acne scars

Hi everyone,

    As you see in the pictures I have an unfortunate linear acne scar under my eye and some other on the cheeks. I have had 2 microneedling sessions done ( I could not get 3 sessions because I got sick for other reasons ) and had a good results. Meantime, I used tretinoin for 6 months but during that time I only used on the cheeks and did not directly apply to that linear scar for some reason.

  So, my question: is direct application important and is it worth  to use tret for another 6 months for that scar and overall. Would it make any difference?

Look into subcision. The filler.

Maybe someone who has had TCA Cross in a paint manner could provide their experience.

You can consider excising the scar but you will be left with another scar - hopefully a flatter one. There are no guarantees with this.

I have another question: Would you recommend subcision + filler alone or subcision + prp ( or mesotherapy) and co2laser? Thank you for the answer.

I would recommend subcision, filler, and TCA Cross.

TCA Cross would be for the icepick and narrow boxcar scars. You have a small scar under your linear scar and maybe some one your cheek but I can't tell from the pictures.

If your cheeks scars are mostly flat and you are comfortable with the risks of lasers, I would consider fractional CO2 later on down the road.

I showed these pictures to a dermatologist and he said '' there is no treatment option available for you other than subcision and filler.'' But, I am having second doubts whether filler is really necessary. And, I wonder if it is worth to try a combination treatment like subcision+prp+ dermapen or lasers. Is there any chance for subcision to work without fillers? I am asking because I will have to travel to get subcision + filler.

Lasers sounds risky ::((  I am quite indecisive right now.

You can try subcision without filler and see what results you get. I am not pushing for filler but I thought you wanted that. I think it will help a little though. When I got subcision, my doctor said I didn't need filler. Some people do though.

I am not so convinced about PRP's scar improvement benefits. I think it helps you heal faster, if anything. When I got Infini with my plastic surgeon, he said PRP was bullshit. But maybe the science is different now. I didn't have the money to spend for PRP anyway.

TCA Cross can widen scars at the same time making them shallower. I haven't noticed any widening from Phenol Cross. Maybe for one scar but it could have been the redness that made it worse. TCA is less safe than Phenol Cross but it's probably the best treatment to get rid of the harsh scar borders that lasers can't. 

I did a dermapen session and it messed up my skin giving me new scars. I'm not a fan of microneedling but some have reported results and doctors such as Rullan use it in his package of treatments. 

Lasers are risky! Don't do it if you don't want to deal with possible new scarring and hyperpigmentation/erythema. Go with what you are comfortable with. I am overall satisfied with my fractional CO2 treatments despite those side effects.

In what order do you do this?

im guessing its best do a couple sessions of TCA Cross first, then subcision, and then filler

But....can you do TCA cross after having fillers? just wondering if the TCA cross can mess up the filler after its in your skin

I would love to see results without filler. And, I saw some paper on the internet saying subcision + prp is effective but yeah it is dubious. I guesssubcision won't harm if I try without filler and have no positive results right?

Actually, I have still redness on my scars ( maybe erythema)because I did a horrible mistake and did not use Accutane 1.5 year ago. Following that six months, acne go deep under the skin and pressured a lot. Luckily, with the dermapen sessions, my skin peeled the bacteriaand %85 of the pressure gone but there is still some. There are also mild breakouts but they dont cause scarring.Do you think I should adress that first? And, is chemical peeling best option for that?
