Hi everyone,

Im struggling a lot mentally with my acne scarring lately. The depressions are quite noticeable in certain lighting, and Im constantly fixated on them.

From what I understand, my scars might be rolling scars, but Im honestly not sure because they look a bit unusual compared to most rolling scar photos Ive seen online.

I have a few questions:

  1. Based on my description/photos, do you think this is the kind of scarring that can realistically improve with subcision?

  2. If subcision is appropriate, do you think combining it with a collagen stimulator / collagen booster (e.g., biostimulator injections) makes sense for better results?

  3. Do these depressions actually look like rolling scars, or could it be something else (e.g., volume loss, tethering in a different plane, or another scar type)?

  4. If its not typical rolling scars, what treatments would you consider most effective (subcision variants, fillers/biostimulators, RF microneedling, laser, etc.)?

Id really appreciate any inputespecially from people who had a similar pattern and tried subcision.

Looks more or less rolling to me.

A few spots seem like maybe subcision could help, but overall, I'm wondering if just the "usual" would be a good option for you, meaning Fraxel laser + PRP and a bunch of sessions to achieve some improvement. Or professional microneedling + PRP.

Perhaps a bit of subcision first on those couple spots and then just a general treatment like mentioned above.

You're right to want to see and hear from people with similar scarring. That's why I always say the best route to go is to get under the care of a super good scar specialist who you trust and who has a bunch of before and afters of similar skin to yours. Only move forward when you are comfortable you've found the right person.

Thanks for the detailed feedback, I really appreciate it.

BUT, Ive already had multiple sessions of fractional CO2 laser (Fraxel-type) and also professional microneedling treatments over the past few years. Unfortunately, I didnt notice any major or lasting improvement from them, especially regarding the overall volume loss / broad depression.

Thats why Im now considering whether subcision (possibly combined with a collagen stimulator like PLLA/PDLLA or filler) might be more appropriate for my case.

Ok gotcha. Please keep us posted if you do get that. I haven't researched collagen stimulators too much so I can't chime in much on that yet.
