The red mark highlighted seems like blood under the skin. I am on Accutane now, so I know healing is slowed, but how can I get rid of this? It has been here for many months with no change. In addition to that, what is the best way to treat tiny icepick scars (you can't see them in this picture). Is TCA cross effective on tiny icepick scars? How about dermarolling or single needling? Is it at all effective?

My icepick scars are similar to this:

You can't do much about the first mark, because being on Accutane really limits your options. You have to wait it out and see what happens.

TCA CROSS is effective against icepick scars, but it depends on the depth, if you can tolerate TCA itself or what you want the "down time" to be. Fraxel lasers are also pretty good with icepick scars.

Dermarolling is really mild compared to lasers. It takes several treatments and over a long period to see results. Dermarolling is superficial. I wouldn't perform the procedure on myself because there are varying needle lengths, is highly technique-dependent, and you can't apply certain topicals right away.

If I were you, I'd wait until you finish your course (and an additional 6 months, but at least talk to a derm first) before you go for any kind of scar treatment.

I am definitely going to wait atleast 6 months after finishing accutane, I just wanted to figure out what the best method would be for treating my scars after. Are there any other alternatives to fraxel?

Punch excision for smaller scars and grafting for larger scars are also options. Medium-depth chemical peels Jessener's + TCA is sometimes used. Dermabrasion is more rarely used.
