I have this deep boxcar scar next to my lip. It was from something called granuloma annulare that was a super bad dry patch and that is how it healed on my skin. I have had this for 2 years and I notice it is starting to worsen over time. I have tried microneedling as well as a Venus Viva NANO RF treatment.
I had a virtual consultation to discuss subcision with filler. She suggested performing subcision and filling it with Restalyne. Any advice on if I should go through with this? I know this is super deep and tethered¦ thats why I think I need subcision. I know it typically is best for rolling, but sometimes boxcar. I am hopeful/curious that it will release the deep tethers to the underlying structures. Any advice???
Microneedling typically does nothing in my opinion. Scar revision or excision would leave a worse result. Subcision/filler might improve it or it might not. It's probably the best attempt for something like that but I always view minor stuff as being is it worth risk vs reward for minor stuff. Just find an experienced doctor who is good with needles.
Subcision makes sense, but it only helps a bit. But at least that's a way to permanently help it a bit. And yeah I agree with @harmlessboy1441 that you want to find an experienced doc who's got good hands and knows what's what.

Do you think combining the subcision with a filler like Restalyne would yield better results? During my consultation, she sounded confident that I would get good results with using a filler with it. She said this is something she typically does and it yields the best results.

Well as I said it's probably the best choice if you decide it bothers you enough to treat, as I can't think of anything else to do that will help. I do agree that you could throw in a little filler after some cautious spot subcision. I don't know much about restalyn except for what I've read. I think it absorbs a little longer than Juvederm. And the amount would be very small given the size of the scar.
Basically if you decide to do it I would just do cautious subcision gently, throw in a dab of filler I guess.
