Like many others, I have quietly followed all the posts here on fraxel information and eveyone's experiences with it so far.
I myself had my first treatment done 48 hrs ago. My face is still very red and swollen (oh...my eyes!) but my biggest concern at the moment is regarding bronzing.
I had a full face treatment, and all over my face a small brown dots...the best way of describing them would be to compare it to scraping your skin over concrete and the little broken scabs that form over the damaged skin. Is this what is referred to as bronzing, as I assumed that was more a skin colour change that would last for a few days. Any information on your experience with this would be greatly appreciated.
On a more practical note, can I ask what you do for post-treatment care? I washed my face carefully yesterday with Cetaphil, and apply CRu lotion every few hours to keep things moist. Can I shower...I read in an earlier post that someone waited 3 days. As I'm hiding out I can hold out that long...but I miss my shower!

Those small little brown dots u described are probably micro blood clot resulting from micro-bleeding. It is not bronzing. I 've had my fifth Fraxel two days ago, and every session of treatment have been accomponied by micro-bleeding. I had bronzing on my firth two sessions too, but not for subsequent sessions. Bronzing is used to describe a skin tone whereby, as the name imply, your skin appears to have a coat of bronze paint on it. Don't worry abt the micro-bleeding, or the lack of bronze look. Everybody react to a litttle differently to the treatment.
As for showering, my greatest joy after the treatment is to jump into a shower and let the cold running water runs through my face. I am not sure where u pick up the idea that showers are not allowed?
My DM recommend Cicaplast for post treatment care, but i find that it irritate the skin. I am now using a off-the-shelf Vitamin E cream for moisturising and post wound healing. It seems to improve the healing process for me.
I feel happy that u have taken the plunge and took the treatment, but do bear in mind that u would be fortunate if u can get 30% to 40% improvement after 4-5 sessions.

Many many pages ago there were discussions on the browns spots. People have reported them so its not uncommon. I'd run a search for them.
It's probably not bronzing. The bronzing is from Microscopic Epidermal Necrotic Debris (MENDs) which is the top layer (stratum corneum) of dead skin that flakes off a week or two after treatment.
Did you have pinpoint bleeding? One of my concerns with Fraxel is that at higher densities and coverage the microscopic wounds become macroscopic (however still very small). This happens as Microthermal Zones (MTZs) overlap. And also, the higher density creates an MTZ with a much larger width.
As long as it doesn't leave a new scar then I guess it's a "good wound". Just don't pick them or exfoliate. Treat them like scabs: keep them moist and let it flake off naturally. This is probably hard to do if you want to wear makeup or go back to work. But remember how much you paid for treatment and how bad you want better skin.

If the appearance is quite obvious, maybe u can try some concealer. What is important is to use a good moisturiser so that the skin is in the best possible healing condition.
I am somewhat satisfied with my 30% improvement so far. My last one seems to have better results than the first four, so I am hopeful that the results would go up to 40% and maybe a little more. Overall, I am glad that I went through the treatment. I hope u would have good results too!

It's now been six days since my first treatment, and so far I'm really pretty happy with the results. Most of the redness and noticable swelling is gone, and my cheeks almost look dewy!
I think both you and fivetotenyears are right, that my brown marks were pinpoint bleeding (even though i didn't see any bleeding) that had scabbed over. They were raised, round and quite hard things that after the first couple of days loosened and starting falling off whenever I would put cream on. They actually hurt my skin as they moved around - I tried very hard not to rub too hard with my suncreen and mosturiser. My skin was dreadful. So I didn't leave the house until yesterday.
Yesterday (day 5 post treatment) I went back to the clinic and they did a mild facial to help remove a lot of them, though they had no real explanation for what they were. I still have quite a over my forehead and around the edges of my face...but they are slowly falling off.
Overall, while I know I still have microswelling and my skin is very fresh from it's first ever major treatment, I'm awed at the result. My skin have always depressed me and made me hide away, and while the scars are still there my overall complexion has already evened out and actually glows. I can't believe this is me, and even if it the result fades or completely disappears before my next treatment, I will always remember that I looked beautiful for a time and was able to look in the mirror without cringing.
On a side note, I found that Fruit of the Earth aloe vera NO ALCOHOL really helped remove the redness for me. It wasn't until the 3rd night that I was brave enough to use it (as the lotion the clinic gave me wasn't doing it for me), and by the morning the redness almost totally disappeared.

those brown spots are the tiny scabs that will flake off -- this is what the doctors mean when they say you will "peel." because fraxel is both ablative and non-ablative, this is the result of the ablative part. your skin will be super smooth when they peel off, no worries.
and like others have said, it is not the same as 'bronzing.'
the brown spots start flaking off for me on day 4 (if treatment day is considered day 1). when you wash your face they come off more, but definitely don't overscrub. by the end of day 5/morning of day 6 i am totally peeled. using a toner after washing your face also helps to get them off gently.

oh i didn't see you had posted again. i can't believe the clinic didn't know what the brown spots were! pretty much everyone seems to peel in this way. considering you are getting tiny tiny wounds all over your face from the laser (about a million i think), it's obvious there would be tiny tiny scabs. if you are paying for that facial following the treatment i don't think its so necessary...try a toner next time first. also, i also have the issue of them not peeling as fast around my forehead and jaw line/near ears, because of the facial hair that's in that area. basically your skin just can't shed as easily where hair is holding it down. no reason to worry about that. THAT's where i use the toner more.
good luck with the rest of your treatments and thanks for giving everyone the tip on aloe vera for redness. i bought an aloe vera gel that i am going to use for a moisturizer next time to see if it breaks me out less (i was slathering shea butter all over my chin/mouth/upper lip area last time and it totally broke me out!).

Thanks, it's a big relief to know that this is normal and I think you're right about the facial hair holding it down. The little bit that I have left is in exactly those places you mentioned shed the slowest.
I find shea butter makes me break out heaps too. The Aloe was amazing, but I would like to stress that I think the NO Alcohol content is a really important consideration when using after fraxel. I use the aloe underneath the post laser lotion the clinic gave me, as for me it wasn't mosturising enough on its own after treatment. And I guess that also depends on the brand.
I'd be very interested in hearing if the aloe works as well for you as it did for me, as I'd never used it before and so was really surprised at how well it worked. Can you keep us informed?

I would like to share another tip with u. Basically, I am now using Vitamin E cream for post treatment care. The Cicaplast that was prescribed to me didn't work for me. It only act to irrirate my skin. I did some research, and decided to use Vit E cream as a night cream. I've been using it for six days now, and I do feel it helps in the healing process and on scar reduction.

One reason why docs are sceptical abt the effect of Vit E on scar healing is that a previous clinical study had not shown any positive result. However, it did seems that the clinical trial itself was flawed.
Anyway, I gave it a try and it seems to work for me.
please clue me in - why is this? It has been one month since my first fraxel. I only had my cheeks done but may have my whole face done in the future. I don't have a clue as to what strength was used or how many passes. I left these details up to my doctor who I have alot of trust in. My skin has improved greatly in this one month alone. I was so scared to have my first one done but now I am glad I did. I have raised scars from acne that have become more visible over time. They are softer and fewer now. My skin tone has also greatly improved. The girls in the office think I will only need two or three treatments. I will continue with more treatments over the next 6 months or so. For just a little more money per month, two or three Fraxel treatments are now included as part of a monthly club plan that I pay into so it is quite affordable. I am still a little red on one side after one month but I am sure I am the only one aware of it. I was very red and swollen for 3 days right after treatment and so I laid low.
i think raised scars may improve more quickly than others...i had one raised scar on my face that went away completely with one treatment. the other scars have taken longer to show improvement.
Basically, you cannot reduce scaring without going deep.
I had PSR3 done with a double pass at the highest energy setting 4 Joules.
My face peeled at day 4. Positive Burning sensations lasted 12 hours.
It works on only small scars ( 30% improvement after 3 weeks ) on my cheeks.
Small scars caused by blackheads taken out. Similar to a needle pricky your skin and leaving small holes. Maybe doing Rhytec 3 times at high energy double pass will completely remove all my small scars
If you want large scars removed, go for CO2 laser because it goes deep and the
heat tightens the skin. PSR3 ( Rhytec ) tightens the skin because it creates heat.
The results I got from PSR3 are only about one third of the results shown on the website of rhytec. ( So you would need to do it about 3 times to get those results )
Next time I will ask my doctor to do 5 passes at high energy to achive better results.
Doing Rhytec at energies less than 4 Joules or less then 2 passes will produce poor results.
People with darker skin tones are at risk of permanent hypopigmentation. (lighter skin)
ie. loss of pigment.
I used zinc oxide cream to accelerate the improvement. ( Diaper rash cream )
This cream can cause some irritation, acne and temporary hyperpigmentation (darker skin )
I would not recommend Erbium laser because it is too weak and does not produce the
heat to tighten the skin.
I would not recommend Fraxel because it is just that, Fractional Laser, affecting
only 20% of the surface each time, meaning fractional results.
I had 10 Smoothbeam treatments done at setting 14 (highest setting ). Each treatment improves the small scars by about 1-2%. This treatment is weak.
I would not do micropeels, microdermabrasion as it does not go deep enough to produce
a result.
Dermabrasion seems like a crude method that I would want to stay away from.
Phenol peels are the only peels that go deep enough to work except that the doctor
has not as much control over how deep it goes, so there is a greater risk of
hypopigmentation and scaring.
Vbeam is very good for removing redness and rosacea, as I had it done to my cheeks.
( 90% improvement - 3 treatments ) Biggest improvement after 1st treatment.
Hope this information helps.
In summary, there is only Rhytec for Light scars and the riskier CO2 for deeper scars (more risk, more results, redness lasts a lot longer ).
Risks of CO2 laser - scaring, hypopigmentation and Demarcation.
Even more Riskier methods include Phenol Peels and Dermabrasion
I actually read the study mentioned above on Fraxel and red marks.
It's a bunch of nonsense.
The study consisted of TWO patients (yes, TWO patients) and both did have some improvement in red marks, but not what you'd expect for 5-6 treatments, which would prob cost $5-$6K. You could honestly get the same amount of improvement or more, and in less time, from doing some superficial chemical peels and using Retin-A and/or Tazorac.
I don't think the results on that website are the norm. I have had three fraxel treatments and almost no visible improvements on my scars. Just had dermaroller and hoping this will do something.

My best improvement came after the fifth session, so I guess sometimes u just have to be patient. Another thing worth noting is that i paid more attention to post treatment care after my fifth session. For example, i applied Vit E cream on my face every night after the fifth session, and it has helped the healing process.

this is a pointless post but i'm glad you commented on this. i've been told yet again to try fraxel by a derm and am only now beginning to sliiightly consider it- but i was told flat out by the dr's assistant that there is really no notable improvement until the fourth+ session.

With the higher levels everyone has been getting:
You have to remember that as the intensity goes up the coverage goes down. It's an inverse relationship.
Therefore acne scars treated at highest levels need many more treatments than 5-6 to get near 100 percent coverage. And without 100 percent coverage you cant expect to have a fabulous improvement, because portions of every scar won't have received any laser at all.

I have a series of six so hopefully I will see some good results and I was happy to see the derm started me at high settings so I will get my money's worth. Also my downtime was minimal despite the high settings which was another plus.

Just had dermaroller and hoping this will do something.After 3 sessions, I didn't notice a marked difference either. After 6 sessions, I've had very significant improvement.
this is a pointless post but i'm glad you commented on this. i've been told yet again to try fraxel by a derm and am only now beginning to sliiightly consider it- but i was told flat out by the dr's assistant that there is really no notable improvement until the fourth+ session.
Yes, this is absolutely true. I've done 4 and can see slight imrpovement, but nothing to jump up and down about yet.
