I would recommend tretinoin, it targets more concern than just acne, knowing you are already 35, tretinoin provides anti aging as well while still taking care of your acne, but unlike benzoyl peroxide...

Looks like a cystic acne, have you tried zinc pyrithione soap? that helps out with flaring, so it wont be as large as that.
