[PIC] Need Advice for my acne scars

Above are my acne scars. I had cystic acne from 15-37 (present). It's well under controlled now. 

Some of the above scars are caused by ultrapulse laser which I stupidly went to Korea without sufficient research (relying on blogs/forums). I only had 1 session with them.

Now I hope to reduce my acne scars after taking a hiatus from treatments for 3 yrs.

 

I have been taking care of my skin by using sunblock religiously these past few months.

I did my research this time and found a board certified dermatologist who has extensive experience in lasers and surgical procedures in my country.

On my part I have come out a plan to tackle my problem effectively.

 

1) Subcision to fix my tethered scars to bring them up to the surface. How many sessions will you think I need?

2) Do fillers like sculptra to prevent tethered scars from reattaching.

3)  not sure whether subcision combine with TCA cross or fractional co2 laser would be best for my scars. I am depending on the dermatologist's views when I see her. Need you expert advice 

4) Should I top up with 1 ablative co2 laser at the end of the course

 how do you rate my acne scars? I feel like dying I've been suffering from depression this past 3 years. My gf dumped me. Couldn't sleep during this period and this affected my quality of life. I really thought of suicide.

my self confidence is at a all time low.

Your scars really ain't that bad tbh.

If you feel like they are really bothering you, you can try applying cc cream to conceal them when you go out.

If you want to do subcision, you can consider using the cupping method cos fillers do come with several cons. Cant really comment on the rest since I only recently started my treatment as well :)

I have also ordered the cupping. Waiting for it to arrive. I don't think i will want HA fillers as they are short lived (6 months) 

Sculptra doesn't help much in the appearance of scars I read. Only a little help. What it does is improve the overall face instead.

So I'm not sure which filler I want to go with.

I thought I'd rejuran but don't think the  dermatologist has that. Currently only Aestheticians in my country provide rejuran injection but I don't feel safe letting them do subcision on my face.

Besides they do not have good laser machines. Also i do not know how proficient they are with those machines.

I had enough bad experience in Korea.

I read that rejuran helps in wound healing so that's perfect for me.

As for the cc cream think it only helps if one has superficial scars like erythema or marks. It sure won't conceal my pit holes

I feel my acne scars are very bad. Im suffering  from depression over it

You mostly have complexion issue rather than scarring its very minimal, you dont have bad scarring at all. Meaning you dont have indentations. You have tinysloping scars that can be fixed with filler and will give you the confidence you need without going into aggressive procedures.

Bellafill is semi-permanent but you should get temporary fillers before considering it. Subcision is a good way to see if your skin will level up.

Hey, I think that you have got nice skin. You worry too much. I have no reason to tell you a lie. I tell you like it is.

My biggest concern is that you think of suicide. Please don't do it.

Thank you for your kind words! Just visited the dermatologist. She say my scar is rolling scar no ice pick or box car. She will bedoingsubcision and fractional co2 laser

Then chemical peel to improve complexion at end of the course

Maybe, these pictures don't show the scars the way you see them, but they look pretty damn shallow. What makes you believe they are tethered? I don't think there will be much improvement from subcision.

No, you need to use HA fillers. Sculptra will get absorbed by the tissues and there will benothing left to act as a "spacer". But then again, I don't think subcision will provide any comfort

Only time you should get TCA cross is when you have show a prominent scar edge like icepick or deep boxcar scars. You have none of those. As for laser, you said you've already had one in the past and it showed little to no improvement. So it makes no sense to get these.

 

Without any prominent scar borders, you're a perfect candidate for dermal filler. I would initially get temporary filler like Juvederm and see how you look. If you like the result, you can then proceed with permanent filler like Bellafill.

I went to  another dermatologist for second opinion as the previous did not use angled lighting when assessing my scars. 

Turns out I have boxcar and rolling scars. This derm says she does not use cannula needle but the normal needle to do subcision.

I asked both of the derm they say  they don't do fillers for subcision . Can say they are pretty risk adverse which i expected.

She says complete 2 subcision first and see how it goes 

 

She says the subcision will go with ematrix laser. Ematrix will improve my complexion 

I chose this derm because she uses angled lighting and can see she has much more attention to details 

Did my research she is proficient in lasers and has alot of them in here clinic.

I'm abit depress she told me subcision might not improve my scars much..

Hey mate, I can understand your skin concerns and desire to improve them. Compared to other scars, yours might look shallower and not as many but I definitely can see them all types of scars and you wil need many treatments to improve them.

I can't understand how peoplecan be so insensitive and dismissive of your issues. Every person with acne and scars suffers no matter how serious the problem and from what I see you've dealt with acne for a very very long time. Iunderstand why you're feeling depressed.

Good news is that your scars are moderate at worst and you can improve them a lot. My advice is to take better photos at angled lighting and post them on realself for dr's opinion. You can also send in photos to Dr. Rullan who is kind enough to offer help per email. Good luck mate and cheer up. These scars can be treated with great success.

I Understand where you coming from. I'll be strong in this long journey. I'm determined to improve my life now. Previously I just thought basic cleansing,moisturizingand acne creams are enough. It's not. I've learnt thatDermatologistsexist for a purpose.

I'm now more determined than everto spend every penny from my bank Just to improve my skin.

My gf dumping me makes me woke up and drives me to be determined. It's a painful experience.

These scars and cystic acne I've gothave dealt alot ofdamage to my self confidence.

I went to look at ematrix before and after photos some ofthe before, theylook like the scars I have.

I have full confidence in my dermatologist now. She specializes in lasers and I understand why she chose this laser.

Also, I'm starting on tretinoin now after she prescribe it tome (takes 6 months to see full effect)

I just saw the before and after in other forums (r****t) it works beautifully after 6 months.

I will post my updates here after treatment.

Yeah mate subcision with Ematrix could give you very nice improvement. Some tca cross on the few ice-picks and boxcar scars and you are good to go. Just my two cents. Oh yeah tret topically is great, also wear sunscreen it really makes a difference.

I agree that subcision does not seem like yourbest bet as your scars do not appear tethered whatsoever. Maybe you could show your skin in angled lighting.Be very careful with the doctor you select because your scarring is very minimal and procedures like lasers, cross, subcision come with complications that can make what you have worse.

Also keep in mind that so many of us here would kill for your skin. That doesnt mean you shouldnt fix it if youre unhappy with it.Your before photos are what a lot of us would love to have as after photos. You have pretty goodskin.

Most acne scar revision procedures have risks, in your case I feel the the reward is not outweighed by the risk since your scarring is very minimal. I would just use some concealer if you are feeling too self concious.

Hi guys just an update 

Had my first subcision yesterday. The photos are taken 3 hrs after the surgery.

The areas are still bluish black I did not have any fillers. The derm says the blood will act as the spacer.

I will proceed with suction after a few days to prevent scars from re tethering

Right now just apply sunblock and moisturizer waiting for it to heal and

The derm applied local anastheticfor my cheeks.she injected several doses on my cheeks. These are the painful part. Not sure why she didn't use numbing cream.

guess she missed an area for LA whenshe subcise,I can literally feel the pain when she is fanning the area.

While she is doing the job, can hear the satisfying snapping sounds.

This is only my first subcision.didn't see any improvement so I have no feelings about it.

My boxcar scars arestill there.

Will be going for my 2nd subcision after 4 weeks.

Right now waiting for my skin to heal and will be going for ematrix 2 weeks later

I noticed some white flakes. What are these? Is this normal? Will subcision cause peeling?

I developed Hematoma on day 3. After rubbing an egg over it my Hematoma has healed considerably. Today's my day 4

Within a day the Hematoma is starting to disperse

The nasty bonds held down my scars and made a deep pitted hole on my cheek. Subcision elevated it to the surface. 

I had done alot of laser procedures prior but did not improve the pitted hole one bit. It was money down the drain

Currently I developed Hematoma. I hope it will go away soon
