I have mostly deep rolling atrophic (fat loss) scars on both cheeks. No icepick, some boxcars. I have 3 options:

1. Subcision with fat grafting without stem cells + RF microneedling.

2. Subcision with Sculptra + CO2.

3. Subcision + Ellanse filler.

Which treatments is suited best for deep rolling scars with fat loss and has the most improvement?

yes, and i would recommend subcision
but i would think a filler would help from re-tethering (just my theory, no experience on it)

It really depends on the scars and your own personal situation but all 3 are viable options. The fat grafting is something I looked at and just be aware it could "shift" within your face, although I really can't get a clear answer on how good it is because 1 doctor said not worth it, another said totally worth it. It might be person specific and scar/area specific.

You might want to try to see if you respond to filler + something first before fat grafting because that's quite permanent. Why not try like subcision and sculptra or another temp filler see how it goes.
