I did this time only for 2 weeks, in previous attempts it was 6+ weeks couple of times.
FMT is interesting, I have not looked much into but from what I have read that it works like antibiotics, it works for xyz timeline but then returns. Like antibiotics, who knows what else gets affected.
Definately a better option than antibiotics, I dont know the cost involved and how you go about finding the right donor though.

Carbs 100% make them worse, but only when eating in a surplus. Im convinced its to do with hormones (+ something else). It could even be something so simple like drooling in the night and the saliva mixing with the oil/ dead skin

For me the pustules first appear on the chin and next to the mouth and then spread like wildfire. When I am not watching what I eat at all I get them between the eyebrows too. Also I often have them around the hairline, so it might even just be fungal. Sometimes I get funny pustule patterns. I once had a perfectly symmetrical triangle on my cheek formed of six pustules.
What about alcohol? Is dry red wine triggering you? I know that I can tolerate one beer without pustules (since that still stays in the keto carb limit).
What I still don't get is why whole milk is so extremely triggering. It is not even that high in sugar. However it is like gasoline over fire for that condition.

My gut bacteria is also good according to the tests - but the thing is, current tests only recognize a small pool of bacteria that if found, are considered to be part of a healthy microbiome, but that doesn't mean we don't luck the ones that are perhaps needed to in order to properly digest food or have good skin, however you want to put it.
Also, another thing to consider, besides killing gut bacteria, antibiotics kill also skin bacteria, so it might be the case that we killed off the good one on the skin and just feeding bad ones with carbs and dairy that cause this issue. What I've read online is that many people claimed how couple of FMT's solved their skin issues for good, and since I tried pretty much everything else I'm heading in that direction as a last attempt.

I get them left side of the mouth corner, the only other times I get them is when I exfoliate too much and I guess disturb the pore/ follicle. When I shaved my head, I also got some on the hairline but not normally, again i think it was the follicle being pushed back with the razor so it could be from styling hair too.
I dont know what triggers them in terms of food, I only know when eating in surplus I get them. Milk never caused them for me directly, no food did. I plan to add 5kg and then stop gaining weight and maybe they will go. Ive been getting wide ones recently but they dont even last 1 day.

I dont trust any of the testing tbh, I have a friend who did 2 food sensitivty tests privately and got different foods each time. Im also sure we all know people who have poor digestion, eat garbage yet have clear skin.
Skin barrier is very important, also all these activies that people with acne use disrupt the natural skin balance (acids, BP etc). Do you also get an abnormal amount of dead skin buildup?
Im convinced it is to do with male sex hormones, pretty sure 99.9% of people who get this are men. Maybe we produce too much of something/ there is an imbalance somewhere. You see when people take steroids their skin goes crazy with outbreaks

What is your idea on why that abnormal dead skin buildup happens? For me it was like that but it got better and better now with dieting and not using anything on my face. I used BP (epiduo) for a time (but just a few weeks, really not long at all) and that was exactly the time this pustule stuff started. Is the natural skin balance restorable or permanently damaged with BP usage? I haven't used a single product on my face for nearly 2 years now. Only water and my bare hands which works PERFECTLY. Smooth and clean face after every shower. However after 1-3 days without cleaning my face by exfoliating with bare hands I get some dead skin on and over the chin, between eyebrows and nose creases + lots of dried up sebum on nose, nose creases and just over the chin. Also sebum plugs on forehead which I can remove by picking very gentle with my fingernails. I just shower every 2-3 days now though and clean my face and its fine.
I still think that my skin produces too much oil and too much dead skin though.. unlikely BP damage though, as I used it for a really short time. Until I noticed that it bleached my bedsheets, so I was thinking this has nothing to do on my skin! Also my skin got so waxy and shiny looking from BP, like if 10 layers of skin where removed. Really really bad stuff- hopefully my skin wasn't damaged permanently, but then almost everyones skin would be damaged as almost everyone with acne used BP for a short or long time.

Exactly my thoughts too. The time the pustules where the worst, my T was the highest (around 900ng/dl). Totally naturally though by eating lots of raw dairy, meat and working out. It was such a weird thing that the better and more energetic I felt, the worse my skin looked. Could also be just the dairy though..

I think its the hormones which can a) make the skin oiler b) increase skincell turnover c) detox excess hormones through skin
If you get all 3 plus have compromised skin (either damaged barrier or bad bacterial profile) from topicals/ antibiotics your skin cant deal with the sudden change. Thats why im trying to taper it out. Would also explain why carbs make you break out becasue carbs are precursers to lots of hormones

How often do you shower? I'd advice exfoliating your face first (by rubbing your dry face with clean dry hands until everything is off) and then hopping in the shower and just rinse the face under lukewarm or cool water. Do this 2-3 times a week. It did make my skin a lot better.

Have you tried triple antibiotical ointment (neosporin)? This kills gram-negative bacteria which are causing our types of pustules. However I suggest putting it in your nostrils as that is the place these bacteria come from (thats why we only get pustules on chin and above lip and cheeks). I did try applying ointment to nostrils for a few months (polysporin), but didn't seem to help in my case. If anything it just exacerbated the pimples I had been getting inside my nose.
Also had a doctor prescribe mupirocin for 10 days, but it didn't help either.
Currently the pimples inside my nose seem to have settled down.
I still get them around my nose, but it's been a while since I've had a truly bad breakout.
I'm still mostly just washing my face with micellar water and then moisterizing. However, a week ago I introduced a 2% zinc pyrithione wash that seems to be helping.
I've been meaning to see a dermatologist at some point, I'm fairly convinced that I have roseacea (type 2) + seb derm.
Over the past few years my nose and the skin around my t-zone has developed a persistent redness (at times it gets worse, at other times it gets less noticeable).
I also started getting a ton of flaking skin on and between my eyebrows. The zinc pyrithione soap seemed to completely eliminate the flaking skin as soon as I started using it.

Wow, I read all 36 pages of this thread. Ive been dealing with this exact same issue. I am 43 years old and used all antibiotics under the sun and used Accutane twice. First time when I was 17 and again when I was around 25. My dermatologist keeps trying to put me back on it and I keep backing away. Just the experience of being on Accutane is a nightmare. There are a lot of things that work but I cant use them long term. Minocycline twice a day will clear me up in days but obviously I cant stay on it. Amzeeq worked for a couple weeks but it became resistant and came back. Ive tried Blue Light lazer and that did nothing at all. One dermatologist said it appears to be Rosacea since its only at the center of my face and every time I go hell say its probably because I was in the sun or I ate this or that. Another dermatologist said it might be perioral dermatitis. This kinda falls in line to what many of you have heard. I originally set up a spreadsheet to track when my breakouts happened but I was unsuccessful if one particular thing would break me out. I am a decently healthy male, lean, dont eat out much, drink healthy green juices and so on. Im currently on Tacrolimus to see if it is perioral dermatitis. So far its been two weeks to no effect. I honestly just want this shit under control. If I can get it down to 1 to 3 small pimples I would be more than ecstatic. I really dont want another course of Accutane but if I have to do it, then so be it. This has impacted my life much more than I would like to admit and would not wish this on anyone.

Yeah it definitely has; not just in my love life but in all aspects of my life. Hanging out with friends, family events, parties, etc.

So far zinc pyrithione is the only thing Ive found that keeps the flakes at bay on my eyebrows and chin

Ketogenic diet with as few carbs as possible.
For me this cured everything. My skin is not oily anymore, I don't get those white pustules anymore and I don't get dead skin buildup anymore. It is just that, just don't eat carbs. Sometimes I eat some Raspberries or have a beer or a glass of milk but then I also know that I put myself into the risk of having 1-3 pustules two days afterwards. And then I get back on track with eating only meat, aged cheeses, eggs, vegetables, salad and fats and enjoy clear skin!
Our condition is kept active by eating carbohydrates. Without enough carbs, the pustules can't develop.

Please don't go 0 carbs. Yes, it clears your skin, but it will fuck up your body in some other ways and it's 100% not worth it and not sustainable. Your body needs carbs. I've done it because I was ignorant and desperate. It gave me appendicitis. Listen to your body.

I know how it feels. It's heart-breaking to see others with this condition. For me it's like a curse that screws up all aspects of my life. There was a girl I liked who showed interest to me and I couldn't even face her with my pustules. It's funny how such a big part of my life was spent to deal with this thing. I always thought Accutane was the last resort because of it's potential life-ruining side effects, and I was on the verge of taking it and gamble with my life.
Btw cleaning your skin thoroughly helps. Like double-cleansing and rubbing all parts of your skin.

I'm sorry that you had to deal with appendicitis. However, how do you know it was the 0 carbs that gave you appendicitis? As far as I know a diet consisting of meat, fish, cheese, fats, vegetables, nuts and salads can't be unhealthy.
I have two options: eating whatever I want but having tons of pustules everyday and be super depressed and ugly/ 0 life quality.
Or eat like that (avoiding all carbs) and be happy. Also I feel good on keto, I can live by meats, cheeses, vegetables, salad and butter alone.
Guys (...and girls?) I have found a new treatment that works for this nightmare condition. Some here have reported that benzoyl peroxide suddenly stopped working, and sadly this happened to me as well. My skin stopped responding to BP treatment and it only caused irritation and skin barrier dysfunction. I would still have itchy, red bumps and new unpleasant huge welts on my jawline that BP seemingly made worse?
I quit benzoyl peroxide and started using ZZ cream instead. It's a sulfur/zinc/menthol cream used to treat rosacea. I have been using it for about 3 months now and it's awesome, really kills any pustules and immediately calms down any inflammation and redness. Actually I am a little worried that there are hidden steroids in the cream because it is from china and they don't have to disclose all ingredients? Who cares, I am glad to use this mystery china cream if it works. Skin is smooth, hydrated, no redness, no pus zits, no massive red bumps on jawline. Don't have to deal with BP damage and irritation.
This makes me wonder if this condition is actually caused by Demodex. Maybe the Demodex mites are vectors for Staph bacteria, carrying it around on the skin and into hair follicles? Some here said they tried Soolantra and it didn't work. I also tried this and it didn't do anything. But I feel like ZZ cream is a way more effective treatment for Demodex and rosacea.
Guys, try the ZZ cream. I use a tiny amount every day mixed with xylitol gel moisturizer. Xylitol busts biofilms and is very hydrating for the skin. I also use the xylitol in my nose to keep the staph bacteria in control.
The other alternative is doing elimination/carnivore diet for the rest of your life. Which I guess is sort of better than having white pustules. But I'd try ZZ cream first.
My second working hypothesis is that this is a problem caused by Staph and Malassezia (i.e seb derm yeast). ZZ cream also contains a lot of ingredients that kill Malassezia and again the xylitol moisturizer I use destroys both fungal/bacterial biofilms. The reason why I believe this may be a form of seb derm is because many people in this thread have skin flaking and I personally have had problem with oily keratin plugs on the sides of my nose (most common seb derm area) which I think are fungal. Either way ZZ cream works quite well.

I do have horrible allergies, my nose doesn't really get runny though, rather it gets really dry and I get a large build up of really dry mucus.
My allergies used to be more of a seasonal thing (in the spring), but the last 1-2 years it's been all year round, and I'm not sure what the hell I'm reacting to. The last 1-2 years is also when I started getting these constant pustules... I'm starting to think there's mold in my house or something.
I've also been working from home for the last 1-2 years (since the pandemic started), so spending much more time in my house.
I've been trying to get more sun exposure (been going for walks every day during this summer), but not sure it's making any difference either way.

imo we have to take the long road of rebuilding a solid foundation of health first and then seeing what happens after. Keep up the daily walks, even if it doesnt improve your skin there are so many positives from doing it. I have noticed my skin shedding improve since being the in the sun and it is pushing clogged pores out my skin. Its almost like a mini laser. For the past 3 summers I have been avoiding the sun with the advice from my derm and wearing spf, probably contributing to developing auto immune issues. I am even starting to think they tell you to do this so you keep coming back for treatments. Gradual skin aging is nothing compared to actual skin issues.

It messed up my digestion. Eating more fiber might help with digesting all the added meat and fats. And I think you probably shouldn't go too extreme, as the human body needs 400-600 kcal carbs per day to function properly, so I wouldn't go below that amount for a long duration.
