I have read so much conflicting information about lasers for treating acne scars and really want to get to the bottom of it. Many people say they don't help while others can't say enough good things. I have come to a conclusion and would love others feedback. I think lasers probably give great results under the condition that the laser is ablative (Fraxel Repair, DeepFx), done at the highest setting, and repeated 3-6 times. Other things such as a good diet, good sleep, staying out of the sun, and plenty of exercise should also be done post care.
Has anyone done a treatment plan like that and not been happy with the results?
I have had a reasonably good improvement from high settings on fraxel non-ablative, 6-8 treatments:

Yes, it is worth it, especially if ablative (I can`t do ablative due to skin type). Go for several ablative laser sessions.
Thanks for your feedback and I can definitely see improvement in your pictures. I don't have a ton of knowledge on this but why can't you get ablative? Your skin doesn't look that dark to me but does it have to do with your ethnicity? I had also heard that ablative fraxel lasers don't cause pigmentation problems as the non-fraxel ablative usta cause.

I can agree with this based on personal experience (i've done 2 so far) and the dozens of medical studies I've read. Just be aware there is a *risk* of side affects, including no improvement. Also, expect 10-15% improvement each time - don't expect miracles.
Don't waste time/money on non ablative unless you can't do the ablative because of skin color.

Thanks for the feedback. 10-15% improvement each time is fine by me. My skin is very pale and I am probably between a I or II on the Fitzpatrick scale. Have you had any side effects?

First time was fine. Only did the second one 2 weeks ago, so not sure yet since it's still red/has grid marks.
I would approve. Lasers are great it is just that you will require a lot of sessions depending on how severe your scars are. So make sure you are ready for the sessions and healing. Sometimes it will take longer to heal as they increase the microns/depth of the lasers. However, after several sessions, you will notice the collagen starting to reform and push up the acne depressions. Also keep in mind that lasers will uniformly ablate the skin where it is treated. What this means is that if you have an ice pick scar, it will also be ablated to the same degree as the regular unscarred skin. This is the reason why sometimes the results are not as good as say a dermabrasion, subscision, or excision treatment. The goal of the laser is to work from the inside out. After all the lasers I have done I was always disappointed to see once the swelling went down, that the scar came back. It was not until the collagen was stimulated, that I saw the results. Most doctors leave this point out because they are trying to make the treatment sound like it will cure all your problems. Good luck with your treatments and be patient. Lasers take time.

ablative carries the risk of hyper or hypo pigmentation in people that have non white skin. It has more to do with skin color than ethnicity. Hyper pigmentation will eventually fade but if you hypo pigment, it is usually permanent. However, I have heard doctors can tattoo over it with your own skin color to hide it very well.
