My guess would be with age, but who knows. I only say that because even his forehead lost texture and had the same discoloration that aging is known to do. Could've been both but looks like aging to me predominantly.

The areas around his mouth in this picture look odd and kind of like maybe he had fillers. Whatever he had going on he never looked healthy aside from in his 30s and earlier (goodfellas days). Dying at 67 is 20 years earlier than expected. Must have had health issues.
