Hello All!
I was told by a dermatologist the scarring is moderate. As you could see in certain lighting it looks very harsh, other lighting it appears scarless and smooth, and when skin stretched it looks like its gone.
What procedure would you recommend:
Subcision, or a modality consisting each session of bio revitalization - needling - TCA peel - PRF (same as PRP but way more potent) ?

I think a lot of scarring is "gone" when you stretch the skin because it levels out the imperfections. I know when I do it does.
It's very rolling-ish. I would suggest a experienced doctor with subcision/filler. You're not going to be able to shave or zap your way to volume.
Might want to test a few spots since there's a bunch of spots to see how it goes. But yeah that's what I'd suggest. Things like needling or anything will not do a thing.

Ive never liked the idea of fillers at all. How did it make your skin a mess?

Im totally against fillers. Just does not feel right, and would like to approach this any other way. I consulted an esthetician who said a combo of bio revitalization/tca/needling and PRF per sessions would do while approaching it all from the inside out diet wise.
I had an appointment with Dr Rullan as well who said it was moderate not severe scarring and suggested subcision/PRF and suctioning afterwards. Im undecided which route to take as Ive heard people end up with worse scars from subcision with Dr Rullan afterwards, and also heard people with pitted scars heal overtime with the prior procedures.

Was it the result of excessive/often fillers? Or maybe just a bad outcome? I'm not against fillers I just think it has to be accompanied by other goals/techniques because fillers can't be used forever.

Well the kind of scars you have just can't really be shaved or nuked with energy/heat to help. They are rolling scars. There is fat loss or imperfect healing/scar underneath the epidermis. I'm not sure how you get at that without a filler or subcision attempt. Unless anyone else knows. Sure I think there are risks. I always tell people to try 1 or 2 spots with things like TCA cross or subcision/filler. This way if it goes awry, you know you only screwed up one spot moreso versus 10.

Never do any acid at home and TCA cross itself is very hit or miss. You could end up getting wider/deeper scars. Or others have noted improvement. Always go to a generally decent plastic surgeon and demand they only test a spot or two first if you choose TCA cross.
But your particular scars are mostly rolling. You don't do TCA cross for rolling only icepick or boxcar in certain cases.

Yikes! Had no idea. Just noticed people casually doing a low percentage across the net. Thanks for the heads up. I was under the impression according to the net and a highly reputable esthetician that Microneedling with PRF (not PRP) would yield beneficial results overtime for rolling scars. You had mentioned its pointless? Im really nervous about subcision causing more scars according to several posters here who had it done by Dr Rullan. Is there a way to avoid that?

Your nervousness is warranted as all procedures have risks vs rewards, the goal is trying to get the reward worth more than the risk. Microneedling imo may be "worthless" for certain scarring or alone, but PRF or other types might be beneficial especially with other things. I'm not a huge fan of microneedling because I've only mainly seen cases that did well combined with other things.
TCA cross as in dotting scars with a toothpick is designed for icepicks/boxcars but it's either very good or very bad apparently. Doctor being careful might have a lot to do with it or people's dispositions. Also, I don't care if you have to pay double or triple, don't let a doctor do it to 20 spots. Do 1 or 2. You will not want 20 widened spots vs 2. But rolling scars are not for TCA cross.

subcision should be your first priority..
your skin is smooth when stretched, the scar bands attached to the skin is what is giving your cheek that look, subcision would cut those bands and lift your skin up
there is no fat loss, it is just the bands holding your skin down

I don't agree it is fat loss, the look is from the scar bands pulling the skin down
I just sort of fixed some of my exact issue on my cheeks using subcision method and it lifted the skin up which originally looked like someone cut a deli slice of meat off my face.

Yes its smooth when stretched. Also in certain lighting it doesnt look like I even have scars, other lighting/harsh lighting intense.
But, if there is fat loss, what does that mean? What would be the right procedure?
Who did your subcision? Did you avoid fillers? And how many have you had?
