I have had this scar for about 1.5 years now. It was left behind by a very, very large cyst that lasted over a month. Not sure if the picture shows it, but the depressed area is about the size of an almond. I feel like it has changed the character of my face.. It looks like its a misplaced 'smile line'. I went to a derm, who said that microneedling/subcision would not help this scar, because it is caused by the loss of fat, and the only thing that would work would be fillers (needing continuous maintenance) or a pricey fat graft. The derm had me move my face around and concluded that the scar didn't look 'tethered down' so subcision wouldn't be the solution.  

I have had one session of microneedling, and planning on getting more.

What do you think?

The scar does look tethered IMO. Subcision with either a fat graft or Sculptra will help. Sculptra will build collagen over time and help plump up the area.
