Help with horrible acne

How do I treat this? (I do have a little concealer on)

That looks like closed comedones. Closed comedones are basically whiteheads that haven't fully come to surface. They usually release a tiny white string of pus when you squeeze them [but please, don't squeeze/pick to remove them!]. Closed comedones are sometimes the result of or made worse by pore clogging products like heavy washes with fatty alcohols or foundation. It's called acne cosmetica. What brands are you using, and how do you cleanse your face?

There are a few options to treating CC's. Some people really love retinoids like Retin-A and Differin cream/gel, though they take patience because they can cause some purging at the beginning (where acne initially looks worse as your pore squeeze out all the junk inside). As long as you don't get cystic or nodular acne and have super sensitive skin, they could be a good option. CC's can also be the result of a hormone imbalance [happened to me, from Spironolactone, so I definitely know it's possible and how frustrating it is]...if you aren't on BC you could alway try that. Good BC pills work by reducing sebum/oil production, which prevent CC's. Here's a Masterpost of the best BC pills for acne. :)
