Background

Hello I have been reading several posts on this forum for awhile and following peoples scar journeys. I am really thankful for the people who dedicate their time and knowledge to help others who are struggling with scars. I do not have acne scars but scars from a mole removal gone wrong. I could not find much help elsewhere so I thought I would post here for some help. I had 3 moles removed from my face 10 months ago. 2 from my cheek and 1 one from my chin. The mole on my chin left a bad scar and it is what bothers me the most. The scar on my chin had a stitch abscess and formed a blister which I mistakenly popped while cleaning my face. The other factor which made the scar worse was that it was on my chin in a area of high tension. Every time I smiled the scar would stretch.

Treatment options offered to me so far

The original plastic surgeon who did the mole removal suggests letting him do a surgical scar revision. He says knowing what he knows now, he will use different technique with dissolvable stitches to prevent stitch abscess and tension in the area. I am scared to go through with that as it has alot of down time and the risk I could end up with an even larger indented scar.  It's also the only scar treatment he offers so I wanted to get a second opinion of someone who offers a variety of treatments. I did a video consultation with a Facial Plastic Surgeon yesterday and asked about subcision but he says that it is only effective for acne scars and even then you may only get a 10% improvement. He suggested letting him do Bellafill. He said we could start with a non permanent filler and see if I like the results before moving forward with bellafill. All of the reviews claim he is a highly skilled injector. A med spa suggested 3-5 rounds of Fraxel laser for a 50% improvement.

What do you guys think? Am I a good candidate for Bellafill? Should I let my original plastic surgeon redo the scar surgically? Should I try 3-5 rounds of fraxel laser? What is the best treatment for my scar type? Please help me :) Thank you :) Edit: uploaded more photos in my comment below.

photo quality is extremely poor, did you these photos with your toaster?

I would hazard a guess, although i can't see for shit, that you have few scars but they are large and deep. in this case, subcision and filler would give you best results fastest. not bellafill alone.

bellafil is permanent, so if they screw it up they can't dissolve it, you can get HA filler instead. def subcision though, maybe also RFMN on top for more collagen boost.

I think the photo quality is bad because some are zoomed in. here are some more photos.

My scars improve when I stretch them with my fingers. I don't think they are tethered. I think they are tension scars or "stretch scars". I am having a consult with a plastic surgeon on monday who does a variety of treatments including subcision on monday. I will see if he thinks I am a good candidate for subcision. I am starting to think I should just let my original plastic surgeon redo the scar with surgical revision. I don't know though I am scared

The larger scar is the better one to be re-excised because it's larger. Maybe you can see how that goes before working on the other two.

Consultation costs add up but you can consider seeking another surgeon's opinion and asking him/herwhat his/her stitching technique would be. See if it's the same or different than your initial doctor's.

I've heard some people say that their doctor's injected them with Botox to paralyze the area so that it doesn't have the too much tension on either side.

I don't think Fraxel is good bang for your buck. It's great for that med spa to make thousands off of you though. My opinion (I'm NOT a doctor) is that re-excising it (done by a good plastic surgeon) will result in a better scar. It will also save you money from the laser treatments and you won't experience any laser side effects.

Are you willing to travel to see a different doctor?

I think you are right. I should probably just get the scar redone surgically. Do you know any good doctors in the dallas fort worth area? If I were to chose not to go the surgical route? What would be my next best option?

He is the only one I know in that area but I don't know if he's the best. Hope this helps you!

Thank you so much. I will give him a call.

What is your opinion on Bellafill? Do you think I am a good candidate and would my chances of complications be very low considering I am only filling in 1 scar on my chin?

I had that injected before for scars smaller than yours and I'm not sure if it did much. I have mostly boxcar scars. I don't think you'll be too satisfied with the result because your scar is much bigger and has harsh borders.

It's semi-permanent so you want to make sure you really want it. Some say to get a non-permanent filler to see if you like the results first. Another thing you must consider is that it can't be dissolved like most other fillers. It needs to be surgically removed or injected with steroids - but some microspheres will most likely still remain present. Also, doctors require that you get a test shot to see if you have any bad reactions to it. Almost everyone is a good candidate.

Bellafill is $1k+ so it's not cheap. Could you use that towards another treatment? Tough decision.

You can always DM me if you need further help! :)

Was the mole larger on your chin than on your cheek? I had two small moles that were close together on my cheek that were removed as one. So a linear stitch was sewn. To this day, decades later, there is a slight linearindent that has made me regret at times of having the moles removed. Regret because of the indent and elasticity of my skin in that area. Your original doctor seems to be inexperienced imo. The only difference in using dissolvablestitches or not is the length of time they are in place. A dissolvablestitch can take a month to dissolve or longer. A nondissolvable stitch could be taken off in a short time like five days but might not be advisable for an area like the chin. Lastly, I think a lot depends on how we each heal. I have seen those with a cleft lip that had surgery heal with just a hairline flat scar to those that had a raised or indented wider scar. Find a doctor that is very experienced. They should have done hundreds of procedures that you had.
