Am I purging or just breaking out?

I started my regimen on August 26, 2023. My dermatologist prescribed me with 4 products, Tretinoin Cream 0.05%, Benzoyl Peroxide 5%, Sunscreen SPF>30, and mild facial wash (I bought Physiogel - Daily Moisture Therapy | Dermo-Cleanser). First thing that I did is I cleansed my face and damped with a soft towel make sure that my face is completely dry. I did the sandwich method, wherein I applied moisturizer on my whole face and wait for it to dry for at least 20-30 minutes, and then I applied a pea size amount of tretinoin cream on my whole face and I waited at least 20-30 minutes before applying anther layer of moisturizer on my face to lock in the moisture because I'm afraid that the tretinoin will cause dryness in my skin. When I woke up, I noticed that my face is experiencing a little redness and some tiny bumps appeared on my cheeks and forehead where I usually breakout and instead of experiencing dryness I experienced oiliness.

Note: The first 2 pictures are the status of my face before using tretinoin (August 26, 2023)

The last 2 pictures are the status of my face after using tretinoin for 1 week (September 3, 2023)

Purging is not backed by data so I doubt it's that, but there's a small chance it could simply be that.

A few thoughts on what else it might be:

  • Scrubbing the skin too much or irritating it in any other way should be avoided. Make sure you're always using a featherlight touch. I have seen skin very much like yours (after pics) from irritation or over-scrubbing.
  • Sunscreens can break people out pretty badly. Very few are non-comedogenic in my experience. Perhaps you could try wearing a wide brimmed hat instead of sunscreen for a while until you get clear and then add sunscreen into your regimen and see if you stay clear? You do need to protect your skin, particularly as you start on tretinoin, but I worry that the sunscreen might not be great. Which one are you using in particular and do you have an ingredient list you can share for it?
  • You may be using too much moisturizer. Two layers of it sounds like you might be overly occluding your skin and clogging pores. Again, which moisturizer are you using and can you share an ingredient list?

Can't really tell you should ask your derm. However, I really recommend the new Accure Laser for Active Acne. I tried it because I was hesitant on taking Accutane cause of the side effects. Definitely helping a lot. I've only had 2/4 sessions so far and I can see a lot of progress.
