scar treatment advice

I used to be active on the board many years ago but forgot my log in and not sure I have access to the email so starting anew. 

My story, I™m in my 40s and had acne as a teenager and didn™t get to see a dermatologist until I was 17, at which point I already had a lot of scarring. I did the meds and did accutane when I was 19 and continued with topical treatments off and on for the last 25 years. 

In my late 20s I saw Dr. Sire in California for saline injections, they looked good right after but unsure if they did anything. In my late 30s I saw Dr. Novack a few times and I feel like maybe it helped, but still all I see are scars when I look at myself. In my early 40s I went to Tribeca Skin and had micro needling with PRP in 2019 and 2020. Now that the pandemic has subsided I would love to try to tackle things again. I know I™m losing collagen at this age as well, but any advice on what I should try? I™ve never done any laser, but the couple of doctors I™ve asked about it have said it wasn™t worthwhile, but it™s been a few years so maybe there is something better?

I™m currently living in NYC but will be moving to Los Angeles later this year so open to doctors in either location if they do something that the average dermatologist can™t do.

Your scars are really really mild. Be happy with that kinda skin compexion in your 40s :)

Appreciate the responses, but all I see are my scars when I look at myself. They have effected my entire life and when I look back its the biggest thing I wouldve changed, to beg to see a dermatologist earlier.

That's the nature of scars, but yours aresuperficial. I would not suggest any procedures that could worsen your skin dramatically in the pursuit of perfection. I would say you could try 2 days a week of tretinoin as JC suggests, it's a retinoid that just speeds up cell production. I can't tell well from the photos but the right side might just have more texture type issues.

Only other thing I'd suggest would be a peel but I have no clue about those. But that's a textural thing. Any deeper indentations you could try some light subscision if others agree, no clue if that would help on superficial scarring.

Honestly if you're living in NYC and LA that's probably contributing to your worry about it. LA is a cesspool.

I would do Genius RF. It works for shallow boxcars. If you're in California and have the financial means I would also just do the phenol peel and move on.

Your scarring is mild but I can see why it's affecting your mental health.

Treatment is straightforward: phenol/TCACROSS + subcision. You also have age-related volume loss that can make your scars look worse than theydid in your youth. For that, you can get HA fillers (temporary but dissolvable, can migrate), Sculptra (longer lasting, builds your own collagen, but some people have a bad reaction), or fat transfer (expensive and less predictable, but lower risk of adverse reaction).

Avoid doctors trying to sell you full resurfacing treatments. If you're going to LA, there are some doctors that have been recommended on this forum (eg Hazany). Harder to find doctors in NY, but some people on this forum have had results with Tatiana Khrom iirc (can't speak for her myself).
