I have had this pitted atrophic scar on the side of my lip for 2 years now. It was not even from acne. I had an awful breakout and went to a derm and they said it was granuloma annulare. They gave me a steroid cream, took forever to heal, and it left me with a deep pit in my skin. It has been getting worse lately and I am extremely self conscious of it. I have tried 2 microneedling as well as a Venus Viva Nano RF treatment and saw no improvement at all. I was wondering if subcision with filler would help this or even just filler with PDO threads??? Help. Sadly there is nowhere near me that offers subcision, though.
how noticeable is this??

Topical steroids are nasty business imo. Messed up my texture like crazy at the collagen level.
Even if there is tethering in the scar? Filler without subcision?

Depends a plastic surgeon would have to assess the specifics including the tethering but it's very small. Meaning it should not involve massive amounts of pondering. I don't think anything other than subision and filler would really be a choice. If you excise it, it would be worse. Needling is not designed well, lasers are risky for such a small thing and so on.
Everyone who asks is doing the right thing to pursue possible treatments but the real move forward is to get 2 opinions from good plastic surgeons then come here and say "they suggested... What do you think" after asking them about filler etc.
