I am a 26 year old male 70KG. I was prescribed by my derm 40mg a day beginning of this year for moderate persistent acne. I seem to have been a super responder because literally after 2 weeks (and a pretty horrendous 4 or 5 day initial breakout) my skin is clear and I have had barely had new acne in the last 6 weeks. Also I have had mild to non existent side effects. Even my lips aren't particularly dry (although drier). The only evident side effect is my skin is slightly redder.
Today I had my 2 month review and my derm has advised I now take 30mg a day for the remaining 4 or 5 months of my course. He mentioned this was because I have responded well to the treatment and he's concerned about the redness of my skin and how a higher dose may make it worse. We discussed the 120-150mg/KG cumulative dose that should be taken to avoid a recurrance of the acne but he mentioned because of my age and the fact I no longer have any active spots this recommendation is less useful.
What do you guys think? I would hate to finish my course at the end of my 6 or 7 months only to find the acne comes back because I took well less of the cumulative course.

If u are responding well and can take/tolerate a lower dose with hardly no side effects and have a successful outcome...win win... I say go for it!
there is no guarantee either way (to be honest) after treatment that u will remain acne free
but there is research that lower dose is just as effective and makes for an easier treatment...i would just hit your cum dose by the end of treatment..since u are responding so well i would def stay on for at least 7 mnths+

In a way I agree with you. Why up the dosage when there is no new active acne and little to no side effects.
However, he is planning on stopping after 7 months max (maybe even 6) so at the end of it all I will be at way less of the recommended cumulative dose of 120mg per KG. He mentioned that the cumulative does recommendations are acutally best for teenagers and younger people as the large cumulative dose should hopefully 'kill' off the acne as they grow out of it. For older people he says its much less useful as the acne issue is no longer as a result of growing pains but simply an underlying issue that will likely always remain somewhat.
