Seems like closed comedones. If you see that white head and the pimple is big it usually means a plug is stuck inside when I manualky extract it when the whitehead gets big with a blackhead extractor you usually hear a pop or you literally feel a pop or when the pinple stops hurting. Yiu could try BHA, but the othe produvts should have taken effect. How manys months have you stook with a regimen? If its less than 4 id say you didnt give ut enoygh time and quit and tried new products too soon. Hope that helps.

How long have you had acne. It's deffo white heads. I'd say take oral antibiotics first or the pill, see if it works and id go back to the doctor if it doesn't. Spriloactone is another one, u cud always try salosylic acid.

I dont reccomend taking pills or any sort when your acne isnt even that bad itll mess your hormones and might make it worse when you stop.
