Are you male or female? And what is your weight? Also 40mg for 4 months is low and only a cumalitve doese of 4800 and depending on your weight and gender it might be even lower of a dose. You need 150 x your weight for Accutane to give long time remission and it should all be taken in a 6 month period to cause enough toxic damage basically to your body so the skin has permanent changes  Thats how accutane works if im not wrong! Basically you never really completed a full accutane course only a sloppy one.  

You should take 150 x your weight for 6 months in your first Accutane course 

and then 150 x your weight for 6 months in your second course and there is no way in hell you would need a third, because you would die at that point from the accutane lol! 😛

I'm currently on my third accutane treatment and I told my derm about all my accutane history.... Your derms wrong... Maybe you just haven't waited long enough in between accutane treatments. Also as strongbodyforgoodskin said you should be doing a full 6 month treatment to get the lasting results; that's the mistake I made the first two times around as well and hopefully this time works

I don't think they would even let me go on it right now anyway because I barely have an acne...I just know I break out every now and then and I have been eating a lot healthier now so my acne is limited. however, I want to be permanently cured and not worry about what im eating

You have had enough accutane and if you haven't had any long time term side effects you are extremely lucky. Side effect of accutane are mostly permanenet, don't let anyone tell you different. Please do you homework and pm if you want more info.
