Hi. I have been through some procedures including lasers, TCA and a bit of subcision (I didnt tolerate it well, since passed out twice). My dermatologist has suggested me having a ablative laser treatment on my cheeks instead of subcision. Subcision, however, is still a possibility since he would be able to put me into a mild narkosis and cut all my scars (the procedure would also be a lot more expensive).
My question is then: isnt ablative laser most effective for box car scars, and subcision for rolling scars? I cant figure out what I should go for, since I think I have both type of scars. Can you please help me identifying my scars?
Also, I am currently on isotretonin, so the procedure would have to be done a long time in the future.
while you on Accutane you can't do nothing but subcision. What do you mean you passed out? You have a fear of needles or something? Subcision with fillers will help you. Try subcision without fillers first. You look young so you have a great chance of producing your own collagen. Then try some fillers.Everything else (lasers, etc.) will be beneficial afterwards.
About subcision: you need NOKOR or Taylor liberator, btw. Couple of strong subcision sessions will help you.

My derm told me that my skin had lifted so much from earlier procedures that my deeper scars would be difficult to balanceout with subcision, which is also why he suggested me having an ablative laser treatment. The order you should treat scars are
1. rolling - subcision
2. ice picks- tca cross
3. Boxcar- tca cross and ablative lasers
4. Fillers

I really think subscision (just based on my reading not personal experience) is something that is your best bet. The laser would probably tighten and/or blend some edges and things like that but you have texture/fat loss that needs things like subscision/filler in my opinion. I don't know if sculptra works or if just numerous subscisions is better but things that build collagen is a necessity for you. I had far different scars as a kid your age and I personally wouldn't have done dermabrasion with scars like yours. I think it won't blend well.
As for passing out, I was like you when I was younger. But you must conquer your fears. That usually takes going through things many times OR just getting older. I went to the hospital the other week and got 3 vials of blood taken out after an issue, for testing. 20 years ago I would've probably been really nauseous now I don't even flinch. Tell your brain this instead, the acne that caused damage was far more "painful" and damaging than any needle will ever be. Try to think logically as it's happening. Try to remind yourself that people have literal far worse things done and don't even worry.
Anyway, beyond that, I think you should seek plastic surgery rather than Dermatologists depending on if you can budget-wise, usually dermatologists don't have the same experience in cosmetics. Back in the day I even got my cosmetic stuff covered/insurance the plastic surgeon put it through at my request and they approved more severe cases as "not just cosmetic" so if you have a long road ahead might be something to give a try.
I personally don't dislike laser just not while you're still too rolling or fat loss. Usually getsmart is pretty logical with cases with rolling or fat loss things. I'm terrible at it because I hate telling people things I haven't seen direct results from and only read about good outcomes. But yeah my guess would be do subscision a lot and try to fill and build collagen. Then talk about re-shaping edges and stuff. I don't think the laser is the right time.
