Atrophic scar in the nose

Hello,

Six months ago the doctor remove an actinic keratosis of my nose. He was very agressive doing electrocautery so I have now a very obvious atrophic scar in the middle of my bridge nose. I think the scar is not tethered since it can be considered kind of surgical scar.. I got three fractional co2 lasers in the scar and 30% ablative in the borders that maybe helped me to sand the borders but the indentation is still there with not much improvement (last treatment was 2 months ago). After the second fractional treatment, I suggest the doctor to change to a fully ablative one, but he didn´t want to do it, he said that it will make the scar look worst so he performed another fractional one. He suggest me to perform and aditional non-fractional 1540 laser that still i didn´t do. Should I go for it, since I already paid for it, or is not worth and can make things worse?

I know scars on nose are difficult to threat, I was trying to find info in the forum before posting my case. Any suggestions of how I can improve the scar? Does filler will help in that part of the nose, or still the borders are very obvious? I think I have a deep lost of tissue and fractional laser itself can not repair the hole.

How about any medium TCA chemical peel, will it improve the texture? I am scared about phenol peel since i was reading it takes a long downtime to heal, and I am still fighting with post-co2 redness (despite it is not very obvious in the pictures, the scar it is more red in real life).

I have some acne scars, but I accept them pretty well, never bother to me as this scar is doing, please help.

Not sure how if it works on the nose, but I think a filler would help a lot.

My first thought was filler as well, but I wouldn't take anyone's word for it on the internet. But if you've already tried laser that many times, maybe it's time for something else.

Since you already paid for one more fractional, perhaps you may as well do it. Would it make it worse? I don't see why it would, but what is your concern there?

Also, there are many doctors out there. Make absolutely sure you're with one who specializes in scar repair and has tons of before and after pictures and is someone you feel really comfortable with. For little repairs like yours, you need someone who is really an artist at this stuff.

I will definitely go for the filler, but i will do the non-ablative 1540 first, to see if i can get some additional elevation of the scar. I was little scared because Iheard about some cases of fat loss with this laser.

It is very hard to find derms with good before/after pictures in Spain, but i found a doctor that gave me more realistic oppinions, not making fake promises, and no pushing me for lasers or any treatment, so my plan is:

  • non-ablative 1540 in the following weeks with my previous derm
  • Medium peeling for trying to sand a little bit more the borders and improve the texture of the scar by middle of May with the new derm
  • HA Filler by middle of June with the new derm as well.

Do you think it can work?

I will keep you update if i get some progress

I have no clue not familiar with these types on nose, but doesn't look bad at all tbh.
