Hello,
I have some pretty bad acne scarring mostly rolling on both cheekswith volume loss. Firstly i know i would need subcision + filler to fill the loss of volume i have. I have 2 options:
1. To visit dermatologist which will do subcision on me + fill the scars with Ellanse (which is simillar to Sculptra).
2. To visit plastic surgeon which will do subcision on me + fill the scars with fat grafting - lipofilling (he said 2-3 procedures would be needed).
Which option is better with filler or with fat grafting as a filler?
Im located in the EU so my English is not that great.
Sounds similar to my scarring. Widespread volume loss and some rolling scars. Honestly subcision and filler made my scars worse. The subcision seemed to have caused more atrophy and the filler that was placed reacted with my skin which cause nodules and once the nodules went away I had crater scars left behind. Everyone is so quick to say subcision like it its 100% fool proof, I think you should approach with caution and start with something less risky like microneedling. I know people talk down about it but microneedling is probably the most used scar treatment around and all I usually hear people say is it had little to no improvement and honestly I would much rather be saying that right now instead of having done something that made my face worse than it already was.

Microneedling really is almost useless in my opinion but obviously no result is better than poor result.
May I ask in what way did the subcision worse? Did you feel it took out more volume or did damage to the dermis, so basically when the filler got reabsorbed it was worse? May I ask what type of scar, was it rolling, surgical scar, boxcar, oval?
Also for op, I think a lot of the more permanent fillers like fat grafting require talent and just happenstance. Meaning I've read that sometimes it's done poorly, is reabsorbed very fast (maybe with weight loss), other times it's very effective or at lest helpful. I can't figure fat filler out to be honest. Some plastic surgeons say no, others say yes. In the end, if there were fillers that lasted 2-3 years at a time I'd rather do that, but juvederm is just insanely short/useless.

Yeah Ive heard that from some people and also heard that its helped others but I rarely hear it made something worse. Yes you hear that from some people but considering the amount of people that get microneedling done I think the chance it turns out bad is less likely. The subcision created more atrophy and the fillers created boxcar type of scars. Im not sure what do do now.

Maybe bead reaction to the filler?
In my case micro microneedling wont help because my scars are missing alot of volume so the fillers or fat are necessary

I did it end of 2020 with Rullan. Yeah I also think I need fillers or fat but Im not sure if I will continue.

That is really not only unfortunate but surprising. I don't really remember reading any/many information about subcision or filler causing either scarring or atrophy. I'm sure it's possible but do you think it was the doctor's negligence, lack of experience or just your own personal anatomy/situation that raised the risks?
Either way, I hope you find something useful.

The filler caused scarring because of the reaction. I got nodules from the filler. And the subcision Im not sure if it caused atrophy but I have clear pictures under same lighting that you can clearer see more scars and atrophy. I think from what Ive read the best thing I can do for scars is sculptra or a filler like volume because I do have A LOT of volume loss.
