Acne scars

Hi guys.

I have been struggling with acne for about 4-5 years. I tried many ointments but did not work upto the expectations. I have heard of laser and microneedling works well on acne scars.

I need help to figure out what treatments do I really need to improve my scars?

Monthly tca cross for at least 4 to 6 months should straighten that out. Try to go to an expert in this method then finish off with laser

Thanks for replying.

I consulted a dermatologist few days back and he recommended me to go for 3-4 sessions of Infini RF microneedling. I am confused now.

Your dermatologist wants to pay for his new expensive car.

Most people on this forum would suggest TCA Cross. You can consult with another doctor who deals more with scars instead of general dermatology.

If you have the money, go for Infini but I believe the results for those scars will be very minimal and disappointing. I had it done for boxcar scars and I didn't see any changes.

As I wrote before I dont understand science behind this Microneedling Radiofrequency when it comes to acne scars revision at all, can someone please explain this to me ? It doesnt work for boxcars, it doesnt work for icepicks, it is supposed to work for rolling, but best thing you can do for rolling scars is subcision +filler anyway. I personally thinkif you want skin rejuvenation go for it, but for acne scars its waste of money and time.

Actually I searched online and no derma performs TCA in my locality.

Btw what do u suggest? Subcision+filler in my case?

Your scars seem superficial and not too deep, mostly textural; from what I can tell, mostly linear and icepick scars? One prominent boxcar on your right cheek. They don't appear to be rolling/tethered except maybe a few on the left cheek, nor deep, so I wouldn't bother with subcision and especially not filler, unless you have difficulty treating these ones I highlighted. I've had sub on such scars (the 3 highlighted below) and they cleared right up.

I agree with the TCA recommendations, however in the meantime I would get an Adapalene (Differin) script or even Tretinoin, they should help these kinds of scars overtime.

You can do the INFINI if you want and if money isn't an issue for you, but its probably not the most efficient, especially cost-efficiency (unless your derm is an exception). It could help, sure, but its a gamble. I have no experience with it though. If you have no other options though, go for it imo, as these shallow scars, theoretically, shouldn't be the most difficult to treat.

Thanks for suggesting me with so much clarity. But I'm concerned with my temple area too- specially the left side. It has big prominent scarring.

Though I'm visiting another dermatologist next week.

No worries, just remember that dermatology is a broad field and there's so many areas one can specialise in. If you are not visiting one who either specialises in scarring, or has been reviewed here (or elsewhere) for scars, or doesn't have essential scar treatments listed on their website (like subcision), then save your money and try and find one first.

IMO, an easy trick is to google subcision + [your city]. If they perform subcision, they are legit. I'm not saying you need subcision (as per my last post), but any place that offers to treat scars without offering subcision may not be worth going to, at least IMO, though granted it can be hard to find doctors who do it. Even your city + TCA instead.

Similarly, google your city + acne org might yield results of recommendations here.

If you want to list your location/city, or how far you would be willing to travel, I/others could try and recommend some here.

You have to find doctor that will examine your skin and identify your scars. He must tell youwhat type of scars you have and then start to treat them . If you have a problem in your life and you want solution , first you need to indetife the problem. If doctor wants to treat any health problem he must identife it - set the diagnosis, without that its useless. Any doctor can look at your skin, tell you that you have scars lets blast you with laser or Infini or any other energy device , but specialist will use different treatment for different scars .

If you have tethers - you need subcision. If you have Icepicks - you need cross technique. Simple as that. TCA CROSS can give you improvement, lasers can give you improvement. However people here are not doctors, doctor needs to see you in person, ask him , do i have tethered scars or not ? Do I have Icepicks ? Rolling ? Boxcars ? Let him take pictures under angled lightining . You can together take a look at it and consult what he is gonna do about it. If you have Icepicks nothing is gonna give you good improvement but CROSS technique. Also you need to find doctor that knows this technique and is good at it, otherwise he will ruin your skin. Also you are probably Fitzpatrick 3-4 ? Doctor needs to consider this.

I think it can work for boxcar scars but scars are different from person to person, in my opinion. Boxcars can be of different depths. Boxcars in certain areas may heal better than others. Sizes of boxcars may make a difference. How you got the boxcar could (this is only my personal perspective) make some difference as well.

The bang for the buck of RFM isn't worth it given my experience. I've read that it has helped some people though. But I definitely wouldn't recommend it for icepick scars.

Hey everyone. 

I have undergone MNRF + TCA CROSS since my last post. These images are taken post 40 days of treatment.

 

I consulted my doc and he said PRP + Microneedling for next session.

 

Please review and tell me what I should do next?? I am more concerned about the right cheek and my left temple area scars.
