Best treatment for long lasting forehead scar?

Recommended Posts

I have some long lasting scarring on my forehead which has been severely affecting my confidence. I’ve attached some clear photos, there is one boxcar like scar which is about one sheet of paper deep and bothers me the most.

Could anybody suggest treatment options for this?

I have recently started using Tretinoin 0.025% + Clindamycin for blocked pores but not sure it will help with this wide atrophic scar?

Thank you!8F1B80A2-94EE-40E5-ACFF-C673C8A86E23.thumb.jpeg.d4cb5f6ddc21fc426a11b1502b650d80.jpeg8F3BCFD0-FB0A-4EF1-AD9C-C84110472F51.thumb.jpeg.4f5ab8b7bf0d1a050a543ec182c49e4b.jpeg32FAFA2E-5899-4D8F-B8FB-448CF7A16B04.thumb.png.5a92f29b9d809008a5725956a2d8de06.png

Yea.. so normally I'm very empathetic with minor scarring but, I really can't even see what you're talking about. Please don't do anything to your skin. It's just not worth the risk.

@jwebs91 I'd be glad to help you, ... can you take better pictures. I need shadows with directional lighting. Scars can be hidden in bright lighting.

I try to respond back in 24- hrs due to Dr Lim's endorsement and heavy traffic of those needing help.

@eyeback ... that's a good variety of scars, can be REALISTICALLY improved by60-80%
1. Manual vertical microneedling with micro Botox. If you hear a crunch its tethered, if not its atrophic.
2. Wait 2-3 weeks, retake photos.
3. Then saline only manual subcision with saline coupled with CO2 angled laser. 3-4 sessions. Can also do 25- 27 g cannula subcision horizontally or oblique. Saline only. TCA conservatively to deep scars.
4. What ever is left- DEEP filler to the periosteum - vvvvvv high risk, see some one who does this all the time.
5. Pick off the Pick scars with TCA CROSS.
6 Retake a photo - if you see the correct dude or dudess you will get a significant improvement.
It is a hard , you do need the right specialist, it will take 6 months , you will succeed .... I wish you well

@beautifulambition thank you. The best pictures I could get are attached.
I’d imagine this is a shallow scar if graded but it is wide with defined edges. It is very obvious in real life.

@jwebs91 Thank you for posting the pics. I don't see anything with the flashlight regarding the scar, ... see the FAQ - top post of scar solutions for how to take a flashlight pic (pic as example). Thank you for trying. I just see the light. I even looked at the square on the left of your forehead.

Is it depressed. Approx how big is the scar, are there large pores.
"there is one boxcar like scar which is about one sheet of paper deep and bothers me the most."

If it's a sheet deep that is not that deep. Do a few acid peels to soften the area and dermastamp it especially the borders. Do one treatment a month.

Use retin-a/differin/tretinorin at night.

Box cars can be lasered as well to the borders of the scar.

Hi @beautifulambition, thanks very much for your reply and suggestions.
The largest scar is indented slightly and can't be covered up by make up. I would say it's about a a centimetre wide (and tall).

You mention having some acid peels, would this be TCA or something else? Could professional microneedling help?
What length derma stamp would work for the forehead?

I am using Tretinoin 0.025% every night. Any treatment that could bring this shallow indent in line with the rest of my skin would be amazing.

I've attached the same picture but highlighted the area that is indented.

It also looks discoloured in normal light but not when a light is shined directly on my forehead.

Again, thanks so much for your expertise.
Joel.
