I had a whole bunch of treatments done on my face yesterday for my acne scarring. The before photo doesn't show the full extent of my scars but I've asked my doctor to send me the photos they took which I will upload as well. My main issues were uneven skin tone, red marks, a couple of small ice pick scars and quite a lot of rolling scars.
First, I had numbing cream put on my face and had my blood drawn for the PRP. Once numb, I was injected with local anesthetic and then had my cheeks subcised with a needle. Next came the Fractional laser followed by more numbing (lidocaine) and a cold face mask. I then had Fractional Radio Frequency Microneedling using a machine called Intracel followed by another cold mask and then 10mins under an Infra Red Lamp. Lastly, I had the PRP injected into my face and applied topically with growth factors.
The whole procedure was not pleasant but with all the numbing cream and injections it wasn't too bad. I would say the most painful thing was the anesthetic injections!
My face was VERY sore afterwards like I had very bad sunburn. It is still stinging today and the swelling has gone down a little but not as much as I thought it would. My doctor tells and the pain should subside in 1-2 day and redness in 3-4 days but I'm doubtful as it's pretty bad!

Thanks I had originally asked for PRP + Subcision + regular microneedling (eg dermapen) but this doctor has lots of machines at his office and said that he gets better results with a light laser treatment prior to RF microneedling. Washing and applying topicals is very painful at the moment but things should get easier soon!

The doctor is Dr Henry Ngai at Reviva Laser Skin Clinic in Richmond which is next to Vancouver, BC. The total cost was $2150 Canadian and he did the fractional laser for free I thought this was a great deal considering other clinics quoted between $1500-$2000 just for regular microneedling (dermapen) and PRP.

Hmm I don't know, maybe it's easier to do the subcision before all the others because the skin still looks normal at that point. Doing it after the face is swollen and red may have been too much, especially with all the anesthetic that was injected before the subcision. Do you think this was the wrong thing to do?
I just imagine it akin to 2 slices of bread.
Do the fractional RF microneedle (you penetrate the bread with the needles) > and then you subcise after (you apply jam between the 2 pieces of bread)
compared to
subcise (apply jam first) > then needle penetration
The latter can't reach the intended depth the RF microneedle is supposed to reach because of the jam

I think I see what you mean but during subcision, nothing is put under the skin except for the needle which is moved around to break up the tethered scar tissue underneath. The Fractional laser and RF microneedle was done after this to work on the surface of the skin. I think the PRP would act as the jam in this example when it is injected into the skin in the areas that were subcised earlier. This was all done within an hour so the PRP should work it's magic and heal under the skin rather than get tethered again to create the scar while the surface of the skin should improve in texture from the laser and needling. At least I hope this is the case!
Just a quick update... it's now 5 days after the procedure and the swelling has finally gone down. I started peeling a lot yesterday and lots of the scabs have now fallen off, leaving only the harder scabs on my cheeks and forehead. The skin under the scabs that have fallen off looks great... it's still pink but smooth with no visible scars so far.
For anyone wanting this treatment, especially the RF microneedling, you will want to book at least a week off. My skin looks amazing today compared to the previous 5 days when I've had hard scabs covering my whole face. I looked like a reptilian monster! There has been more down time than I expected but things are looking promising and I am excited to see the results

Dr Henry Ngai did not care that I was breaking out as a result of microneedling and I have had a massive acne problem for a year as a result. Now on Accutane. I literally have more acne scars than before, which he admitted. I've had to seek mental help as a result of his work.
