all those marks and my lame makeup. It is one of the most difficult phase of my life. I have been through alot at 27. But, in this world where everything turns around looks, appearance, skin..every day becomes a struggle. To keep going, I watch other accutane users' videos on you tube or I roam in here to get some support and comfort. But it is not easy. At the point I am, I doubt I will ever be able to enjoy a clear skin. Even if I won't get any zits thanks to accutane, the dark marks will remain. I have been using multiple topical creams but nothing seem to fade them away. I used 4% hydroquinone, melano-expert, la roche posay hyperpigmentation lotion and so many more before being on tane..I even talked to my derm about it and he always seem to be prescribing me with more creams that I already know will not bring any difference. Lastly, I would say that it's been one and a half year I cannot go out without makeup and I wished I could go back to the time when I used to have clear skin just to feel again what it is to not have any zit or dark mark on your face. From a desperate tane user!

Sad to hear how your acne is affecting your mental health - I just wanted to offer some hope. Your story sounds a lot like mine, I would worry about the post-inflammatory hyperpigmentation left behind after a breakout - these marks were often a lot worse than the acne itself. Once my acne was under control with accutane, the marks faded much faster - a year after my course of accutane they were completely gone. Accutane didn't just clear up my acne, it improved the actual quality of my skin (softer, brighter, less inflamed, less oily, less red) so these marks were finally able to heal. I'd recommend a mild gentle AHA face wash and a good moisturiser to help speed up the healing process. Also, maybe ask your derm about azelaic acid and V Beam laser. Hope this helps :)

Thanks for your comment dear..Sorry for popping here so late! I am today on day 91 and I was pretty happy yesterday. Then, I saw a small blackhead on my face and I tried picking it up! Today I woke up with a huge big red scar!!! So hard to cover even with with concealer and foundation on..I do not know what to do. I am feeling desperate.

1 month in....
been on isotretinoin for about 1 month now and my skin started to have less redness although no actual acne was gone then another cyst on my left cheek and one under another on my right cheek so that sucks. my blackheads have been more prone to coming out for example before i could squeeze them and they woudlnt budge now if i squeexe them they come out...at least the ones on my cheeks not my nose... wihtin the first 3 days my lips were really dry and i started applying aquaphor which is a life savor btw.... 2 weeks in it looked like i was sunburnt and all my skin was peeling. IB is settling down..which happening by the 10 day mark i think...im not yet sure wether or not i would reccomend or if my current rating fits my experience but for now...we shall see.....

The only thing that really works for me
Well, I should preface this by saying it works WHILE I'm on it, and for about two or three months after, but I start breaking out again after that. This is possibly because I've only ever been on a low dose (20mg). Accutane works extremely fast for me, I experience clearing within two weeks, and often nearly perfect skin within 1 month. Yes I get a little dry skin and lips but it's not bad. I imagine if I took a higher dose it would be a problem. I've been on it three times for very long courses. I haven't suffered any health side effects from it or anything. I'm probably going to go on it again soon, because I just can't figure out why I keep breaking out. :( It's definitely hormonal, all on my chin and jaw and forehead. I'm trying DIM right now and will give it 2-3 months but if it's a no go, I'm doing Accutane again. It's extremely hard to live with a zitty face that I can't even cover with makeup .. you all know how it can make you feel like less of a person. My self esteem suffers so completely when it's bad like this. Thank you Accutane!

Hi! :) I just wanted to say, you are not alone and Isonoin will help your acne. It will be a little bit of a journey to get through but it will have positive results! I am on accutane right now too and I just started a week ago. This is my second time being on accutane (last time, there was a 50% chance it would come back after my first course, and my acne came back but after this course now my acne will be gone). Good Luck! :)

i started the 9th and i would love to talk since this is my first experience with isotretinoin and maybe getting rid of my acne,....

Month 4 no change
Last year I came off the combined pill due to migraines (not by choice) and after getting mini pill then depo provera injection I developed really nasty acne on my back and chest, I had the same issue years ago when I got the implant put in, I must be sensitive to progesterone only contraception. I was put on antibiotics but I know they wouldnt work as they didnt the last time, so after to-ing and fro-ing from the doctors I was referred to hospital dermatologist who offered me doxycycline (more antibiotics!) or isotretinoin. I have had zero improvement the last 4 months, I really think mine is hormonal as I breakout as part of monthly cycle. I had not had any hormomal contraception for a year but I am convinced the injection triggered my problems. Out of desperation I have bought the combined pill online as I have now realised this is what cleared my skin after the implant ruined it. I am onto packet number 3 (taking lucette which has same ingredients as Yasmin but £35 cheaper) and my skin seems to be improving so I personally think its the pill that is stopping the acne NOT isotretinoin. Also I am from UK and work so it is not exactly free, I have to pay prescription charges £8.20 a month as I am female so have to have a pregnancy test each time they give me it. I started 20mg, then 40mg for 2 months and now on 60mgs. Luckily just seem to have dry lips and skin as side effects with the odd rash. Hope everyone has better luck than me but I don't think its the way to go for hormonal acne

Finding a Doctor

The right dermatologist can make a big difference to your patient experience and the success of your acne treatment plan. Here are the steps to find a dermatologist who is a good fit:

  1. Use the search feature on the American Academy of Dermatology website to look for board-certified dermatologists in your area, and filter the search results for doctors with a special interest in treating acne.
  2. Read online patient reviews of any dermatologists you are considering and ask people you know whether they have any experience with these dermatologists.
  3. Do your research and go to your first appointment with questions prepared.
  4. Listen to what your gut feeling tells you once you see a dermatologist in person. If you are not completely comfortable, try a different dermatologist.

Finding a Doctor

Only a select few plastic surgeons specialize in acne scar revision surgery. Be certain to find a provider who specializes in acne scar repair and who is passionate and experienced in this area.

Be sure to:

  • Look at before and after photos, the more the better, especially patients with similar scarring to your own.
  • Be realistic about results. Look for improvement, not a cure.

Questions to ask a potential scar revision specialist:

  • Are you board certified? Be certain that they are board certified.
  • How long have you been performing these procedures? Normally, the more experience the better, however, some younger surgeons may be more on top of the latest procedures.
  • Can I speak to some of your other patients? Ask for references for several patients who had similar scarring and speak to them about the process and their satisfaction with results.

Red flags:

  • Their story changes: As you discuss different treatment options, if they tend to change their mind easily, or agree with whatever you say, consider this a red flag. A confident, experienced surgeon will possess strong, unwavering opinions.
  • Your gut tells you "no": Trust your gut. If you just don't feel that the doctor is the right fit, trust that and move on. On the other hand, if you feel they are the perfect specialist for you, trust that feeling.
