Enlarged pores + ice pick scars on nose and raised scars on neckline, help please!

Hi guys,

I wanted to ask if you guys can help me on enlarged pores and ice pick scars on my nose and raised bumps or scars on the neckline.

First, I didn't have enlarged pores or ice pick scarsa year before,

but I've got a lot of them on my nose since last summer so I've been taking Accutane.

ButI really wanna minimize enlarged pores and ice pick on my nose and I can feel it's gonna be so hard.

I got 2 hypertrophic scars on my nose removed by CO2 + steroidinjection as you can see the 2red spots.

I guess they'returned into rollingscarsnow(I'm done with 7 sessions of TCA spot peel and XCAR subcision for those scars).

What treatment is suggested for scars on my nose?

Do you guys recommend few sessions of TCA Cross 70~80% + Subcision + Needle RF and then ablative/fractional laser?

 

My second concern is, what I believe is, raised scars or maybe some elastolysis on my neckline.

As you can see, it's way more obvious when I move my head down than I'm just sitting as usual.

I'm still confused about what it is actually. Some derm says it's kind of white heads so I need to get Needle RF with peeling

and others say it's hypertrophic scars and it should be treated by CO2 + steroid injection.

Can you guys tell me if anyone knows what it's called? And what kind of treatment would work on these scars?

David Lim uses TCA cross for icepicks even for the nose. However, the nose is also the most unpredictable when it comes to results following an invasive treatment. If you can't get hold of a plastic surgeon or dermatologist who is experienced in treating nose scars (and successful at it!) then, I suggest you will have to wait until you do or do at-home treatments and gamble. Some people in this forum did single needling on their scars, but they never really followed through to report back. This was years ago. Stay away from lasers and acid peels for the nose cause those just make more scarred pores and enlarged pores.

If you want a low risk alternative while you can maybe try retin-a. If you want to be even cautious then start with a low dosage of retin-a and slowly build up. Keep careful watch of it and stop if it's just getting worse. NEVER OVER PEEL YOUR NOSE. I had to learn the hard way. I have enlarged pores and icepick scars as well as hypertrophic scars on my nose.

Also, most people wont be helpful on this forum since the people who have scars on their nose are veeeeryyy few. Additionally, not a lot of people know what to do with it. I'm sorry but I've also been looking for answers back in 2014 here. I kinda gave up on my nose scars and accepted it.

@AwareFellowSadly if you want Dr Lim you travel to Dr Lim, few will do treatments the same way he does, he takes risks.

The nose is a terrible place to heal. I answer several of these a week in your skin type. Large pores, sebum, scarring, cartilage not collagen(skin), the nose does not heal well, it's variable.

You can do tca cross with a experienced practitioner, if they are not then you could make the scars worse. Please note the scars will get wider and the floor"can raise", you resurface after several treatments. Ask your practitioner for pictures of this procedure specifically on other's they have done, they should treat your skin type often.

You can at home get a dermastamp and stamp the scar walls at home. Some do peels like salicylic or mandelic to clean out the pores

A dr would do several tca cross, then he would co2 laser and finally resurface with a peel or ablative laser. Please note we all have pores on our nose so it will never be smooth just softened. Another alternative is electrosurgery where they scrape the walls of the scar, and finally excision but that must be done through a very highly experienced plastic surgeon (surgery always has risks). Consult a few Dr's and see what your options are locally.

 

Regarding your other scars you have pebblestone hypertrophic scarring and a red hyperpigmented area. On the red PIE or hyperpigmented areas I would use pulse dye laser.

For the pebble stones again inject with steriod let them flatten, then co2 laser or laser resurfacing.

Honestly, I'm a little disappointedthat scars on the nose are terrible to heal or recover.

But I'll try every option that I have now.

 

What treatment would be goodfor the 2 red hyperpigmented atrophic scars on my nose?

(They werekind of hypertrophic scars in the first place but I removed them by CO2 laser)

 

And I think punch excision is one of best options for theice pick scars on my nose.

What laser do you recommend for me touse after the punch excision surgery to make flat them?

@AwareFellowItdepends,... regarding the nose, from your response it seems your biggest issue is your PIE or redness, get pulse dye laser done, tell them to be aggressive. That is the biggest thing that shows up on your nose right now, the redness.

Makes since about the scars on the nose being red, they were lasered, and now wounded needing healing.

CO2 laser is used often after punch excision
