Hey guys. I just found some pictures I've taken awhile ago before I had done 100% TCA cross that totally ruined my skin. It made all of the scars I treated 10x worse. The chicken pox scar wasn't there before (so depress over that). I realized that it looks so much better than what it is now (with excision scars). I really regret all of the stupid choices/mistakes i'd made. I should of went with Fraxel treatment so long ago.

One week since 4th treatment, Fraxel 2.
Still some minor swelling. Still some redness.
I now have a breakout around my chin. I think it was the sunscreen - I used it twice on Saturday because I had yardwork to do. By Sunday morning, the breakout had started.
I don't think I can comment yet on how much improvement I'm seeing because I know there is residual swelling.
If only my skin would stay plumped up to this level - it's perfect because the scars are so dramatically reduced.

I just had my 2nd treatment 3 days ago. I can say treatment #1(20%): not much improvement. Treatment#2(40%): I'm still at a healing stage. And I been told that you don't really see a difference until your 3rd treatment (60%). Each fraxel treatment covers 15%-20% of the scarred area.
So far Fraxel hasn't done anything bad to my skin.
Is it more beneficial to take antibioic before treatment or after. My derm is having me on Tetracycline 30 days after every treatment.
I am also Asian and he has not discussed any hyper/opigment treatment specific for me. What questions should I ask to make sure I'm not affected by this?
Also, he put me on Finacea cream indefinately. What are the beneifts of this? Is anyone else on this?

Has anyone else had a MASSIVE breakout from Fraxel? I had at least 15 deep acne lesions several days after Fraxel, around my mouth and nose only. Don't know if the Fraxel gets to bacteria under the skin and sends it upward and outward or if it could have come from the slather of products I was on. I am not looking forward to this happening again. I am just now getting it under control. Switched to Cetaphil wash and moisturizer...the product I was on from the doc was full of copper peptides. I thought copper was good for the skin. Any info on this? Thanks.
BTW, I am waiting to post pics until all of the acne is gone because the inflammation from so many makes the scars worse as you all know.
Breakouts from Fraxel are pretty common and are caused by follicular keratinization (dead skin cells) coupled with an increase in oil production. (Your body relies on oil glands for reepithelization which can tend to put your oil glands in hyperdrive).
The dead skin cells are due to the MTZ (ablation points) from Fraxel leaving dead skin cells trapped in the follicles which is the basic recipe for acne. These problems usually resolve post treatment with time. If it becomes a real problem, there are ways to prevent this from happening prior to receiving treatments with Fraxel.

In your particular situation with cystic acne, topical treatments are of very limited benefit as cystic acne tends to encapsulate deep within the follicle which is far beyond the reach of topical medications.
Once cystic acne has already developed, nothing can really be done about the actice cysts except gently applying ice directly on the cysts for 3 minutes 2x daily to reduce inflammation and prevent the follicular wall from rupturing which can lead to further scarring. When done properly, the inflammation will greatly reduce over the course of 1-2 days. To apply properly you must place the ice directly over the effected area until it is numb usually three minutes. This will also help reduce the inflammation from your Fraxel treatments.
You can also prevent future breakouts by asking your P/S to prescribe oral antibiotics preferably in the tetracycline family such as minocycline at least 1 month prior to Fraxel treatment. This works as a prophylactic by killing the P acnes before the cyst develops and can encapsulate.
If you elect to go this route, it is very important that you take the antibiotics for the entire prescribed course. DO NOT discontinue treatment prematurely or you run the risk of developing P acnes bacterial resistance. This is very bad thing and would render future treatments with the same class of drugs useless if it happens. Drugs like Minocycline are class D and should not be used if you are or are planning on becoming pregnant.

The prescribed course is usually 2-4 weeks beyond your last Fraxel treatment. There are many factors involved in determining the correct dose such as body weight, current medications you are taking and health history so I will defer you to your P/S with determining your correct dosage. The usual starting dose with Minocycline is 50 to 100 mg twice a day.

In my opinion your scars are minimal. The excision scar could possibly benefit from fillers or for a more permanent approach, revision and subsequent resurfacing to feather in the edges of the scar after the tissue has been rejoined. Excessive atrophy from a surgical scar usually occurs when the scar widens. This sometimes happens after a P/S fails to place the scar within the RSTL's (Relaxed Skin Tension Lines) of the face and the scar stretches excessively from normal facial expressions. There is so much more then meets the eye to performing excisions properly and so many stories on this forum about failed excision attempts. I will offer a few suggestion for avoiding a poor outcome.
In general, an experienced, board certified, plastic surgeon is an expert in scar revision. For example an experienced plastic surgeon will take into account the scar size, RSTL's of the face as well as modifying techniques to change the direction or orientation of the scar such as W or Z plasty ect.. This can make all the difference in the outcome. I have personally seen many otherwise excellent dermatologists that take the correct surgical approach to scar revision but not necessarily the best cosmetic approach.
This is not to take away any merit from dermatologists. Some are very skilled in proper cosmetic surgical techniques but they are much harder to find then a competent plastic surgeon in this area. To others that would argue the merits of seeking a local dermatologist for scar revision I would ask:
How many dermatologists have you met skilled enough in cosmetic surgery to perform a SMAS facelift/neck lift with undetectable results? How about cleft lip/palate repair? These two procedures leave extensive scaring on the face but in the skilled hands of a plastic surgeon the results are often nearly undetectable.
Perhaps this isn't a fair comparison as a general dermatologist lacks proper training in these specific areas of focus but for the purpose of comparing scar revision techniques. Who do you think has the training necessary to deliver a better outcome?
hey troubled skin just wondering since you know alot about scar treatments do you know if the results of fraxel are permanent like if someone hets fraxel done and has great results but once they get older and lose collagen will the rolling scars become apparent once more
Fraxel induces your body's own collagen so there would be no reabsorption as with fillers. You would likely need maintenance treatments as you get older and continue to lose collagen.

That is my plan as well. So I too am looking into the best fillers. I heard there is something new coming out soon.
"Troubled skin" I'm planning on doing fraxel for my mild acne scarring. In New Zealand there is only two places that have the fraxel laser and i'm assuming it's fraxel 1. Being asian (chinese) male I plan on only doing my cheeks as it's the only scarred place you think I should do a full face or only the cheeks as i'm afraid that the tone of my skin maybe different if I only do the cheeks. Also since my scarring is mild you think 12-14mj can still stimulate collagen. thx in advance
Also i'm not sure how many patients this clinic has treated with fraxel you think it's necessary to find an experienced person performing this procedure or will one that may have performed it 10 or less times be alright as the population in my country is 4 million and only 2 clinics have fraxel

Hello. I'm going to agree with you on this one because I had 5 excisions done and 2 of them went bad. I wished that someone would have told me about this before. The reason I agreed on getting excision was because during my consulation I was told that fraxel would level those surgical scars. I'm really hoping that its true. What type of fillers do you suggest? And what are other alternatives out there? Should I see a P/S regarding that particular scar? I have to admit that the chicken pox scar is bothering me the most. It's just too deep and not even makeup could cover it up. I think I should be patient and wait to talk to my doc. (or P/S) after the 3rd treatment regarding other options. What do you think??

Face less swollen (almost normal). Skin is tight and turning brownish color but not peeling yet. Pain stopped completely today. I'm applying MD Forte moistuerizer right now.

Thanks for the info, I got my script today for minocycline 100 mgs 2 times per day. I STILL have residuals from the Fraxel and red marks now. It really sucks. Scars are bad enough, I certainly don't need giant pimples to complete the package and possibly result in more scarring. As usual, I appreciate the advice. Don't know where I would be without you guys to help out with the info.
BTW, I will commit to saying I see a definate improvement after my Fraxel treatment. My husband and other family members have commented on it also. The scarring is certainly not gone by any means, but it is softer. My cheeks are the most improved and around my mouth the least. I am hopeful that with 3 more treatments I will be happy with the results. Hope to be able to post some pics soon...as soon as more of this redness is gone I guess. I don't want to influence the supposed results with coloring difference, but on the other hand I am not sure the redness will be gone any time soon.
i read that some ppl's doctors said that they find a better improvement when they do lower settings but more passes. If you were to get fraxel treatments this way would you not get red and swollen?
from my experience, yes
i'm having an area treated at lower settings purely for pigmentation and this area gets far les red, swollen and flaky than elsewhere.

Silicone Microdroplet has been used in some cases with good results. The best technique is through under correction spread out over the course of several visits as there is a tissue adjustment phase after each injection (As you know there is really no margin for error). In the hands of the right Dr. you can expect an excellent outcome though. I can provide you with a referral to an excellent Dr. that has done this for many years if you should choose to go this route.
Hyaluronic Acid based fillers i.e Restylane, Perlane ect. are much safer however and have more predictable outcomes. True, repeated injections are necessary but in some patients the rate of absorption begins to slow after a few injections.
If it were me I would probably take the conservative approach and wait for some of the more promising semi/permanent fillers in the works to arrive in the US.

Great advice. I had restylane in scars once. It worked great, but was temporary and expensive. I tried punch floats on the bad scar, which failed. I am convinced filler is right way to go. I am probably going to have another spinal surgery this winter, so I think I will put off next round of fraxels until I survive that. But if all goes well, I will recover from surgery, then fraxels and filler.
Hopefully the timing will be right for these new semi permanent fillers.
Whom do you recommend in SF Bay Area for fillers?

I had my very first fraxel today. SO they first clean your face with a cotton ball drenched in alcohol..i guess the skin needs to be completly free of oils so that the blue dye and the numbing cream will stay in place. Otherwise it will run all down your neck..and trust me it numbs everything... i came home from my procedure about 6 hours ago and the inside of my nose and my neck are still numb... ANYWAYS.... I left the office out the back door because i was too embarressed once i seen my face in the mirror.. i was SO blue! So anyways... I sat with the numbing cream and blue dye on for about 45 minutes...and then the lady came and and said we will begin now... she put some cloth over my eyes and then some metal goggles for protection... she started on my forhead and and she said was going to go all over my face with the laser pressing vertically and then go over my whole entire face again going horizonitally....WELL that had to have been the WORST pain i have EVER had... When the doctor finished the vertical way, she said now the horizontal way is much more painful.. i felt like dying... it really felt like someone was taking a HOT HOT knife and pulling it over my skin... i could feel every little prickle inside my skin... it was hot instantly... they had me to hold onto a piece of gauze just incase the numbing cream got into my mouth... but by the end of the preocedure it was soaking wet from my sweat.... I have one more treatment and i am terrifed for it...I went for my full face treatment today but next time its just my cheeks and chin..OH and where it hurts the most are the upper cheeks and the chin and that skin between the nose and the lips...Oh myy i almost lost it... but anyways... i am home now... there is not much swelling...just around my eyes... kinda looks funny... they are soo white compared to my face...i think its going to look good afterwards.. the laser already smoothed out some scars that i have had for years..so i think i am going to be happy...give it a week so that the swelling goes down i guess... i guess its worth it... lets hope so!!!!!!!!

How many passes did you get? Didn't they use cooling air on you?

Yeah I think fraxel will help. Your scars are very small size but ask your doc. if excision or a scar revision is needed first.
