Hard to know. Sometimes re-excising helps, I have seen reconstructive surgeons say this. But do you own research, ultimately it is your choice.

Update: putting hydroquinone prescribed from my other erbium treatment because the doctor who excised said to do it. So I started yesterday. The spots are reallllly red now. I think in two weeks it should calm down. Im going to use it for one month, as I used to do before. Also I saw this on Dr. Mascaros social media, talking about botoxing scars. I think its relevant in terms of a doctors opinion.

Here is me smiling. The one near my mouth looks indented. Im concerned now. I guess I could get it re-excised. But Im going to do ablative laser in a matter of time and hope for the best.

Whats the appropriate time frame to subcise? I can probably do it at the same time as the laser.
Ask your doctor. Probably a few months, after it has stabilized. Scar volume typically peaks at 2 months after excision. You may also need Sculptra if it turns out there's atrophy underneath there (I doubt it though).
Finding a good plastic surgeon or dermatological surgeon is key.

Remember when the excision area is large you are removing a big chunk of skin. So there is a lot of space gone.It will take atleast 2 month for the skin to heal together as a normal skin. And slowly it will look flatter. My father had a mole removed on his cheek. It looked indented for some time but after 6 months it was unnoticeable. So we just got to wait. Also you can keep on remodeling scars with subcision and deep peel ,lasers. That is why I only say to reserve excision for the deep scars where it actually looks like a hole. Then this slight indent would not have bothered you. To me the most ugly scars a hole with sharp borders. But dont do excision again. Give it time, it looks red so it could look worse than actually it is.

Based on this, I scheduled laser + subcision for this monday at around 4pm.
Update: Got three different lasers with subcision only on the punch scar near my mouth. I believe it was pico, erbium, and Co2 Ultrapulse fully ablative. Heres the photos of what I look like right now.
Since you guys are familiar with the healing process as Ive documented it here before, I wont be doing that this time. Ill report back in three or four days after its mostly scabbed off.

Pain? Yes generally there is pain but it's something I am used to by now.

I'm quite familiar, haha. The healing part is most easy to me in general but being around members of family is probably the most annoying part. They are very squeamish.

I agree. I objected against scupltra filler on that punch near my mouth but I got subcision there. I want to see how it looks on its own without any assistance from filler. Maybe I am naive but I care about how it looks naturally. If it's still annoyingly present, I can either accept it or re-excise it with scar revision expert. But to re-excise, I would probably wait 4-6 months. Not sure.
I have yet to see Dr. Mascaro, who is a scar expert. I'm going to see him late december. I'm afraid of being artificially swollen from the laser but I think that will resolve itself by then because I workout excessively. Maybe my body will absorb a lot of the retention, idk.
Generally though, the other excision spots look fine, at least from what I saw. Hopefully it only improves from here on out. At the moment, I don't regret the excisions though, even the poorly healed one. I will probably get more done some time next year for the ones on my right cheek, but I think this time I'll go to more scar revision experts if it's not dr. Mascaro.

Could not punching the scar wide enough not have good results? Right after I already noticed that the stitching circumference my doc did was smaller then the scar.
subcision was by the laser doctor Iwent to in march and the punch was a derm who did mors surgery. I'm still not sure if he did a good job because the one on my cheek is very large, worse than before. The other punches seems fine though. It's hard to tell as I have little lumps under them and I don't know if thats artificially propping them up. I'm going to sit and be patient.
