Liver Enzymes On Accutane

Hi, I would greatly appreciate some advice from some people that have been through something similar. Last month, or maybe it was two months ago which were my 5th month on accutane (beginning sixth) I started having a bit elevated enzymes (ALT was at 53 iu and normal levels are <40) I was pretty freaked out so I cut my dose in half (my doctor actually wanted me to double my dose) so I did it without consent.

This month I get a call telling me that it has went up to 70 iu (no other problems, no cholesterol, nothing else significant everything else looks fine). Nevertheless, as I sit here furiously searching the web for some answers I know I probably won't find, I hear about this girl that was on spiro and hers as well had started to rise during her treatment, and even when she went off antibodies started attacking her liver, ending up with cirrhosis and she is now waiting for a liver transplant. Is she kidding? Could this actually happen? My doctor recommended that I lower my dose, yeah well, should I just go off it? I'm pretty sure she isn't going to be the one who has to have to live with a destroyed liver at 24 years old. Acne is terrible, but this would be worse. I don't have acne, hasn't for a couple of months (at last 2 month since last zit) so maybe I should just chance it?

Your doctor knows what to look for. I wouldn't panic. They would take you off immediately and send you to an internist if they thought you were close to liver failure.

 

Anyway, the liver can regenerate itself it it's not past the point of no return.

 

Try not to drink alcohol or take Tylenol as both of those are processed thru the liver.

 

If you want to know more about liver function/ failure, ask the pharmacist the next time you go to the drug store.

Have u reached your cum. dose

Does Dr want u on tane for another mnth? how much longer is your treatment?

If you are VERY close ot the end I would finish my dose

I hate to say it..but stopping now or staying on it another mnth I dont think would make or break

Im on 2 mnth+ tane and my chol is elevated

My dr said they normally dont worry because its a temp thing...

Im sure it is scary though..i would discuss the game plan with your Dr and go with your gut

If you are that uncomfy with staying on tane than stop all together or maybe reduce your dose even further?

Yeah I didn't say... I'm on low dose... 20 mg/day... I have about three months to go, but I was just going to increase dosage to at least 30 mg for just two more months. Then this happened. I'm off it now until my next blood test, my doc wanted me to just do very low dose but I'm just gonna stop for now until then. I'll keep this updated in case anyone winds up in the same scenario.

So ALT (liver enzymes) went down from 70iu to 40iu in two weeks. I'm just borderline now. I had to change derm because she wouldn't 'tailor' a course for me since my liver is sensitive. I have been off it for three weeks. No acne back yet. But, she did say that I might not need anymore since I haven't had real acne in more than two months. We'll see, I guess. I'm reluctant to go back on it though, I can not deal with the side effects anymore. I'm gonna try something else. My skin is so sensitive that if I pick at it I get hives, like soft cystic bumps that take a couple of hours to go away. I'm now gonna try homeopathy, since that it the only thing I remember helping my skin conditions by not creating new ones.

I will wait a little while, if it comes back I can go back on it. Hopefully not though, regardless of acne, because it has been hell apart from being acne free (most of the time).

For everyone in the same position, I will try to update if my acne returns or not (I promise). But there is evidence that low dose (20 mg/day) for at least 6 months might help long-term. I did 5.5 months on 20 mg, 2.5 months on 10 mg, then 5 mg one day the last week. My weight is 53 kg, but I was 47 kg when I started. So around 4100 mg so around 80mg/kg (I averaged my weight) cumulative dose. The recommended dose is 120 mg/kg but the new research says 70mg/kg can be enough. I guess we'll see. The last two months were acne free, I did have still two closed comedones still hanging around but none has become inflamed. They are still around, sometimes they look like they want to turn into a pimple but after a couple of days decides against it and goes back to normal.

I might also say that I eat very healthy, juice everyday, do not eat my food sensitivities (tested high for yeast, egg) but have also taken out dairy and wheat (not grains I still eat some rice). I make my own 24-hour yogurt with coconut cream for probiotics. I run or walk every day, don't use makeup only vaseline, and I don't wash my face anymore because it just becomes so red. No moisturizers, but vaseline. I do not believe chemicals can help me as I have eczema, everything that dries it out causes a reaction of some sort. This sounds fucking awful, but honestly I don't think I'm alone people do extreme things on here haha. I do not eat low gi, since I juice carrots and sweet potatoes. I mean fucking carrots, they are a vegetable I will not become obsessive enough where I count sugar content in vegetables, but alas they are high gi (or enough of them are). And I also eat maybe a piece of rice bread (without gluten, yeast, or egg) every day and bananas!! I don't care. If it comes back it will not be because I ate a banana.

I planned a low dose/longer term regimen

BUT y Dr after starting tane insisted on a higher dose (60mg)and I have to say after 3 mnths I am feeling my patience wear thin!

(thankfully no issues with b/w)

BUT i have also been reading great results with low dose tane as well

Hi, yeah I went off it completely. I might go on it again though if it comes back in real form. Not like if I get a couple of papules.

I was clear though when I went off it, and had been clear for two months. Before that I had about one zit per month (from old whiteheads lingering around).

I have gotten some weird shit though, like small blister like bumps. They aren't red until I pick at them, and they go away so fast. You can burst them too. Some itching bumps but that could be from excercising or wearing a hat. About one papule that went in a day. No whiteads or blackheads or anything. I do have some sort of rosacea now though, waiting to be properly diagnosed.

I get new results tmrw.. Last time it (ALT) went down to just below normal so we'll see how it looks this month.

How much were your values when you started? Mine was normal, but then at month 5 started climbing. I kept going though until month 7. My derm said that I might not need any more, but of course I would like to do the cumulative dose just in case, but I'm seeing an homeopath now and I hope she'll help me regulate my hormones and other stuff and maybe that will help too.

Keep me updated. I will post again on my next results, if they are bad I'm probably in trouble since I have been off it now awhile.

If you can deal with the hassle, try going grain-free and jucing raw vegetables every monring. You can eat lost of starchy veggies, even potatoes and lots of good fats (olive oil, meat, coconut oil). I think this actually prevents any zits because I can see a red dot but then it crusts over and disappears in a day. Could be accutane residue though, but I had normal zits on accutane too though, so not sure.

For your liver enzymes take a lot of liver thistle and maybe NAC and if you get any cramps, gas, stomach pains on it try L. Plantarium (probiotics - it needs to be this strain specifically) it stopped it for me (took awhile for me to find this strain though).

Wish you luck. Remember that there are other ways (light therapy, maybe try differin if your skin can handle it?). No need to ruin your liver, but I do with all my heart understand that sometimes accutane is worth it if it is seriously causing problems in your social life.

Ps. weight training can def affect your liver enzymes so just stop training days before your test and see if it makes a difference

my enzym levers were high too in my first course ...like 180 for both ALT and AST

so i stopped accutane and they went back to 20 and 21.....my doctor was suprised ...then i remembered i did some serious hard exercice the week before my test (my muscles were still a little sore) ....so in my second round of accutane ....after 3 weeks of starting it did another test (no hard exercice for 10 days) and my alt and ast are normal with a little high ast (ast= 28) but still normal range

i hope that the ast going from 22 to 28 this time doesnt mean that its going to get even higher the next month and the month after that

PS: my alt is still the same before and after 3 weeks of starting this second round( alt= 20)

what do ou think OP ?

I think that is fine then! As long as they are below the normal level... Don't worry, and don't exercise (or at least hard exercise) before taking the test again!

if you're worried start taking some liver support (like milk thistle) to help your liver with accutane

Btw got my results back... about 2 weeks after I got off the accutane for real my results were ALT 22 iu/L ( normal is <40) so it is all good. I don't know if the liver thistle helped me here, I took a lot of it. Anyways happy my liver isn't failing! Good day! :P

Kimberly how is everything going for you? Did your blood results (cholesterol) get better?

I been in accutane for 2 years in Low dose 10 mg taking IT Daily I was HAPPY with the result My skin so clean never 1 pimple even I put heavy make UP for 2 years and Not only my skin beautiful but My hair grow fast and ThickThat never I had before. The side effect for me is headache and memory fog but after a month I stop my body is normal again.
