Acne scars

Hi everyone, im new in this forum and sorry for my english because im from kosova and I dont speak good english.

What is the best treatment for white flat hypertrophic scars cause from acne on shoulders ,because those scars cause me depression I don't go in beach or pool from age 17 now 21 and I heard are hard to treat these scars Do you know anyone from his/her experience whether these scars can be removed with any treatment.

Im totally ruined. :(

I hope I am reading this right. It's a white scar. If so look at the below FAQ link in my signature, goto the skin section, then find " hypopigmentation " there is a few treatments for white scars there. The shoulder is harder to work on than the face, because of less blood vessels, still possible to get some improvement. I can not see a picture of the scar so I do not know any other treatments I can recommend unless you give me more description of if it's raised at all and needs steroid or depressed and needs microneedle, etc.

Two are raised others are flat and white not very much large but about 5mm some more small some more large can to treat these scars with dermabrasion and skin graft I heard for this method is very successful for this scars ??

Without pics I cannot help fully. I have no idea what your scars look like. Dermabration is for softening scars. It may be appropriate for the Keloids but you must seek the advice of a facial plastic suergon. Skin grafts don't work well for scars currently, unless it's very tiny. 

IF they are raised they are keloids. I have a whole section in the faq link (my below signature) on keloids. 

Dr's often inject them with steroids to flatten them, use cryotherapy to freeze them, or surgery. 

I will paste from my faq since you speak a different language. 
 

here are two basic types of Raised scar types:

  • Hypertrophic scars - are scars that are raised above the skin surface but remain within the boundaries of the original injury. Scarring and can be treated with silicone sheets , liquid silicone, or cortisone injections (provided by dermatologist). Color can be treated with the V-Beam / QuadroStar Pro Yellow Laser. The treatment options include laser ablation, electrodessication, or surgical excision. Other options for the treatment of elevated scars include injection of bleomycin, cryotherapy, topical application of imiquimod, flurandrenolide, tacrolimus, and silicon gel. Intralesional injection of botulinum toxin has been shown to reduce scar formation. Pressure, flavonoids, TGF, mannose-phosphate, and IFN25 were also listed as helpful.

  • Keloid scars - which are raised scars that extend BEYOND the boundaries of the original sore, blemish or injury. It is scar tissue gone wild. It can continue to grow. Some people are genetically prone to keloid scarring. (Treatments are the same as Hypertrophic Scars above, with steroid injection & cryosurgery being the number one treatment for the raised texture).

   WHAT IS: HYPOPIGMENTATION 
     - white spots (loss of pigment)

Q: What Is Hypopigmentation? 
A: Hypopigmentation is: Vitiligo, pigment loss due to burns, scars, skin discoloration.

  • Treatments: XTRAC Excimer Laser at 308nm or Light Box phototherapy treatments UV-B lamp, Melgain (Decapeptide - Basic Fibroblast Growth Factor), and microneedling at home. Clinics use costly Recell which only has a small percentage of working on scars.

Thank you for this information about these scars and treatment of it.

My scars are hypertrophic not keloid but mostly are flat and white (hypopigmentation).my scars look same with second photo to hypertrophic scars but more small in size is mild-moderate.

At home I would first try microneedling ther derminator is great by owndoc.com the spots and UV-B withMelgain (Decapeptide - Basic Fibroblast Growth Factor). See if you can get some pigment. Otherwise for pigmentation reversal It's going to take a doctor that has theXTRAC Excimer Laser at 308nm is best (search for someone near you who has this), or Light Box phototherapy treatments UV-B lamp.

If it is small enough they may be able to do a skin graft and excision. It will take several treatments, for now I would use makeup over them to even out the color.

For texture you can do tca peels on the spots or laser.

Clinics use costly Recell which only has a small percentage of working on scars.

how much can improve hypopigmentation and texture with these treatments do you now? Have you seen anyone has success with these treatments.

Dermiantor by owndoc.com aka dermastamp also helps monthly to soften scars.

There is also permanent medical tattooing for extreme cases and makeup

Isawproblem of 'aesthetic beast' but his problem was on face and in face is more easy to get results. But anyway I will try but im little skeptic because none of these treatments does not guarantee success.
