Hey all new to the forum, I have been battling acne most of my life probably like the rest of you. Went on two rounds of accutane in college and have since been pretty pimple free with the occasional break out once or twice a month. I'm left with some scars which from the few doctors I've seen have been classified as moderate ice pick/box scars. I'm looking for advice on what treatments you suggest, I'm looking into Dr Rullan in San Diego and Dr Novik in NY because I've heard those two are some of the best at treating scars. Any advice is greatly appreciated, thanks!
Thank you for posting I see your name is 49ers, ... Rullan would be closer to you and a better choice. He also handles more ethnic patients.
As you know you have surface textural scarring. Icepicks, Boxcars, and some rolling scars. No fat loss. Not severe scarring.
Have Rullan do Cannular subcision. 4treatments, with prp, saline, or filler.
You need tca cross, Dr Rullan calls it phenol cross / paint on phenol to the scars. Multiple sessions.
Erbium resurfacing or Rullan is a expert in acid peels (same thing) for the texture if you wish further improvement.
Use differin (OTC - target) and a bleaching cream konjac and albutin nightly. Stop one week before treatments. This prevents discoloration.
Wow thank you so much for taking the time to reply so in depth! Im actually on the east coast but willing to travel to the west coast if Dr Rullan deals with ethnic patients better. Do you know if the subcision and cross can be done at the same time or spread apart? Whats the difference between saline and filler? Im aware what prp is. Thanks again.
@49ersFan If you're boston area you can get cannula subcision done pretty cheaply with Shanthala MD [Link removed]. She seems honest and reliable. IIRC I think they said subcision was only like 300$ or something. She also does TCA cross. Might be worth checking out. Once I get my damn acne cleared up I'm going to go for a few rounds of cannula subcision + filler spaced some time apart.

I would suggest you go for NOKOR instead of Cannula, which won't be able to subcise those deep pits in the bottom pic. Cannula was never designed to cut anything. It was specifically made to inject fillers without leaving bruises. Hence, while it might be ideal for superficial pits, it's not the best instrument for severing tethered scars. Just my 2-cents.

@thepwhispI checked her site and when I clicked on acne scars it doesn't mention subcision or tca cross, kind of worries me that they wouldn't mention that under their acne scar treatment page. Have you had any experience with her performing either or know someone that has? @beautifulambitionI also was looking into Dr Taylor in Salt Lake City, according to some reviews he does a one time procedure and people have seen some pretty drastic results, do you have any thoughts on him?

Actually nokor subcision is not entirely risk free. Although 5 months have passed since my nokor subcision there are still two raised lines on my right cheek. So it's not just about hematoma, sometimes it can lead to further scarring. Therefore it might be a good practice to lessen side effects

I understand your worries, but which is far worse? Acne pit that can and often will last a lifetime or hyperpigmentation that will fade in a year or two? Scars are tough to treat, let alone correct. It requires invasive treatment, and all invasive treatments come with side effects. If you believe otherwise, you're being too naive.
@49ersFanDr Taylor is extremely expensive $$$$ and he stacks treatments, similar to Dr Emer Hollywood. Treatments of course are always better when someone spaces them out and let's the body heal naturally (Slow). When you stack that many things there can be side effects but he is a experienced plastic / derm. I would do 3 consultations even if it costs some money before deciding who you go with.

Hi! Wonder if you check out the Boston Acne Specialists and Dermatology Institute of Boston? I think both of them do subcision. I called them to check though derm institute of Boston only does localized subcision.
Hows ur visit to dr.shanthala?
@thepwhispHi just saw your message mentioning Dr. Shanthala. Hows your visit with her? Im wondering if she performs Nokor subcision since the website says she only does non surgrical procedures. What worries me is that she is an anesthesiologist and not sure she has much experience with dermatology.
And also have you checked out Boston acne specialist? How r they?
Hello everyone.
I have been suffering with an awful form of acne if thats what you want to call it. I get recurring boils and carbuncles on my face, back and bum consistently. I will share with you everything I have tried to stop these from recurring. I have a dermatologist appointment in September, until then.. I am willing to try anything!!
I have done round after round of antibioticsto no avail.
I have tried salicylic acid, benzoyl peroxide, hydrogen peroxide, rubbing alcohol, baking soda, coconut oil, tea tree oil, almond oil.
I even bought chlorhexidine which is a surgical grade antibacterial wash that surgeons use to wash their hands before surgery. Still I get them.
I have cut everything out of my diet. Gluten, dairy, eggs, fish, nightshades.. nothing helped.
I wear loose fitting clothing, I try not to sit for too long.
I wash my bum with chlorhexidine 2x a day.
The breakouts happen sporadically and turns my life and everydayhealth. I can not put two and two together to know what is causing this!!
Does anyone have any suggestions for me? Anything else I can try? I have two on my face, one on my forehead and 2 on bum right now, they are dormant but bound to flare up any moment...

Marisa do you think subcision and tca cross are too aggressive of a treatment? Some other doctors I have talked to have just recommended Infini RF, but I am skeptical of needling/lasers since I got 6 microneedling sessions done and didn't even see a change in my texture. But if my scarring is not severe I would think subcision and/or tca cross could possibly make things worse.
I heard back from Dr Rullan after he examined my pictures, he recommended 3 sessions of Subcision, TCA Cross, and Needling. He mentioned if my scars were still not gone then I would need fillers, he did not mention anything about using fillers, prpr, or saline, should I ask him about this or do you think his method makes more sense? Would I still need fillers since I have no fat loss and my scarring is moderate?
